Before You Call

If the problem does not occur... the problem is in your wiring. Call your local tele phone company or another company to repair your inside wiring. Charges may apply. If the problem is in your telephone set... follow the instructions on your warranty or contact your equipment retailer for repair or replacement. If all the phone accessories are working, go to your network interface box (outside, where the phone line enters the house). Copper Installation 4

After Hours Trouble Reporting To report a trouble and receive support after hours, call 1.800.235.5414 or visit www.cvtc.org to use our live chat support. 1

Hang up all telephones. Make sure phone accessories – answering or fax machines, computers, modems, securi ty systems, and telephone sets – are working. Unplug the phone cord from the unit or the jack and check for damage. Leave everything unplugged for 5 minutes. Then try plugging in the phone sets one at a time to see if the trouble still exists. 2 Test to determine if the problem is inside or outside the house: 1. Take a working phone to the net work interface box (single line phone only). 2. Remove the modular plug from the test jack and after waiting 1 minute with the jack empty, insert the modular plug from your phone. 3

CUSTOMER PROPRIETARY NETWORK INFORMATION

Copper Valley Telephone Cooperative and its subsidiaries, including Copper Valley Long Distance & Copper Valley Wireless, follow the Federal Communications Commission (FCC) rules for protecting customer information. Based on the Customer Proprietary Network Information (CPNI) ruling by the FCC, we implement specific standards to protect customer information. Even though we consider everything about your account and services (except for information found in the public directory) to be private/confidential information – and we do not share it with anyone other than you without your written permission – our process became stricter, effective December 8, 2007. When you contact us directly to receive information about your account, the following safeguards apply: • We ask you for picture identification in person. • We ask you to provide a password if you contact us by phone. • If you prefer not to use a password, we can call you back at the phone number of your account or mail your information to the billing address on your account (address needs to have been in effect for at least 45 days). • We send you a notification when your billing address, password, secret question, or email address has been changed. • We only use information on your account for target marketing within our Copper Valley com panies. You may opt out of these campaigns if you wish. An annual opt-out card is mailed to all customers. You can also contact us at any time to opt out of these campaigns. • We do not share your account information with any 3rd party for marketing purposes. If you choose to use a password, we encourage you to review it and make sure that it meets the following criteria. Passwords need to be at least 8 characters long, including at least 3 numbers. These pages provide answers to some of the questions asked most frequently about our services and billing procedures. For more detailed information, contact our business offices in Valdez and Glennallen.

What Will the Phone Company Need to Know to Set Up a New Account? To help speed along your service request, please have the following information when you call or visit our business office to apply for service: • Complete street address, including apartment number if applicable. • How you would like your name to appear in the directory. For an additional charge, you may order additional listings for other mem bers of your household or business. • Letter showing good credit from past utility (telephone or electric), if available. • Employment status. • Social security number, valid driver’s license or state ID. How Long Will it Take to Get My Phone Service Installed? Most appointments for residential services can be scheduled within five business days after an application is completed, as long as facil ities are available at the requested location. Appointments for business service installations vary in length depending on the complexity of the services being installed. How Am I Billed? The monthly charges for all your services are billed one month in advance. Long distance calls and wireless calls are rated against your calling plan after you make them. Your first bill will be for one full month plus any prorated amount for days in current month. When Will I Receive My Bill? You should receive your bill approximately five days after your bill date.

Can I View My Statement Online? Yes, we have a program referred to as SMARTHUB – that allows you to view your statement online. You will need to call in to your local customer service office in order to activate your account. We highly recommend it! When is My Payment for Services Due? Your payment for billed services is due the last day of the given month and will be considered delinquent if not paid by that date. If your pay ment will not reach us by that date, call the Valdez or Glennallen business office to discuss payment arrangements with a Customer Service Representative. “Failure to receive mail” will not be recognized as a valid excuse for failure to pay bills when due. If your bill is not paid by the last day of the month, it will become delinquent. You will receive a notice of delinquency with a date for temporary disconnection if not paid. If payment is not received by the disconnection date, your service(s) will be disconnected and you will be required to pay any past due amount plus the reconnect charge before they are reconnected. You may also be required to provide additional security deposits. Will I Be Asked to Pay a Deposit? All residential and business customers are asked to pay a deposit for service based on credit history.

If I Pay a Deposit, When Will It Be Returned?

We want to return your deposit as soon as pos sible. After you’ve had service with us twelve months, we’ll review your account and if you’ve been making your payments when due, we’ll refund your deposit.

Financial Assistance for Basic Telephone Service

Rates, Rules and Regulations A copy of the Copper Valley Telecom tariff is available for public inspection at the Valdez and Glennallen business office and at www.cvtc.org . Incorrect Directory Listings The vast majority of directory listings are correct, but, as with any job of this size, some errors are bound to occur. If your listing in the directory is incorrect, it cannot be changed until the next edition of the directory is issued. How Do I Go About Disconnecting My Phone Service? Contact our Valdez or Glennallen business office in advance of your disconnect date and request a disconnect order to be entered. Number Changes CVT will make every effort to avoid changes of a cus tomer’s telephone number, but reserves the right, when ever it becomes necessary in the conduct of business, to change the customer’s telephone number or central office designation.

Non-Published or Non-Public? Federal regulations require CVT to provide cus tomer billing name and address information to telecommunications companies for use in billing you directly for any calls you make using their services. CVT will only provide this information upon the request of the telecommunications provider unless the subscriber to a nonpublished or unlisted telephone number has affirmatively requested that its billing name and address not be disclosed. Upon such request the information will not be provided. If you have a problem with your phone service or billing, call our business office. If you have a tele phone repair problem, call our Repair Service. CVT customer service and technical support personnel are specially trained to solve your telephone problems and to be sensitive to your needs. However, if you’re not satisfied, feel free to ask for a supervisor. If your problem can’t be solved by the supervisor, higher levels of man agement are available to help you. Remember, we’re here to serve you. Tones and Signals The following types of tones are sometimes heard on landline telephones. If you are calling our offices for technical support, our personnel may ask if you hear one or more of these signals. • Dial Tone A continuous hum, the signal to start dialing. • Ringing Signal A burring tone repeated at regular intervals. • Busy Signal A steady buzz-buzz tone. Credit for Telephone Service Outages Upon request of the customer, CVT will allow customers credit in all cases where telephones are “out of service” for a period of two days or more from the time the trouble was reported by the customer, except when the “out of service” Reporting and Solving Telephone Problems

is due to the fault of the customer. No other lia bility shall in any case attach to CVT on account of interruptions of service. Can I Have My Home or Addition Pre-wired for Phone Service? If you are remodeling or have a house, duplex or triplex under construction, we can install telephone wires so that they do not show. This pre-wiring must be done before wall framing is covered with wallboard. Call our business offic es to schedule an appointment for this work. Consumer-Provided Equipment Equipment, apparatus, devices or systems not furnished by Copper Valley Telecom may be used with company facilities and equipment on one party service, provided such use complies with the Copper Valley tariffs. Any item used with the Company’s facilities and equipment in violation of the Company’s tariffs may lead to disconnection from the network of the equip ment, apparatus, devices or systems, or in the suspension of your telephone service. Please contact our business offices for other informa tion concerning these regulations. Telephone Safety The telephone is one of the safest appliances in your home or office. There are, however, a few situations where a telephone user should use caution. • Using the telephone near water – The tele phone should not be used while you are in the bathtub, shower or pool. Immersion of the telephone or handset in water could cause an electrical shock. • Using the telephone during an electrical storm – Avoid using a telephone during an electrical storm in your immediate area; calls of an urgent nature should be brief. Although the system uses protective measures to limit abnormal electrical surges from entering your home, absolute protection is impossible. There is a remote risk of a dangerous elec-
