Custom Calling Instructions (for customer leasing these optional services)

Calling Name/Number Delivery cont. Note: • The “switchhook” is the round, flat, or square button you push to hang up the phone. • The class features listed presently do not function on a long distance call. • The Calling Name/Number Delivery or Calling Number Delivery requires subscription to these services and the purchase or lease of a display telephone and/or an add- on display unit. Anonymous Call Rejection (Activate dial *77, deactivate dial *87) This feature allows subscribers with or without Calling Number Delivery and/or Calling Name Delivery to reject calls for which the caller has intentionally blocked calling name/number display information. Per Line Blocking Prevents your phone number from being displayed to the person receiving your call. Calling Number Delivery Blocking suppresses your directory number delivery so that parties with calling number delivery will not receive that information. If you have “blocked” your number, the called party’s display will show a “PRIVATE” message. Per Call Blocking capability is on your line, unless you have requested per line blocking (with this option, your directory number is always blocked unless you unblock for any indi- vidual call). With Per Call Blocking, your directory number will be displayed unless you dial the blocking code before making each call. To block your number from being displayed: 1. Pick up the handset and listen for the dial tone. 4. The person you’ve called will not be able to see your number displayed on their telephone display screen. Instead a “private” message, or “P” will be displayed. Note: • You must dial *82 before each call you wish to block. Otherwise, your phone number will be released to the person receiving your call. • There is no monthly charge or per-use charge for Calling Number Delivery Blocking; however, should you wish to change from Per Call Blocking (normally not blocked… the standard option) to Per Line Blocking (normally blocked), or vice versa, after 90 days of service estab- lishment, a service order charge will apply. * 8 TUV 2 ABC PRIVATE 2. Press *82. On a rotary phone dial 1182. 3. Dial the number you’re calling as usual.

Per Call Blocking Prevents your phone number from being displayed to the person receiving your call. Calling Number Delivery Blocking suppresses your directo- ry number delivery so that parties with Calling Number Delivery will not receive that information. If your number is “blocked,” the called party’s display will show a “private” message. To block your number for display to a called party: 1. Pick up the handset and listen for the dial tone. 4. The people you’ve called will not be able to see your number displayed on their telephone display screen. When you hang up and place another call, your number will again be displayed unless you use the blocking code before dialing. Note: • You must dial *67 before each call you wish to block. Otherwise, your phone number will be released to the person receiving your call. • There is no monthly charge or per-use charge for Calling Number Delivery Blocking. Call Forward Busy When your line is busy, you can be sure your callers can reach someone else. How it works: Whenever you’re on the phone, you can be sure your callers can talk with someone else or can leave a message for you. Easily turn it on or off, or change the forwarding number – anytime. To “turn on” the service: 1. Lift the handset and listen for the dial tone. 4. Dial the number where you want your calls forwarded. 5. When the phone is answered, your Call Forward Busy is in effect. If the line is busy or there’s no answer: 1. Hang up. 2. Within two minutes, repeat steps 1-4 above. You’ll hear a confirmation tone to let you know your Call Forward Busy is now working. To “turn off” the service: 1. Lift the handset and listen for the dial tone. * 6 MNO 7 PRS * 9 WXY 0 2. Press *67. On a rotary phone dial 1167. 3. Dial the number you’re calling as usual. 2. Press *90. (On a rotary phone, dial 1190.) 3. Listen for the dial tone.
