Annual CPNI Notice To Our Valued Customers;

ASTAC respects the privacy of our customers and our employees do everything possible to protect information that may be found in your account records. The Federal Communications Commission developed rules in 2007 that require certain steps be taken to protect Customer Proprietary Network Information (CPNI). If requested, ASTAC will provide account informa- tion to our customers in one of the following ways: 1) We will verify the password you provided to us when you call in with questions about your CPNI; 2) We may call the telephone number of record and provide the account information; 3) We may mail the information to the address on record; or 4) We may provide the information at our office if we are presented with valid photo identification and the individual so identi- fied is on the account. We do not anticipate that these rules will interfere with providing quality customer service. If you wish to receive additional information regarding CPNI rules and how they are implemented in ASTAC, please call or stop by our office. 21st Century Communications & Video Accessibility Act Of 2010 The 21st Century Communications and Video Accessibility Act (CVAA), a federally mandated program, requires communications service providers to communicate with customers on accessibility offerings. Providers must keep records that show what they have done to comply with the CVAA and must certify their compliance each year. If you have any questions regarding accessibility for people with disabilities, please contact ASTAC at 1-800-478-6409 . Red Flag Program ASTAC maintains covered accounts and, in accordance with FTC rules, has developed a written Identity Theft Prevention Program. It is designed to detect, prevent, and mitigate identity theft in connection with the opening of a covered account or with any current existing covered account.
