Financial Assistance for Basic Telephone Service

Rates, Rules and Regulations A copy of the Copper Valley Telecom tariff is available for public inspection at the Valdez and Glennallen business office and at www.cvtc.org . Incorrect Directory Listings The vast majority of directory listings are correct, but, as with any job of this size, some errors are bound to occur. If your listing in the directory is incorrect, it cannot be changed until the next edition of the directory is issued. In the mean time, we can make sure the listing is correct in Directory Assistance records. Please call your business office to report any errors in your listing. Your service representative will be happy to help. The Company’s liability for damages arising from errors or omissions in listings is limited in accor dance with its tariffs. Your Number is Important When preceded by your area code, there is no other telephone number exactly like yours in the U.S. or Canada. Make it easy for people to reach you by displaying your area code and telephone number on all stationery, statements and advertising pieces.

Struggling to make ends meet? Consider apply ing for federal Lifeline & Linkup telephone assis tance through Copper Valley Telecom. The application is easy to complete and discounts go into effect almost immediately. The Lifeline and Linkup Programs provide local or wireless telephone service at reduced cost to qualifying low income customers. To qualify, customers must meet the following criteria: Certify that you are not receiving Lifeline dis counts on any other telephone lines (even with another provider). Meet income-based criteria as shown on the Lifeline Program application or certify that you subscribe to one of the following needs-based assistance programs: • Supplemental Nutrition Assistance Program (SNAP), formerly known as Food Stamps • Medicaid • Supplemental Security Income (SSI) • Federal Public Housing Assistance (FPHA) or Section 8 • Veterans Pension and Survivors Benefit • Tribal Programs (and live on federally- recognized Tribal lands) • Bureau of Indian Affairs General Assistance • Head Start (only households meeting the income qualifying standard) • Tribal Temporary Assistance for Needy Families (Tribal TANF) • Food Distribution Program on Indian Reservations For more information, please call the CVT business office. How Do I Go About Disconnecting My Phone Service? Contact our Valdez or Glennallen business office in advance of your disconnect date and request a disconnect order to be entered. Be prepared to provide forwarding information.

Number Changes CVT will make every effort to avoid changes of a cus tomer’s telephone number, but reserves the right, when ever it becomes necessary in the conduct of business, to change the customer’s telephone number or central office designation.
