The Arkansas Public Service Commission requires utilities to provide the following information. Public Service Commission rules are available to the customer on request at our business office. Service Application, Changes and Discontinuances Applications for new service, changes in existing service or discontinuance of service may be made at our Business Office in person, by telephone or by mail. Charges for installation of new service and changes in existing service will vary depending on the type of work involved. Rates All tariffed rates for service are on file with the Arkansas Public Service Commission and are available to the customer on request at our Business Office. Information related to this Bundle Service is The Lifeline Assistance Program provides for a monthly discount off any qualified service offered by Madison County Telephone Company. The discount applies to monthly recurring rates for qualifying residential customers and is limited to one per household at the customer’s primary residence. To qualify the customer must partic ipate in one of the listed programs approved by the Federal Communications Commission. • Medicaid available through the Business Office. • Veteran’s Pension or Survivor’s Pension Benefit • If your household income is at or below 135% of the federal poverty guideline for Arkansas customers The Affordable Connectivity Program Madison County Telephone participates in the Affordable Connectivity Program (ACP), which is an FCC program that helps connect families and households struggling to afford Internet service. The ACP provides up to $30/month discount for broadband service and is limited to one monthly service discount per household. Who is Eligible? A household is eligible if a member of the house hold meets at least one of the criteria below: • Has an income that is at or below 200% of the Federal Poverty Guidelines; • Participates in certain assistance programs, such as SNAP, Medicaid, Federal Public Housing Assistance, SSI, WIC, or Lifeline; • Participates in one of several Tribal specific programs, such as Bureau of Indian Affairs General Assistance, Tribal Head Start (only households meeting the relevant income qualifying standard), Tribal TANF, or Food Distribution Program on Indian Reservations; • Is approved to receive benefits under the free and reduced-price school lunch program or the school breakfast program, including through the USDA Community Eligibility Provision; • Received a Federal Pell Grant during the cur rent award year; or • Meets the eligibility criteria for a participating broadband provider’s existing low-income program. Contact MCTC to select a plan and have the discount applied to your bill.
