900 Pay-Per-Call Billing For 900 billing disputes or inquiries, please call our business office. You have 60 days from the date of billing to dispute a 900 billing error. You have the right to withhold payment of the disputed 900 charge during the billing error review. No collection activity for disputed 900 charges will occur while the charges are under investigation. After investigation, if it is determined that the disputed 900 charges are legitimate, the interexchange carrier or the information provider may proceed with outside collec- tions against your account for non-payment of these charges. Your local and long distance service cannot be disconnected for non-payment of 900 charges. Failure to pay legitimate 900 services may result in involuntary blocking of access to 900 ser- vices. Voluntary blocking of access 900 service is available upon request. You should not be billed for pay-per-call services not offered in compliance with Federal laws and regulations. These rights are provided under the Federal Telephone Disclosure and Dispute Resolution Act. Your oral communication of a billing error will be considered sufficient notification of a billing error. Tennessee Do Not Call Registries The Tennessee Do Not Call Registry and the National Do Not Call Registry give residential telephone subscribers the opportunity to restrict unwanted telemarketing calls. There is no charge to register your telephone number. Exemptions to the restriction are businesses with whom you have existing relationships and some non-profit, educational and political organizations. Enrolling in the ‘do not call’ registries is limited to residential telephone num- bers; business numbers may not enroll. To register with the Tennessee Do Not Call Registry online, go to the TPUC web site at www.tn.gov/tpuc. You may also enroll by calling toll-free 1-877-872-7030 from the number you wish to register. Telemarketers are prohibited from calling your telephone num- ber 60 days after you enroll. To register with the National Do Not Call Registry by telephone, call 1-888-382-1222. TTY (hearing impaired), call 1-866-290- 4236. You must call from the telephone number you wish to register. You may also register online at www.donotcall. gov. Telemarketers will have up to 3 months to remove your number from their calling lists. Your number will remain on the registry for five years, at which time you may re-enter your number on the list. You may remove your name from the list at any time by calling the telephone numbers listed above.

This program, implemented by the Tennessee Regulatory Authority, will distribute appropriate telecommunications devices so that persons who are deaf, deaf and blind, severely hard of hearing, severely hard of hearing and vision impaired or severely speech impaired may effectively use basic telephone service. Devices are issued on a first-come, first-served basis, without charge. However, there are certain qualifiers that may enable individuals to receive devices on a priority basis. Information on distribution, eligibili- ty requirements and qualifiers and an application may be found on the Tennessee Public Utility Commission web- site at www.tn.gov/tpuc or by calling the TPUC at 1-800-342-8359, extension 179 (V) or 1-888-276-0677 (615-741-2904) (V/TTY). Federal Government Required Notice to All Customers of Ben Lomand Connect Ben Lomand Connect is designated the “Eligible Telecom- munications Carrier” for its service areas for universal service purposes. The goal of universal services is to provide all cit- izens access to essential telecommunications services. This telephone service includes access to: • Single Party Touch Tone Residential and Business Service • Long Distance Carriers • Emergency Services • Operator Services • Directory Assistance • Telecommunications Relay Services • Other Services Designed for Persons With Disabilities • Toll Blocking Use of these services results in added charges in most cases. In addition, Ben Lomand Connect provides one copy of its annual local directory without charge. Ben Lomand Connect offers qualified customers Lifeline Service. If you are a low income customer you may qualify for the Lifeline program. This means you may receive a monthly discount for your basic phone charges. This service also includes a discount for line connection charges and free toll blocking for qualifying customers.
