Residential and Business Customers For the quality service you deserve When You’re Away In order to avoid disconnection of telephone service when you are away from your residence or business for one month or more; call Dobson Telephone at 964-8600. Establishing Service In order to establish telephone service, you will need to present the following information: 1. Complete legal and billing address, including street and number (apart- ment number, if applicable), route and box number, plus city and zip code. 2. The type of basic or optional monthly service desired. 3. Directory listing preferences. 4. Other credit information, including employment and social security number. Deposits The amount of the deposit to establish telephone service is determined by estimated two-month billing and other credit information. Service Charges New customers are assessed a one-time service connection charge, which includes the cost of processing the order and connecting your service to the network. The charges for work done at your residence are based on the actual amount of time needed to install, move or change the wiring in your home. The initial charge for the first 30 minutes of work includes travel and preparation time. In addition, there is a charge for each additional 15 minutes of time needed to complete the work at your home. For more information on charges, call Dobson Telephone at 964-8600. Telephone Harassment It is a crime under state and federal laws for anyone to make obscene or harass- ing telephone calls. These laws have penalties of imprisonment and/or fines. Unlawful Wiretapping It is a crime to wiretap or otherwise intercept a telephone call, unless that person has first obtained the consent of one or both parties actually participating in the call. Properly authorized law enforcement officers can take part in interceptions without the consent of either party when proceeding under court orders issued following the appropriate state or federal law. The penalty for illegal wiretapping can be imprisonment and/or fines.

Optional Services Repair Service

Dobson Telephone Company is only responsible for the maintenance and repair of the telephone lines from its facilities. If a customer-requested service call is made, and it is determined that the problem is located in your telephone or inside wiring, and not in the Dobson facilities, you will be billed for the service call. If you feel that the problem is not in your telephone or inside wire, please call the repair office at 391-3359. Additional Listings The purchase of a separate and additional listing in the telephone directory and with directory assistance. Additional Line If you are frequent users of the telephone, or if a modem or fax is needed for your personal computer, an additional line with a different telephone number can be installed in your home. Looking For Symptoms Before calling the telephone company, do some checking on your own. This will save you time and money. When you check, look for the following: A service charge will be applied if a serviceman is dispatched and the problem was a phone off the hook. This charge will apply even if someone, other than the person on the account, calls to report the trouble. If you have just one phone that isn’t working, borrow another working phone and plug it into the same jack. If the borrowed telephone works, it can be assumed that it is the telephone that needs repair. If the telephone seems to be in good working condition, please call the repair office at 391-3359. Unlisted Or Non-Published Number Your telephone number is not listed in the directory and will not be available through directory assistance. To be unlisted there is a monthly charge. Fraudulent Billing Fraudulent billing can lead to prosecution. It is illegal for a person to charge any calls to another person’s number or calling card without their permission. Direct Distance Dialing For your long distance needs

With Direct Distance Dialing, you can dial most station-to-station long distance calls. To make Direct Distance Dialing possible, the United States has been divided into numerous geographic areas. Each area has its own three-digit area code. You can obtain the area code of the place you are calling by checking the map located on page eleven. If the map does not clearly show the area code you want and it is not listed in the introductory pages of this directory, call the operator for the area code or look up www.555-1212.com on the Internet. You can dial direct to any telephone with an area code plus the seven digit number. How to Dial From All Telephones Except Pay Phones First, get the correct local number and the area code of the person you want to call. For Station-To-Station Calls, Dial 1 + area code + telephone number For International Calls Many International calls can be dialed direct by dialing 011 + County Code + City Code + Local Number. How To Dial For Information On Numbers* If you do not have the number of the person in your local area, dial 1 + 411. Or, dial 1 + area code + 555-1212 for long distance information of the town you want to call. For Other Calls, Dial 0 + area code + telephone number (0 + dialing is used with operator assistance for person to person, calling card or collect calls.) Busy Signals and No Answers If you get a busy signal or if no one answers the telephone you are calling, hang up and try the call a little later. No charge is made on calls not completed. If You Need Help If you are cut off during a call, encounter difficulty in hearing or experience any other trouble, just dial the operator and report it. Charges are based upon rates in effect at the time of connection at the calling point. This also includes calls beginning in one rate period and ending in another.

Station-To-Station Station-to-station calls are not directed to a particular person. The caller talks with whoever answers the phone. Rates for calls that you dial are lower than calls dialed by the operator. Charges begin when the called telephone is answered. Conference Calls You can talk with several people in different places at the same time. Tell the operator you wish to make a conference call. A special charge will apply. Person-To-Person Call person-to-person when you want to talk to a particular person or extension number. Tell the operator you want to make a person-to-person call. Rates are higher than station-to-station calls and also apply when you talk to an alternative person or extension. Charges start when the conversation begins. Long Distance Calls There are three basic kinds of long distance calls. They are: Customer-Dialed, Station-to-Station is the least expensive type of call. If the customer dials the call directly without operator assistance, the customer is charged at the lowest rate in effect at the time the call is placed. Operator-Assisted, Station-to-Station requires operator assistance for comple- tion. The customer talks with whoever answers the telephone, but needs an operator’s assistance to reach the number. Examples of these calls include coin, collect, calling card and code billing (special billing numbers, guest, time and charges are sequence calls). Person-to-Person, the most expensive type of call, is for when you want to speak directly to a particular person, telephone, department or office reached through a customer’s switchboard or PBX. Collect You can call collect if the person or business you are calling agrees to pay for the charge. If you want to make a collect call, please tell the operator when you place the call.

General Information You need to know If You Hear A “Beep”

A short, high “beep” tone on the telephone line about every 15 seconds means the person you are talking to is recording your conservation by means of an electric recording machine connected to the telephone line. This signal is provided by Dobson Telephone for your protection. If you don’t want your conversation recorded, ask the person with whom you are talking to disconnect the recording machine. When the recorder is disconnected, the beeping will stop. The “beep” is produced automatically by the device used to connect the recorder to the telephone line. It stops when the recorder is disconnected. Use of a recorder without this signal is unlawful. Service Application Changes And Discontinuation Applications for new service must be made by contacting Dobson Telephone business office. Changes or additions to existing service or equipment may be made by mail, in person or by telephone. Ample notice will help us move telephone equipment so that service will be practically uninterrupted. Liabilities Dobson Telephone does not assume liability for errors or omissions in this directory. Dobson Telephone reserves the right to change any telephone num- ber at any time, and cannot guarantee uninterrupted service. Subscribers are responsible for the payment of all toll calls originating at the telephones. Incorrect Directory Listings If your listing in the directory is incorrect, it cannot be changed until the next edition of the directory. In the meantime, we can make the listing correct in directory assistance records. To arrange this, call Dobson Telephone at 964-8600. 900 Call Restriction 900 Call Restriction blocks calls from being directly dialed from your line to any 900 information service program number. Call Dobson Telephone at 964-8600 for information on how to obtain 900 Call Restriction. 3rd Party And Collect Block 3rd Party and Collect Block restricts anyone from making these type of calls and charging it to your phone. Annoyance Calls It is unlawful for any person to make a telephone call during which they make any comment, request, suggestion or proposal that is obscene, lewd, lascivious, filthy or indecent. It is also unlawful to make certain telephone calls with the intent to annoy, torment, harass or embarrass any person. Violation of this law is punishable by $1,000 fine, one year in jail, or both.

Customer-Provided Equipment Equipment, apparatus, devices or systems not furnished by Dobson Telephone Company may be used with the company’s facilities and equipment, if such use complies with the company’s tariffs. Any item used with the company’s facilities and equipment in violation of Dobson’s tariffs may lead to suspension of your telephone service or disconnection from the network of the equipment, apparatus, devices or systems. Contact the Dobson Telephone business office for other information regarding these regulations. Application Of Business Rates The use of telephone service for business purposes will subject that service to business rates regardless of the type of premise on which the telephone number is installed. Advertising a resident phone number for business purposes will be considered sufficient cause for Dobson Telephone to charge the business rate. Any subscriber practicing a profession or operating a business and having no office other than their home will be considered a business subscriber. Procedure For Petitioning The Commission To Examine The Reasonableness Of Proposed Local Exchange Rate Change Under Oklahoma law enacted during 1986 (17 O.S. § 137), this telephone company can raise your local exchange rate up to $2 per month every year without the review and approval of the Oklahoma Corporation Commission. At least 60 days before the effective date of such a proposed rate change, each subscriber will be notified by the telephone company. Such an increase can be suspended if 15 percent of subscribers petition the Oklahoma Corporation Commission to examine and determine the reasonableness of the proposed local exchange rates. Every Petition Must Contain 1. The name, address and telephone number of the subscriber. 2. The signature of the telephone subscriber in the name that the telephone service is listed. 3. The reason the subscriber opposes the proposed increase (optional). Only the subscriber whose name is on the telephone service shall be counted as a petitioner.

Abuse or fraudulent use includes the use of Dobson Telephone service or facilities to transmit a message, locate a person, or otherwise give or obtain information with a payment of a long distance charge. Abuse or fraudulent use also includes charging a long distance call to a false number, to an unauthorized calling card number or to another number without permission. For your protection, new equipment and procedures enable Dobson Telephone to detect and investigate fraudulent use of telephone facilities. Federal and state laws prohibit any person from defrauding Dobson Telephone of its lawful charges. Violators are subject to imprisonment, fines, or both, upon conviction.

Questions About your bill

Charges for local monthly service are billed one month in advance. Charges for long distance calls are based on actual use up to the date your bill is prepared. Charges for installing or changing your monthly service usually appear on the first bill after your order is completed. Q. When is my bill due? A. The bill is due upon receipt and becomes delinquent on the 15th of each month. Q. What happens if I pay my bill with a check my bank will not honor? A. A disconnect will occur unless the check is replaced promptly with cash or a cashier’s check. A returned check fee will be charged the following month, along with the amount of the check. Q. What happens if I can’t pay my bill on time? A. If you can’t pay your bill by the due date, please call Dobson Telephone office at 964-8600. We may be able to make payment arrangements with you. Q. How can I get service restored if it has been disconnected for nonpayment? A. All delinquent charges must be paid before service can be reconnected. You also must pay a charge to have the service restored and your credit will have to be reestablished. As a result, advance payment and a deposit may be required. Q. How do I pay my bill? A. How to Handle Obscene or Prank Calls

If you receive abusive, harassing or threatening calls, follow these suggestions:

1. Hang up • At the first obscene word. • If the caller doesn’t say anything the second time you say hello. 2. Give no information • Such as name, address or telephone numbers, until the caller has been identified. 3. Advise your children • To give no information to strangers. • That if you’re not home they should say, “Mother/Father can’t come to the phone right now.”

Automatic Callback continues to dial and redial busy numbers, alerting you as soon as busy lines are open again. It even allows you to continue to make and receive other calls, while Automatic Callback pursues those callers with busy lines. How To Use Automatic Callback 1. When you’ve made a call and hear a busy signal, depress the switch hook for one second and release. 2. If you’ve already hung up, pick up the handset and listen for a dial tone. 3. Press * 66. (On a rotary phone, dial 1166.) 4. If the line is still busy, you will receive a recording that the party is busy. Hang up; your phone will continue to check the number for 30 minutes. 5. Call Forward Remote Access The simple way to forward your calls Call Forward Remote Access is used to turn on, turn off or change the telephone number your calls are forwarded to from any touch-tone phone with touch-tone service. With Call Waiting, you won’t miss another important call! When you hear the special tone, simply put the first caller on hold and answer the second call. Call Waiting puts the power to regulate calls at your fingertips. You can easily override the feature for uninterrupted calls. How To Use Call Waiting 1. When you’re on the phone with call #1, a special tone will sound to alert you of an incoming call. 2. To answer call #2 (and place call #1 on hold), depress the switch hook for one second, then release. You’ll then be connected to call #2. 3. By depressing the switch hook, you can continue to switch back and forth between call #1 and call #2. How To Temporarily Cancel Call Waiting 1. Before you make your call, dial * 70. (On a rotary phone dial 70, and wait four seconds.) 2. Wait for a dial tone, and make your call. Cancel Call Waiting is in effect for that call only. 3. By depressing the switch hook, you can continue to switch back and forth between call #1 and call #2. Custom Call The ring tells you which calls are yours Custom Call* is the inexpensive service that allows you to add an additional telephone number while using your existing phone line. This additional number has a different ringing tone to distinguish between incoming calls. Each number is listed separately in the Dobson Telephone Directory. With Custom Call, there will be no more mad dashes to the phone to answer someone else’s calls because everyone in your home can instantly tell who the incoming call is for.

Custom Toll Restriction Keep control of long distance calls

Over the years, friends and family get scattered around the country. Unfortunately, long distance charges can add up and unnecessary calls can become a burden. With Custom Toll Restriction, no long distance calls can be placed without first entering a special PIN, so you can keep control of long distance calling.

How To Set Up Your Custom Toll PIN Number 1. Pick up handset and listen for dial tone. 2. Dial * 97. 3. When prompted for your PIN number, enter#. (- - - -#) 4. Re-enter the same number again and press #. (- - - -#)

How To Make Long Distance Calls When Custom Toll Restriction Is Activated 1. Dial 1 + area code + seven-digit telephone number. 2. You will hear a recording “Sorry this call is not permitted please hang up”. Dial your pin # at this time and press #. The Dobson Telephone business office cannot change or look up PIN numbers. If you forget your PIN, the business office can reset your PIN to 0000. There will be a service charge for resetting your PIN.

Distinctive Ringing/Call Waiting The easy way to know your most important incoming calls You’re on a phone call with a friend. Call Waiting lets you know if someone is trying to reach you. But how will you know if the caller is someone important, such as your boss, spouse, or client? Distinctive Ringing/Call Waiting identifies important calls with a distinctive Ring or Call Waiting tone. Other non-priority calls receive the usual Ring or Call Waiting tone. Distinctive Ringing/Call Waiting can store over 30 priority numbers of family, friends, employers, customers or other important people you don’t want to miss! This service requires that Call Waiting is active. How to Use Distinctive Ringing/Call Waiting 1. Pick up the handset and listen to the dial tone. 2. Press * 61. (On a rotary phone, dial 1161.) A recording will tell you if the service is off or on and then give the following options. Dial 0 for instructions. - Dial # to add an entry. - Dial * to delete an entry. - Dial 1 to hear the entries on the list. - Dial 3 to turn service off or on. When Someone Calls 1. If the call is from someone on your VIP list, you’ll hear a special ring. All other calls will have a normal ringing pattern. 2. If you have Call Waiting and get a call from a number on your list while you’re on another call, you’ll hear a special Call Waiting tone. All other calls will have a normal ringing pattern. You may press 1, 0, # or * at any time rather than wait for voice instructions. The VIP list you create here is separate from any other lists you may be using for Selective Call Acceptance, Selective Call Forwarding and similar services.

Hot Line And Warm Line Fast help in an emergency

Do you have loved ones that may need to get in touch with you in an emergency? Hot Line and Warm Line are two fast and easy ways for your loved ones to contact you in their time of need. The Hot Line feature allows a customer’s phone to dial a predetermined telephone number automatically after the phone goes off-hook. The predetermined number could be a pager or mobile phone number, ensuring that you are contacted immediately. The Warm Line also contacts a predetermined telephone number, but does not begin dialing until the phone has been off the hook for 30 seconds.

Selective Call Acceptance You decide which calls to take and when You’ve just sat down to a family dinner when the phone rings. Is it a call from someone you are expecting or an unwanted sales pitch? With Selective Call Acceptance, you program in the phone numbers of callers you would want to speak with. How To Use Selective Call Acceptance 1. Pick up the handset and listen for the dial tone. 2. Press * 62. (On a rotary phone, dial 1162.) A recording will tell you if the service is off or on and then give the following options. Dial 0 for instructions. - Dial # to add an entry. - Dial * to delete an entry. - Dial 1 to hear the entries on the list. - Dial 3 to turn service off or on. When Someone Calls When your service is turned ON, you’ll receive calls only from those on your acceptance list. Callers who are not on your list will simply hear an announcement that you are not accepting calls at this time. You may press 1, 0, #, or * at any time rather than wait for voice instructions. The list you create here is separate from any other lists you may be using for Selective Call Forwarding, Selective Call Rejection, Distinctive Ringing/Call Waiting and similar services.

Selective Call Forwarding Receive only the calls you want when you’re away Selective Call Forwarding makes pre-selected calls reach you when you’re away from the home or office. Simply program in the phone numbers of the calls you don’t want to miss, and Selective Call Forwarding will send calls from those numbers to you. All other calls will get your answering machine or voice mail. How To Use Selective Call Forwarding 1. Pick up the handset and listen for the dial tone. 2. Press * 63. (On a rotary phone, dial 1163.) A recording will tell you if the service is off or on and then give the following options. Dial 0 for instructions. - Dial # to add an entry. - Dial * to delete an entry. - Dial 1 to hear the entries on the list. - Dial 3 to turn service off or on. When Someone Calls If your service is turned ON, and the caller is on your forward list, the call will be re-routed to your “forward-to” number. If the caller is not on your forward list, the call will ring at your home as usual. You may press 1, 0, # or * at any time rather than wait for voice instructions. The list you create here is separate from any other lists you may be using for Selective Call Acceptance, Selective Call Rejection, Distinctive Ringing/Call Waiting and similar services. This service can work with Call Forward (all calls). Numbers on your selective list will follow you to your Selective Call Forward number. All other calls you receive will be routed to your regular Call Forward number.

Selective Call Rejection Lets unwanted callers know that you are not accepting their calls Selective Call Rejection routes unwanted calls to an announcement that you are not taking calls right now. Perhaps it’s a persistent caller or salesperson who won’t take ‘No!’ for an answer. With Selective Call Rejection, once you’ve given your telephone the numbers of unwelcome callers, they get the message. How To Use Selective Call Rejection 1. Pick up the handset and listen for the dial tone. 2. Press * 60. (On a rotary phone, dial 1160.) A recording will tell you if the service is off or on and then give the following options. Dial 0 for instructions. - Dial # to add an entry. - Dial * to delete an entry. - Dial 1 to hear the entries on the list. - Dial 3 to turn service off or on. Simultaneous Ringing “SimRing” Need someone to find you? SimRing offers anytime, anyplace “Find Me” service for busy people that have multiple directory numbers (office, home, mobile). You can set up your phone to ring to four other numbers all at one time. The first SimRing member to answer is connected to the calling party and all other members are dropped. This feature is easy to turn on and off. Dial * 78 from the line that has the SimRing feature assigned. A recording will tell you if the service is off or on and then give the following options.

Speed Dialing This is the fastest, easiest way to access frequently called numbers To search for and dial an important phone number can be time consuming, especially if it’s an emergency or you’re pressed for time. With Speed Dialing, you can pre-program up to 30 important or frequently called numbers then dial them simply by pressing a one or two digit Speed Dialing code. In addition, it works from every phone. How To Use Speed Dialing Pick up the phone and enter * and then the Speed Dialing code for the number you want to call. (Omit the * for rotary phones.) How To Set Up Speed Dialing 1. Listen for dial tone. Dial * 74. (Omit the * for rotary phones.) 2. Listen for second dial tone. 3. Dial the Speed Dialing code you want to assign that number (2 through 9 for customers subscribed to 8 or 20 through 49 for customers subscribed to 30). 4. Dial the complete number you wish to program. Remember to include the 1 and area code for long distance numbers. 5. Press the # button. (Omit this step for rotary dial phones and allow a four-second pause.) 6. Listen for two short tones to confirm your entry. You may continue entering all the numbers on your list at this time or enter them one at a time later.

Spontaneous Call Waiting* You will always know who is calling

Spontaneous Call Waiting is a feature that will display the telephone number of an incoming call – even when you’re on the phone. With Spontaneous Call Wait- ing you will never have to wonder whether or not you’re missing an important call. How To Use Spontaneous Call Waiting 1. When you are on the phone with the first call, a special tone will sound to alert you of an incoming call. 2. With proper equipment, the telephone number of the person on the incoming call will automatically appear on your display screen. 3. If you choose to answer the call, the number will remain on the screen until you or the caller hang up.

Three-Way Calling Two’s company, but Three-Way Calling is a necessity

With Three-Way Calling, you can talk to two different parties – in two different places – at the same time. You’ll save time and hassles by being able to make arrangements and solve problems faster. It doesn’t matter if the calls are local or long distance, you’ll enjoy two-party clarity in every three-way conference call! How To Use Three-Way Calling 1. Call the first person and put them on hold by depressing the switch hook for one second. 2. Wait for the dial tone and then enter the phone number of the party you want to add to the conversation. When that person answers, you can talk privately before adding the first person back into the conversation. If the line is busy or no one answers, depress the switch hook twice to return to your first call. 3. Depress the switch hook for one second to add the first person. Now all three people will be on the line at the same time. 4. To disconnect either call, ask one person to hang up. You can then continue the conversation with the remaining party. 5. When you hang up, all callers will be disconnected. User Transfer Now, everyone can talk on the same call at the same time Now there’s no reason to run around trying to get the right parties on the same phone call at the same time. User Transfer allows you to transfer a call to another number, and then drop out of the call, leaving the other callers connected. You must have Three-Way Calling. How To Use User Transfer 1. Pick up the handset and place your call. 2. To transfer call to another number, press “FLASH” or “TRANSFER” or depress the switch hook button on phone. 3. Dial the second number and wait for answer. When you hang up, both callers will be connected.

National Do Not Call Registry The National Do Not Call Registry is a registry designed to eliminate annoying telemarketing and unsolicited sales calls to your home. This service is free of charge and you can add your name and telephone number to the registry by calling the Toll Free number: 1-888-382-1222 or simply visiting the website at www.donotcall.gov to register online. This will register your telephone number with a national database. Do Not Call Notification for Telephone Solicitors If telephone solicitations or telemarketing calls are made in the course of your business, it is important that you be aware of, and comply with, the rules and regulations governing such activities. As you may know, the Federal Communications Commission (FCC) and the Federal Trade Commission (FTC) jointly established the National Do Not Call Registry (Registry), a database containing the residential telephone numbers and wireless numbers of consumers who want to avoid telephone solicitations. The national Registry rules apply to all telemarketers and sellers engaged in telephone solicitations and cover all commercial interstate and intrastate marketing calls. It is illegal for telemarketers and sellers to call a number listed on the Registry, unless they fall under one of the exceptions established in FCC/FTC rules, such as exceptions for non-profit charitable organizations or for prior business relationships. (The rules should be carefully reviewed before relying on one of the exceptions.) Telemarketers and sellers are required to access the national Registry at least once every 31 days and drop from their call lists the telephone numbers of consumers who have registered. They are also required to have a written policy, available upon demand, for maintaining a do-not-call list and train personnel engaged in any aspect of telemarketing about the existence and use of the do-not-call list.
