STATEMENT OF NONDISCRIMINATION OTZ Telephone Cooperative, Inc. is the recipient of Federal financial assistance from the Rural Utilities Service (RUS), an agency of the U.S. Department of Agriculture, and is subject to the provisions of Title VI of the Civil Rights Act of 1964, as amended, Section 504 of the Rehabilitation Act of 1973, as amended, the Age Discrimination Act of 1975, as amended, and the rules and regulations of the U.S. Department of Agriculture which provide that no person in the United States on the basis of race, color, national origin, age or handicap shall be excluded from participation in, admission or access to, denied the benefits of, or otherwise be subjected to discrimination under any of this organization’s programs or activities. The person responsible for coordinating this organization’s nondiscrimination compliance efforts is Kelly Williams, CEO. Any individual, or specific class of individuals, who feels that this organization has subjected them to discrimination may obtain further information about the statutes and regulations listed above from and/ or file a written complaint with this organization; or the Secretary, U.S. Department of Agriculture, Washington, D.C. 20250, or the Administrator, Rural Utilities Service, Washington, D.C. 20250. Complaints must be filed within 180 days after the alleged discrimination. Confidentiality will be maintained to the extent possible. REQUEST FOR SERVICE - Make any request for service by dialing the Customer Service number listed above. An applicant for service must pay all previous indebtedness to the Telephone Company before service will be connected in accordance with the tariff. PAYING YOUR BILL - Disputed toll calls can be returned with prior assistance from our Customer Service Department. All undisputed charges must be paid by the delinquent date to maintain good credit. Your bill is due and payable upon receipt. Your account will be considered past due if full payment is not received by the 15th of the month. Accounts 30 days overdue will be placed on toll restriction without further notice. Accounts 45 days overdue will be disconnected without further notice. Customers must call to make arrangements prior to 30 or 45 day deadline past-due dates. We accept Mastercard and Visa. When service has been disconnected for non payment it will not be restored until a deposit is paid on file. All reconnect charges will be placed on the customer’s bill. DOING BUSINESS WITH US...
