Your Rights As A Customer The Arkansas Public Service Commission requires utilities to pro- vide customers with information pertaining to telephone service. The Commission’s Rules are displayed in our Business Office, which is open to the public and shall be provided to anyone upon request. Telephone Fraud Charging long distance calls to a number other than your own, with- out permission, is illegal. Obscene, Harassing Or Threatening Calls Placing obscene or harassing telephone calls is a crime. Contact your police department or telephone company for help if you receive such calls. Telephone Used For Business Purposes A telephone number which is used substantially for business pur- poses or is advertised in connection with the sale of products or services should be billed at the business rate. Downgrades - Business To Residence Service If you are a business customer who wishes to change from a business telephone service to residence telephone service, you will be required to have your telephone number changed to a different telephone number. Service Problems If you experience service problems, report them promptly to our Repair Service. Repair and replacement of telephone company equipment due to ordinary usage or unavoidable accidents are made without charge to you. (For more details, see page 15.) Liability If your listing in the directory is incorrect or omitted, please notify our Business Office. It cannot be changed until the next edition of the directory. In the meantime, we can make sure the listing is correct on Directory Assistance records. In some cases, it is possible for calls to be forwarded to the correct number. The Telephone Company is not liable for errors or omissions in this directory except for extra listings in the alphabetical white pages for which a charge is made. If an error or omission occurs in an extra listing in the alphabetical white pages for which a charge is made, the Telephone Company’s liability is limited to the cancel- lation of charges and refunding of any charges already paid for by the customer. TELEPHONE SERVICE IS PROVIDED ON A NON- DISCRIMINATORY BASIS. Complaint Procedures A billing problem should be reported promptly to our Business Office. A service problem should be reported promptly to our Repair Service. These telephone numbers are listed in this directory. If you are not satisfied, feel free to ask for the supervisor. If your problem can’t be solved by the supervisor, ask for the manager.

If we are unable to resolve the matter to your satisfaction, you may contact the Arkansas Public Service Commission by phone or in writing at Consumer Services Office, Arkansas Public Service Commission, P.O. You may withhold payment if you dispute these charges within 60 days. Action to collect disputed amounts will be suspended pending investigation of the dispute. Neither local nor long distance services can be disconnected for non-payment; although, other action may be taken to collect amount unpaid but not disputed. You may request 900 blocking from Prairie Grove Telephone Company. You are not to be billed for pay-per-call services that do not comply with federal laws and regulations. Toll restriction of 900 numbers is available to you at no charge. Contact our Business Office for more information. 800 Or 900 Dialing When an 800 or 900 number is dialed from your telephone, your telephone number may be transmitted to the company you have called and may be available to that company’s service representa- Applications for new service, changes in existing service or discon- tinuance of service may be made at our Business Office in person, by telephone or by mail. Charges for installation of new service and changes in existing ser- vice will vary depending on the type of work involved. The one-time charges apply when: Service Order Charge – Work done in receiving, recording and pro- cessing information necessary to fulfill your request for new service or changes in existing service. Line Connection Charge – Work done in central office and/or on outside plant to provide each access line for service. Premises Visit – Work requiring a trip to the customer’s premises.
