You should know that restricting your BNA will invalidate your joint use calling card, restrict your ability to bill third number calls to your home number and prevent you or others from making collect calls to your home number. Just one number to dial for any number, anytime, anywhere in the United States. Rates are $.75 for in-state and $.95 for out-of-state numbers. To obtain a toll-free listing: Dial 1-800-555-1212 . There is no charge for 800 Directory Assistance.

Lifeline Program Lifeline assistance lowers the cost of basic monthly local telephone service. Eligible consumers will receive a $9.25 discount off their normal monthly local service charge. Lifeline service is limited to one line per household at the customer’s primary residence. Individuals are eligible if he or she participates (or the household) in one of the following programs: • Food Stamps/SNAP • Medicaid • Supplemental Security Income (SSI)—(Not the same as Social Security Benefits) • Federal Public Housing Assistance (FPHA) • Veterans Pension & Survivors Benefits Programs In addition, a consumer may be eligible if his or her household income is at or below 135% of the federal poverty guidelines. Visit or call your local telephone company at 846-7200 for more information; or you may contact Lifeline Administrator, 30 Lanidex Plaza West, PO Box 685, Parsippany, N.J. 07054-0685. Under the FCC rules, your BNA (Billing name and address) can be released for non-marketing purposes to an interstate tele- communications provider which (a) is not your presubscribed long distance company, and (b) does not use Prairie Grove Telephone Company to bill and collect for their services. As a non-listed/non-published customer you still have the right to request that your BNA will not be disclosed in connection with col- lect calls, third number billed calls and joint use calling card calls.
