We are committed to providing you with the best possible service. Please read this page before you call for repairs to minimize the inconvenience and costs of a service outage.

BEFORE YOU CALL Try to locate the problem before you call MTCO.

1. Unplug all telephones (and other devices) from your telephone line jacks . Reinstall the equipment one piece at a time to determine which piece is creating the problem. If your phone equipment is the problem, contact the manufacturer or the store where you bought it. 2. Plug in a single line phone directly into the modular jack that’s enclosed in the network interface device that is installed on the outside of your home. By plugging into this modular jack, you will remove any problems that could be caused from the internal wiring in your house. If the trouble is in the premise wiring or jacks, MTCO will repair it if you cannot, but you will pay for the repair unless you have an MTCO “Home Guard Line Minder” Plan.

Machine, etc.) Network Interface All telephone devices that also plug into A.C. power are especially susceptible to damage during an electrical storm. Please be sure to disconnect these devices from the telephone line jack before calling to report a line that is out of order. You will be charged for a service call if the problem is found to be in your customer-owned equipment. FAIR RESOLUTION OF COMPLAINTS We believe that our customers deserve to be treated with respect, courtesy and honesty. If you have a complaint or grievance about our telephone services, billing or other policies and procedures, we want to hear from you so that the matter can be settled fairly. Our service representatives and technicians have the responsibility to answer your questions in a professional manner. Establishing Service

APPLICATIONS FOR SERVICE A customer can initially establish service by coming to the Marseilles Telephone Company, 244 Lincoln Street in Marseilles during our normal business hours (8 a.m.-5 p.m., Mon.-Fri.). Requests for additional services can be made in person or by calling our office at 1-815-795-5161 during the same business hours. ESTABLISHMENT OF CREDIT MTCO, in order to assure the payment of its charges, will require applicants and customers to establish and maintain credit. The establishment or reestablishment of credit shall not relieve the customer from responsibility for payments, and shall in no way modify the tariff provisions regarding disconnection and termination of service for failure to pay bills. MTCO may refuse to furnish service to an applicant who has not established credit, or may require a security deposit or guaranty. If a customer fails to keep current with his account as required by the Company, service may be discontinued after delivery of a written notice of intention to disconnect service. If a deposit is required, it will be refunded upon establishing a history of paying the monthly bill by the due date. If service is disconnected, the deposit will be applied to the final bill. SCHEDULE FOR PAYMENT Your telephone bill will be mailed to you on the 1st day of each month. This bill is due when rendered and is past due by the 21st of each month and must be paid in full. If payment in full is not made by the 21st, you will be assessed a late payment charge of 1.5% or $10.00, whichever is greater, on your unpaid balance. This charge will be added to your next month’s statement. Also, failing to pay in full by the 21st may result in your service being disconnected for non-payment. You will receive a notice in the mail informing you of the situation. If your service is disconnected for non-payment, you must pay your bill in full plus a $10.00 restoral of service charge. MTCO may also require a security deposit in the amount of 2 times your average monthly bill before service can be re-established. IF YOU CAN’T PAY ON TIME Please call a service representative at 1-815-795-5161. We may be able to make payment arrangements. Late payments will impair your credit standing with us, and an additional deposit may be required. Repeated late payments may result in disconnection.

Billing & Payments

UNDERSTANDING YOUR BILL All charges for services, including local and long distance, are billed on a monthly basis. Your local service charge is billed one month in advance. Your long distance and other service charges will be billed for the past month. Your “Billing Date” is shown at the top of page 1 along with the “Total” you owe and the “Past Due” date. A summary of your current charges is also printed on this page. On page 2 you will find the detailed description of your current long distance charges, unless these charges are billed by another long distance carrier. PAYING YOUR BILL You are responsible for the payment of all charges, including any calls originating from or accepted at your premises and billed to you through your calling card. You may pay by mail or in person the total amount you owe before the “Past Due” date. Please note your Account Number on your check and return it with the top portion of your bill in the return envelope. You also can log into www.mtco.com and click on eBill to pay. Any check returned due to insufficient funds will result in a non-sufficient fund fee.

Rights & Responsibilities

FRAUD For your protection, new equipment and procedures enable the Telephone Company to detect and investigate fraudulent calls. State law provides that no person shall defraud or attempt to defraud the Telephone Company of its lawful charges. Violators, upon conviction, are subject to imprisonment for up to one year, a fine of up to $1000, or both. Any person who charges services to your number or calling card without your permission is subject to prosecution for violating both federal and state laws. OBSCENE & HARASSING CALLERS It is a criminal offense for anyone to make an obscene, harassing or threatening phone call. Anyone convicted of such an offense is subject to a fine of up to $500 or six months in prison or both. If you receive such a call: • Hang up immediately. Nothing is gained by staying on the line after the first obscene word or if silence lasts past your second “hello.” • Give no personal information such as your name or address. • Advise your children to give no information to strangers. If you’re not at home, they should say “Mother/Father can’t come to the phone right now.” • If the calls persist, contact the State’s Attorney and/or MTCO to file a complaint. • You may want to consider initiating a customer originated trace which will be needed if legal action is to be taken. Please contact our office for further information (see p. 17) about initiating this trace. 900 AND 976 CHARGES Though 900 and 976 numbers do offer a wide variety of information, there is a charge for every call placed. In addition, some 800 numbers can lead to billable charges. Residence and business customers can block all 900 and 976 numbers through an optional service. For further information, please contact our business office. UNLAWFUL WIRETAPPING It is a criminal offense to intercept a phone conversation unless you get prior permission from one of the people participating in the call. Law enforcement officers with court orders can wiretap without prior consent.

RECORDING OF CONVERSATIONS Recording a conversation is not legally permitted unless: • All parties consent verbally or in writing, or • A distinctive tone is heard every 15 seconds during the recording, or • The party which is intending to record the conversation notifies the other party at the beginning of the conversation that the call is being recorded. Law enforcement officers with court orders and broadcast stations who are recording for broadcast purpose are exempted from these restrictions. CUSTOMER PROVIDED EQUIPMENT Customers may connect customer-owned equipment to the Telephone Company’s facilities under certain conditions specified by the Federal Commerce Commission. Generally, these conditions require that the connection be made through FCC approved jacks or other connecting arrangements. The Telephone Company does not repair customer-provided equipment. The customer will be billed a service charge for each repair visit made by the Company, if it is determined that the service difficulty resulted from the use of customer-provided equipment. BURIED CABLE LOCATIONS Before you (or your contractors) do any digging, trenching or landscaping on or near your property, please call JULIE (Joint Utility Locating Information for Excavators) for cable locations in the area. You are legally responsible to call the JULIE toll free number 800-892-0123 to contact utility companies in the area. This call should be placed at least 48 hours before the digging begins, but even short notice is better than neglecting to call at all. Customers will be held responsible for the repair or replacement of damaged telephone cable and other MTCO property. Help With Basic Phone Service Basic and enhanced telecommunication services are offered to all consumers in the Marseilles Telephone Company service territory at the rates, terms and conditions specified in company price lists. Eligible low-income households can receive financial assistance with telephone installation and basic local telephone service. The Illinois Universal Telephone Service Assistance Program (UTSAP) offers a one-time discount of $16.00 off the cost of telephone installation. The Federal Lifeline Program provides a $5.25 monthly discount on basic telephone service or a $9.25 monthly discount on broadband Internet service. If you have any questions regarding our services or these specified telephone/Internet assistance programs, please contact the Marseilles Telephone Company at 800-227-5161 or visit our business office at 244 Lincoln Street in Marseilles, Illinois. To qualify for assistance, households must participate in one of the following governmental programs: Public Housing Assistance (FPHA Section 8), Supplemental Nutrition Assistance Program (SNAP), Supplemental Security Income (SSI), Medicaid, or Veterans Pension and Survivor Benefit or have a household income at or below 135% of the federal poverty guidelines.

Restricted Call Registry Illinois law protects residents from certain unwanted telemarketing calls. For more information about the Restricted Call Registry and how to enroll, visit the Illinois Commerce Commission web site www.icc.illinois.gov , or call 888 382-1222 and for TTY access, 866-290-4326.

General Information

TELEPHONE DIRECTORIES Telephone directories are annually distributed to our customers without charge. These directories provide the general information that we believe will help our customers to efficiently use our telecommunication services. We will gladly provide additional directories if needed. DUAL LISTINGS (in Directory) Dual listings are available to residential customers which will permit the given names of a couple to appear after the family surname in the following manner: Brownfield Mark & Michelle This option is available to any two residential customers with the same surname, address and telephone number. DIRECTORY ERRORS OR OMISSIONS We do our best to provide an accurate directory for the benefit of all our customers. On occasion, however, errors do occur and should be promptly reported to our business office at 1-815-795-5161. Any errors, omissions or concerns about the Yellow Page advertising section of the directory should be reported directly to your representative at DPS Media at 866-221-4300. MTCO is not liable for any errors or omissions in the compiling, printing, or distribution of its directories. TELEPHONE NUMBERS MTCO reserves the right to assign telephone numbers and equipment, and to make any changes when necessary. Of course, we will always do our best to consider our customers’ concerns in these matters. TELEPHONE SAFETY The telephone is one of the safest appliances in your business or home. In a few situations, however, a telephone user needs to exercise caution. • Don’t use your telephone when you are in a bathtub, shower or swimming pool. • Avoid using your phone during electrical storms. • Don’t use a telephone near a suspected gas leak.

• In the event of a flood, tornado, or other disaster, avoid using your phone except for an emergency. REPAIR SERVICES We are committed to providing you with the best possible service. Please report any service needs promptly to our office at 1-815-795-5161. If you are having trouble with your own line, be sure to read the information on page 2 before calling. If you’re calling after business hours, clearly describe the difficulty and follow voice mail prompts so that we can effectively respond to emergencies. If you notice damaged underground cable pedestals on or near your property, please contact us so that the inconveniences of a service outage can be avoided. MAINTENANCE OF MTCO FACILITIES Facilities furnished by MTCO remain the property of the Company until transferred or abandoned. Customers shall provide employees of the Company access to those facilities, at all reasonable times for the purpose of installing, repairing, maintaining or otherwise servicing the facilities. USE OF RESIDENTIAL LINES FOR BUSINESS PURPOSES Residential lines are installed with the understanding that they will be used for normal social or domestic purposes. A residential line will be changed to a business service classification if used for business purposes, or if the residential number is advertised in connection with the sale of products or services. TARIFFS Detailed information concerning customer billing, due dates of bills, denial of service, restoral of service and security deposits can be found in the tariff filed on February 24, 1975 with the Illinois Commerce Commission. CUSTOMER INFORMATION Customer information which is maintained by MTCO is protected by law and can be released only through a court order.
