Your Rights And Responsibilities
Be careful who you let use your phone. If telephone service is in your name, you are responsible for all calls made from your phone, including long distance calls, regard less of who places the call. Your telephone may be used for legal purposes only and may not be used in any way that interferes with the telephone service of others. Equipment connected to the Telephone Company’s lines must be in compliance with our tariffs and Federal Communications Commission regulations. Interruption To Service When requested by the customer, if service is interrupted for more than twenty-four (24) hours, and for causes other than the negligence or willful act of the customer, an allowance shall be made for the minimum rate for the telephone facilities and class of service affected by the interruption, for such time as the interruption continues. No other liability shall be attached to the Telephone Company for such service interruptions. Business And Residence Service Determination as to whether a customer service should be classified as business or residence will be based upon the character of use, regardless of the type of premises.

Customer Complaints And Disputes
A complaint or dispute regarding service or billing which cannot be satisfactorily resolved within a reasonable amount of time may be submitted by the complainant to the CEO. The Utility may require that the complaint be submitted in written form. All written complaints shall be sent to the CEO of the Telephone Company. If the complainant is still not satisfied with the disposition of his complaint, he may then (per 3 A.A.C. 48. 120, Alaska Administrative Code) file the complaint with the Alaska Public Utilities Commission.
Statement Of Billing Error Rights For 900 Pay-Per-Call Service Charges This statement sets out your billing error rights with respect to telephone-billed purchases made using interstate 900 pay per-call services that are billed to you through your local Telephone Company. The rights and obligations of you, the con sumer, and of your local Telephone Company and your long distance carriers, who are billing entities, listed below, are provided under the Federal Telephone Disclosure and Dispute Resolution Act. This is the procedure that you must follow to notify your local Telephone Company of a billing error and the steps that must be taken in response to your notice:
Telephone Numbers The customer has no proprietary right to an assigned telephone number. Your local Telephone Company may make changes with the telephone number or central office designation as the requirements of the service demands.

You may provide notice of a billing error to your local Telephone Company by telephone or in writing within 60 days of receiving the bill. If you write, you should: • Give us the customer’s name and telephone number to which the charge was billed. • Tell us the date and amount of the error, and the type of error you believe occurred. • Tell us why you believe it was an error.
If it is determined that no billing error ocurred, your rights and obligations are: You will be given an explanation stating the reasons why it has been determined there was no error. If there was an error – even a partial error or an error different than what you asserted – your account will be adjust ed appropriately. If you request a written explanation and copies of any documentary evidence, it will be provided.
You should contact your local Telephone Company first, since we have an agreement to receive billing error notifications. You may also notify the long distance carrier, but they may ask you to contact us directly. A customer who calls us about a billing error within 60 days of receiving the bill will have provided sufficient notice to initiate a billing review. You have the right to withhold payment of any disputed amount pending completion of the billing review. All actions to collect that disputed amount will be suspended until the review is completed.

Once a billing review is completed and the charges are judged to be yours, failure to pay the outstanding balance may result in collection action being taken by the provider of the 900 service, the long distance carrier or your local Telephone Company. If collec tion action is taken, the collection agency will be advised that the amount is in dispute. You will also be notified that collection action is being taken. If the dispute is sub sequently resolved, that fact will be reported to all who received the initial report that the account was delinquent. If your local Telephone Company or the long distance carrier company, as a billing entity, fails to follow the billing and collection pro cedures prescribed by #308.7 of the Federal Trade Commission rule implementing the Federal Telephone Disclosure and Dispute Resolution Act, we are obligated to forfeit any disputed amount up to $50 per transaction. The FCC also requires disclosures to tele phone subscribers, which are as follows: Your local exchange and long distance service cannot be disconnected or inter rupted as a result of your failure to pay charges for interstate pay-per-call service, charges for interstate 900 pay-per-call services provided under a presubscription or comparable arrangement or charges you have disputed for interstate tariffed collect information services. You may obtain blocking of access to 900 pay-per-call services where it is technically feasible. Requests for blocking and requests for unblocking will be subject to a reason able one-time fee. Our tariffs include the applicable terms and conditions. You have a right not to be billed for pay per-call services not offered in compliance with Federal laws and regulations estab lished under Titles II or III of the Telephone Disclosure and Dispute Resolution Act. Your access to 900 pay-per-call services may be involuntarily blocked for failure to pay legitimate pay-per-call charges.
This action will be taken within two billing periods. If you send the notice to the long dis tance carrier, there may be up to an additional fifteen days to complete the billing review. The long distance carrier will notify the pro vider of the service of the disposition of the billing error investigation and the reasons for that disposition. Your local Telephone Company will notify you in writing, normally with your billing statement, of the date when payment is due on the amount determined not to be in error. Payment must be made in the normal course of billing or as otherwise provided, by not less than ten days after the request for payment is made. Once your local Telephone Company and the long distance carrier have complied with FTC rule #308.7 (d) regarding a claim you make of a billing error, your local Telephone Company has no further responsibility under that rule to reevaluate the claim if you continue to make substantially the same allegation. You will not be charged for a billing review.

You can register over the Internet at www.donotcall.gov or call 1-888-382-1222, TTY 1-866-290-4236, from the phone number you wish to register. Your number will stay registered for five years, or until the number is disconnected or you remove the listing. Requests For Service To order service or to change your tele phone service, please have the following information ready for us: • The type of service you require. • Whether or not you need jacks or inside wiring installed. • The type of listing you want in the phone book. • Your employment and credit information. • Address and alternative number at which you can be reached. • The telephone number of the previous occupant at your location if applicable. Service For A New Construction Call as soon as possible for service to a new building or a building that has not had previous telephone service installed. Note that telephone facilities may not be available in all locations and some delays to your request for service may occur while telephone facility construction takes place. Inside Wiring Customers own the wiring inside their prem ises. If repairs are needed on this wiring, you have several options: • Your local telephone company can install and do the repairs for you at time and materials cost (contact them regarding wiring maintenance plans available). • You can hire a contractor to install or repair your inside wiring. • You can install or repair the inside wiring yourself. Before You Call 1. Hang up all telephones. Make sure phone accessories – answering or fax machines, computers, modems, security system, and telephone sets – are working. To check, unplug the phone cord from the unit or the jack and check for damage. Leave everything unplugged for 5 minutes. Then try plugging in phone sets one at a time to see if the trouble still exists. 2. If all the telephone accessories are work ing, go to your network interface box (outside, where the phone line enters the house).
3. Test to determine if the problem is inside or outside the house: 1. Take a working phone to the network interface box (single line phone only). 2. Remove the modular plug from the test jack and insert the modular plug from your phone. If the same problem occurs... the problem is in the phone line. Call your local telephone company by dialing 611 and they will repair the lines at no charge to you. If the problem does not occur... the problem is in your wiring. Call your local telephone company or another company to repair your inside wiring. If the problem is in your telephone set... follow the instructions on your warranty or contact your equipment retailer for repair or replacement.

Alaska Relay service provides telephone accessibility to people who are deaf, hard of-hearing or speech-disabled. Alaska Relay is available 24 hours a day, 365 days a year, with no restrictions on the number of calls placed or on the length of the calls. Alaska Relay provides state-of-the-art technology, a full range of features, and highly trained pro fessional communication assistants (CAs) to ensure that users are able to communicate easily and effectively – every time they place a relay call. This 24-hour relay service is provided at no cost to callers. Long distance calls will be billed accordingly. What Equipment Do I Need To Use Relay? The most common telephone device used to make a relay call is a TTY (text telephone). There are other telephone devices avail able, depending on the type of relay ser vices used. Alaska residents who are deaf, hard-of-hearing, deaf-blind or speech-dis abled are eligible to receive specialized telephone equipment through the equipment Distribution Program at CSD at minimal or no cost. TDD Legislation Under Alaska state law, speech and hearing impaired persons who are determined by the state to be eligible can obtain special teletypewriter equipment allowing telephone calls to be sent or received by those with speech or hearing impairment. Because these special services are to be provided at a consumer cost no greater than paid by other customers, this program is funded through a monthly surcharge on all tele phone customers in the state.

Century Communications And Video Accessibility Act Of 2010 (“CVAA”) The CVAA requires telecommunications companies to make their products and services accessible to people with disabil ities, if such access is readily achievable. To clearly show that OTZ recognizes the importance of meeting the needs of people with disabilities, OTZ added a question on all OTZ service applications asking whether the applicant has any dis abilities that may inhibit access to service offerings. Warning Sec. 42.20.030. Civil liability for taking utility service and for damages to and or interfer ence with a utility line. (a) A person is civilly liable to the utility whose property is damaged in a sum equal to three times the amount of the actual damages sustained and three times the value of the service taken if the person intentionally or in the exercise of gross negligence (1) damages a pole, tree, pipeline, or other object used in a utility line; (2) damages an insulator in use in the line or damages or removes from its insulator any wire used in a utility line; (3) damages the insulation of the line or interrupts the transmission of the service through it or damages the protective wrapping of a water, oil, or gas pipeline; (4) damages property or materials belong ing to a utility; (5) interferes with the use of a utility line, or obstructs or postpones the transmission of service over a utility line; (6) procures or advises damage to any utility line; (7) interferes with or alters a meter or other device for the measuring of service from a utility; or (8) without the permission of the utility takes service from the utility or taps a wire or line used for the transmission of service or procures or advises this be done. (b) A person is civilly liable to the utility whose property is negligently damaged under (a) (1), (2), (3), (4), or (5) of this section in a sum equal to the amount of the damages sus tained. (§ 49-5-12 ACLA 1949; am § 1 ch 39 SLA 1964, am §§ 1,2 ch 68 SLA 1980). Sec. 11.61.120 Harassment. (a) A person commits the crime of harassment if, with intent to harass or annoy another person, that person (1) insults, taunts, or challenges another person in a manner likely to provoke an immediate violent response; (2) telephones another and fails to terminate the connection with intent to impair the ability of that person to place or receive telephone calls; (3) makes repeated telephone calls at extremely inconvenient hours;
(4) makes an anonymous or obscene tele phone call or a telephone call that threat ens physical injury; (5) subjects another person to offensive physical contact; or (6) violates a provision of an order issued under AS 25.35.010. (b) or 25.35.020 restraining the respondent from com municating directly or indirectly with the petitioner. (b) Harassment is a class B misdemeanor. (§ 7 ch 166 SLA 1978; am § 10 ch 61 SLA 1982) Sec. 42.20.310. Eavesdropping. A person may not (1) use an eavesdropping device to hear or record all or any part of an oral conver sation without the consent of a party to the conversation; (2) use or divulge any information which the person knows or reasonably should know was obtained through the illegal use of an eavesdropping device for per sonal benefit or another’s benefit; (3) publish the existence, contents, sub stance, purport, effect or meaning of any conversation the person has heard through the illegal use of an eavesdrop ping device; (4) divulge, or publish the existence, con tents, substance, purport, effect or meaning of any conversation the per son has become acquainted with after the person knows or reasonably should know that the conversation and the information contained in the conversa tion was obtained through the illegal use of an eavesdropping device. (c) In this section “eavesdropping device” means any device capable of being used to hear or record oral conversation whether the conversation is conducted in person, by telephone, or by any other means; provided that this definition does not include devices used for the restoration of the deaf or hard of-hearing to normal or partial hearing. Caller ID Caller ID lets you know who is calling before you answer the phone (but will not identify callers with unpublished numbers). OTZ offers Caller ID service, but to use the ser vice you will need a separate display unit, or a telephone with a built-in display. You can buy display units from OTZ, an electronics store, or in the electronics department of larger variety stores. Instructions for using Caller ID will vary slightly with the display unit you choose; follow the directions pro Many OTZ subscribers choose the extra benefits of Custom Calling features. These features are designed to enhance the qual ity and enjoyment of your telephone calls. Let us help make your life simpler with Custom Calling. Using Custom Calling Features Three-Way Calling This feature allows you to bring a third party into a previously established call. For example, if you’re talking with a relative in Anchorage, you can include another friend or relative in your village – then all three of you can talk at the same time! When someone hangs up, you are still connected to the other person sharing the call. To Use Three-Way Calling 1. Call the first person you want to talk with. After establishing a connection, you can go ahead with the next step. 2. To add a third party, depress and release the hook switch (or “flash” button); this puts the first party on hold. Listen for three bursts of dial tone followed by a steady (normal) dial tone. 3. Dial the number of the third party. When this person answers, you can talk pri vately. When you are ready to establish the three-way connection, depress and release the hook switch. (Note: If the person you call does not answer, or the line is busy, just depress and release the hook switch and you will be reconnected to your original party.) 4. When you hang up, all parties are discon nected and the call is completed.

Call Waiting When you’re on the phone and someone else is trying to call you, Call Waiting sounds a tone to alert you to the incoming call. Call Waiting also lets you answer incoming calls and talk privately, without losing the original phone connection. You can even use Call Waiting to switch between phone conversations.
Call Forwarding Call Forwarding is a convenient way to help phone calls catch up with you when you’re on the go. There are two ways to set up Call Forwarding (Fixed and Variable) and you will want to discuss the choices with an OTZ Customer Service Representative. To Use Call Forwarding Each community in the OTZ service area has unique programming numbers for Call Forwarding and Variable Call Forwarding. Please ask an OTZ Customer Service Representative to help you write in the correct numbers for your service area. Fixed Call Forwarding When Fixed Call Forwarding is turned on, the feature automatically forwards your calls to one other phone number that you have chosen. Forwarding to this pre-set number is automatic, or “fixed.” 1. To turn on this feature, dial *95 and wait 4 seconds* for two bursts of dial tone. Call Forwarding is now turned on. Hang up, or, if you want to call the “forward-to” number (so you can tell someone you are forwarding your calls), just stay on the line and wait another 4 seconds and the call will be placed. 2. To turn off this feature and restore normal service, dial *96 and wait 4 seconds* for two bursts of dial tone; then hang up.
To Use Call Waiting 1. You will hear an “alert” tone when you are already on the phone and an incoming call is waiting. 2. Depress and release the hook switch (or “flash” button) to put the original con versation on hold. This simultaneously connects you with the incoming call. 3. To switch between callers, depress and release the hook switch once for each transfer. (Note: The caller on hold cannot hear the other caller’s conversation with you.) 4. If you are ready to end the original con versation when you hear the Call Waiting alert tone, simply hang up. Your tele phone will ring. Lift the receiver and you will be connected to the incoming caller.

Variable Call Forwarding With this option, you have to dial a new “forward-to” number every time you activate Call Forwarding. 1. To turn on this feature, dial *72 and wait 4 seconds* for a dial tone; then dial the forward-to number and wait another 4 seconds* for two bursts of dial tone. Hang up, or if you want to call the “for ward-to” number, stay on the line for another 4 seconds* and the call will be placed. 2. To turn off this feature and restore normal service, dial *73 and wait 4 seconds* for two bursts of dial tone; then hang up. *If you own a push-button telephone, you can skip the 4-second wait by depressing the (#) symbol. *Not all services are available in all locations. Call customer service for your area.
Speed Calling (Abbreviated Dialing)
Speed Calling makes it easy to dial numbers that you call often. Instead of looking up the number, or dialing lots of long distance digits, the phone will automatically do the dialing for you – when you dial just one or two coded digits. Size Options You can have just a few numbers set up for Speed Calling – or as many as 30! This deci sion can affect the cost of service, so please be sure to talk with an OTZ Customer Service Representative before you make a choice. 8-Number Option With this option, your Speed Calling codes are the single digits 2 through 9. This limits you to eight Speed Calling codes. 30-Number Option Your speed calling codes are the double digits 20 through 49. You can choose to code up to 30 phone numbers.

To Use Speed Calling To use Speed Calling, simply dial the desired code number and wait 4 seconds* for the call to be put through. * If you own a push-button telephone, you can skip the 4 second wait by depressing the (#) symbol. Selective Call Rejection Description This feature allows a subscriber to reject calls from any party that is programmed on the subscriber’s Selective Call Rejection list. Rejected calls are routed to the Selective Call Rejection intercept treatment. Conditions The CLASS 1 feature must be purchased and enabled. Programming 1. Set the feature access codes for Selective Call Rejection Activation and Selective Call Rejection Deactivation in the Digit Translation Assignment view. 2. Program the Selective Call Rejection Assignment view with the subscriber’s directory number. Note that the “activate” parameter must initially be set to “no” (the parameter may be set to “yes” after step 3 is completed).
Speed Calling (cont’d.) Coding Choices
There are two ways to set up the codes for Speed Calling. Before you decide which will work best for you, talk with an OTZ Customer Service Representative. OTZ Coding With this option, you tell us what num bers you want to call, and OTZ Telephone Company will take care of all the coding. But you will not be able to make changes to codes from your telephone. Subscriber Coding This enables you to set up and change the Speed Calling codes from your telephone. 1. To enter a new Speed Calling code, dial 74 and wait 4 seconds* for a dial tone. 2. From the available code numbers, dial the code number you want to use. 3. Then dial the telephone number you want to assign to the code number; wait 4 seconds for two bursts of dial tone; then hang up. 4. Repeat steps 1 through 3 for each number to be stored, using a different code number each time.

Selective Call Rejection (cont’d.) 3. Program the Selective Call Rejection List Member Assignment views with the directory numbers from which calls are to be rejected. Operation To Activate Selective Call Rejection: 1. Lift the handset. Dial tone is returned. Press *60. 2. Dial the Selective Call Rejection Activation feature access code. Confirmation tone is returned. To Cancel Selective Call Rejection: 1. Lift the handset. Dial tone is returned. Press *61. 2. Dial the Selective Call Rejection Deac tivation feature access code. Confirmation tone is returned.
the subscriber is required to activate/deac tivate the feature or modify the SCR screen ing list using SLE. However, the telephone company also has the ability to administer the active/inactive status, and the screening lists for SCR on an individual subscriber basis. The SCR subscriber or telephone company must assign one or more SCR screening list DNs before SCR activation is allowed. The SCR subscriber can activate the feature, deactivate the feature, modify the screening list and/or obtain a status report by dialing an access code. The SCR feature access codes are administered by the telephone company. Access codes are from one to five characters in length and are administered on a per-office basis. The recommended SCR default access codes are *60 and *80 for DTMF subscribers, and 1160 and 1180 for Rotary subscribers.
Selective Call Rejection (SCR) Selective Call Rejection allows subscribers to create a list of numbers from which they do not wish to receive calls. SCR can also be used to block calls from a number after an undesirable call has been received (even without knowing the number). All calls on the screening list are intercepted and rerouted to a rejection announcement. The telephone subscriber is required to con tact the telephone company to request SCR through the service order process. Usually,
