Customer Information

Lifeline Assistance Program The Lifeline Assistance Program provides for a monthly discount off any qualified service offered by Madison County Telephone Company. The discount applies to monthly recurring rates for qualifying residential customers and is limited to one line per household at the customer’s primary residence. The amount of the discount varies based on the type of service received. This program discount is non-transferable. To qualify the customer must participate in one of the listed programs approved by the Federal Communications Commission. • Medicaid

The Arkansas Public Service Commission requires utilities to provide the following information. Public Service Commission rules are available to the customer on request at our business office. Service Application, Changes and Discontinuances Applications for new service, changes in existing service or discontinuance of service may be made at our Business Office in person, by telephone or by mail. Charges for installation of new service and changes in existing service will vary depending on the type of work involved. Rates All tariffed rates for service are on file with the Arkansas Public Service Commission and are available to the customer on request at our Business Office. Customer Information

Billing Procedures Bills are issued at the end of each month and include local service for one month in advance, appropriate charges for long-distance service and any other charges incurred since the previous billing. All bills are payable on receipt, and past due 23 days after the date the bill is mailed. Customers whose bills are not paid when due will receive a late payment charge and a “Shut-Off Notice” stating the date that service will be terminated. Once suspended, the full delinquent amount, plus a reconnect charge, must be paid before service will be restored. A residential customer may designate a third party to receive a copy of the “Shut-Off Notice.” Deposit A deposit may be required for new applicants for service to guarantee payment for this service. If desired, the deposit may be paid in two equal installments. In lieu of a deposit, the Company will accept written guaranty of a qualifying third party. The amount of the deposit generally will not exceed the charges for one month’s long- distance. Interest on deposits is credited to customers’ accounts annually. Deposits will be refunded after twelve months if the following two conditions are met during the preceding year: 1. The customer has, for a consecutive twelve month period, paid all bills prior to the due date; and, 2. The customer has rendered no check that was dishonored for reason other than bank error. For existing customers, a deposit will be required (or increased) if any of the following conditions occur during the preceding twelve month period: 1. Servicetothecustomerhasbeendiscontinued for failure to pay a delinquent account.

2. The customer has rendered two insufficient funds checks to the company in the preceding twelve months. 3. The customer has failed to pay his bill by the due date twice in the preceding twelve months. 4. The customer has misrepresented his identity for the purpose of obtaining service. 5. The customer has engaged in unauthorized or fraudulent use, or procurement of service, or tampering with wires or other telephone company equipment. When final discontinuation of service is made by the company, the amount of the deposit, with interest, will be applied to any amount owed to the telephone company. Payment of Bills By mail – P.O. Automatic Payment We have an automatic bill payment plan. Call us for more information. Delayed Payment Agreements Delayed Payment Agreements are available to customers who are unable to pay a bill in full. If a customer has not broken an agreement in the last 12 months, he may pay ¼ of the past due amount down by the third business day and pay the balance in installments over the next three months. Current bills and installments must be paid by the due dates or service will be subject to suspension.

Customer Information

Use of Residence Telephone for Business Purpose Residence telephones are installed with the understanding that they will be used for normal social or domestic purposes. A residence service will be changed to busi- ness service if used primarily or substantially for business purposes, or if the residence telephone number is advertised in connection with the sale of products or services, and the subscriber has no business service elsewhere within our system. You must dial 1+ or 0+ Area Code and Seven Digit Telephone Number for all long-distance calls. Delay of Termination of Service on Grounds of Serious Illness A utility shall postpone termination of service to a residential customer, or reconnect previously terminated service, for a reasonable time up to thirty (30) days if the customer presents a certificate from a physician stating it is likely that termination of service will either aggravate a serious illness or give rise to a substantial risk of death or a grave impairment of the health of the customer, of a member of the customer’s family or of another permanent resident of the premises where service is rendered. The certificate shall identify the medical emergency, specify the effect of termination of service, and specify the time during which termination of service will aggravate the illness. Delay of Termination of Service for Identified Elderly or Handicapped When identified elderly (65 or older) or handicapped customers tell the company they cannot pay a bill on time, the company will offer a delayed payment agreement, provide the name of federal, state, and local bill payment assistance agencies and explain the right to third party notice before suspension of service. Extended Due Date Plan The Extended Due Date Plan is available to qualifying residential customers and is intended to enable the company to change a customer’s payment due date, at the customer’s request, to

coincide with or follow the customer’s receipt of income. Customers who qualify under this plan and pay by the new date will not be considered late on their bill payment. Customer Complaints / PSC Info Contact our Business Office if you have a complaint concerning your bill or service. Late Payment Charges If payment is not received by the due date on your bill, a penalty charge of 10% of the first $30 of the overdue portion and 2% of the remainder of the overdue amount will be charged to you. Telephone Number The subscriber has no property rights to the telephone number or any right to continuance of service throughout any particular office. The telephone company may change the telephone number or the central office designation, or both, of a subscriber whenever it deems it desirable in the conduct of its business to do so. Do Not Call A utility shall inform their subscribers of the opportunity to register or revoke registrations on the National Do-Not-Call database at www.donotcall.gov or by calling 1-888-382-1222.

If You Need to Call Our Repair Service Please be prepared to answer these questions when you call: 1. What is the telephone number of the line in trouble? 2. Does the problem occur on all your phones? 3. Do you have a dial tone? 4. Is noise the problem on the line? 5. Can you call out? 6. Can you receive calls? 7. How often does the problem occur? 8. Is the problem on local and/or long-distance calls? 9. What number can we call to reach you during the repair? Once you call our repair service, we will see to it that your repairs are made as quickly as pos- sible. Some will be made at no charge to you. Other repairs may require a fee, depending upon the source of the trouble. If the Trouble is In Your Telephone Set Fees will apply if you ask us to make a repair visit and the trouble is found in your telephone set(s) or other phone equipment and you do not have a Maintenance Agreement. If the Trouble is In Your Inside Wiring Fees will apply if you ask us to make a repair visit and the trouble is found in your telephone wiring inside your home or place of business. To repair the damage, your options are to hire us, do it yourself, or seek outside assistance. If you already have our Inside Wiring Agreement, we can repair the inside wiring and jacks at no extra cost. Certain restrictions may apply. If the Trouble is In Our Outside Lines There are absolutely no charges for any repairs in our telephone lines, which are located outside your home or place of business. TO CONTACT OUR REPAIR SERVICE AFTER HOURS CALL 738-6611 – A worry-free option to inside wiring repairs . You can avoid unexpected charges for telephone inside wire repair by subscribing in advance to our Inside Wiring Agreement. A small monthly fee protects you from normal repair charges on the telephone wiring and jacks inside your home or business. For more information, call our busi- ness office. The number is 738-2121.

We do everything possible to make sure your phone service remains the best. And we try just as hard to fix any problems that do occur as quickly as possible. We make repairs to the phone lines outside your home or business at no charge to you. Unless you have our Inside Wiring Agreement, any repairs to your inside phone lines require an additional charge. Because some of your repair assistance requires a fee, we work with you to help determine the source of the problem, whenever possible. Listed below are a few things to do before you call our repair service. • Look for symptoms on your telephone set(s) like cracked plastic housing or a broken or frayed cord. • If you have just one phone, unplug it and plug in a borrowed phone. If the borrowed one works, the problem is in the phone set and not the phone line. • If you have more than one phone, make sure each phone’s receiver is on the hook. If none of them are off the hook, unplug every phone. Then plug each phone back into any wall jack one at a time, and check each phone for a dial tone as you go. The telephone set that causes the line to go dead is the one needing repair. • If your phone or phones appear to be working, take a working phone to your telephone net- work interface box. This box is usually located on the outside wall of your home or business. It is located where our outside lines and your inside telephone wiring connect, or “interface.” Open the network interface box and locate the “test jack” inside. Remove the modular plug from the test jack and insert the phone jack from your phone. If the same problem occurs when you plug your phone into the test jack, the problem is our outside lines. Call our repair service. The number is 738-2121. If the problem disappears when you use the test jack, the problem is in your inside telephone wiring or your telephone set.

General Information & Helpful Tips

Directory Accuracy Every effort is made to make the directory as accurate as possible. In case of error or omis- sion, the liability of the company, if any, is limited to the amount of actual impairment of the cus- tomer’s service and may not in any event exceed the amount paid for the service during the period covered by the directory in which the error or omission occurred. Classification of Service A residence telephone may be changed to the business service classification if the use of such service can be considered as being primarily for business. Such change may be made if the residence telephone number is advertised in connection with the sale of products or services if the subscriber has no business service within It is illegal for a person to charge any calls to another’s number or Calling Card without their permission. Persons placing such calls to avoid payment for service are subject to prosecution, and if convicted, are subject to imprisonment and/or a fine. Harassment by Telephone It is a crime under both state and federal laws for anyone to make obscene or harassing telephone calls. These laws have penalties of imprisonment and/or a fine. Annoying Calls If you ever receive obscene, abusive, harassing or threatening calls, follow these suggestions: 1. Hang up at the first obscene word. Hang up if the caller remains silent the second time you say hello. Remember, you control your telephone , not the person calling. 2. Do not give any information, such as your name or address, until the caller has been properly identified. 3. Advise your children not to give out any information. If you are not at home, those answering your telephone should be instructed to say, “They’re busy right now, may I take a message?” 4. If calls persist, call the telephone company. the same exchange area. Fraudulent Billing Can Lead to Prosecution

Telephone Safety The telephone is one of the safest appliances in your home and office, but there are a few situations where a telephone user should be cautious. • Do not use the telephone while you are in the bathtub, shower or swimming pool. Putting the telephone in water could cause a shock. • Avoid using the telephone during electrical storms in your immediate area. Urgent calls should be brief. Madison County Telephone Company uses protective measures to limit electrical surges from entering your home, but absolute protection from lightning is impossible. • If you suspect a gas leak, report it immediate- ly, but use a telephone away from the area in question. The telephone’s electrical contact could create a tiny spark when you dial. While unlikely, it is possible that this spark could ignite heavy concentrations of gas. Tariff Provisions The communications services furnished by the telephone company are public utility services which are offered by the company subject to the rules, regulations and charges set forth in its tariffs. Abuse of Telephone Service The tariffs provide for suspension or termination of telephone service for abuse. Abuse of service

includes the use of service or facilities of the telephone company to transmit a mes- sage or to locate a person or otherwise to give or obtain information without payment of a long-distance charge. Guarding Against Fraud

For your protection, new equipment and procedures enable the telephone company to detect and investigate fraudulent calls. State and Federal laws provide that no person shall defraud or attempt to defraud the telephone company of its lawful charges. Violators, upon conviction are subject to imprisonment or fine or both.

breaking into your line. To CANCEL CALL WAITING, simply dial an access code prior to placing your call. If you have THREE-WAY CALLING, you can CANCEL CALL WAITING during a call. In either case, the cancel request stays in effect for the duration of the call and any incoming calls receive a busy signal. This feature is especially helpful if you use a computer to send or receive data transmissions through your phone line. To Cancel Call Waiting Before Calling: • Lift the handset and listen for the dial tone. • Dial the deactivation code 7-0-#, and listen for a special dial tone. (On a rotary dial phone, dial 7-0 and wait four seconds.) • Now, dial the number you wish to call. When you disconnect your call, CALL WAITING is automatically reactivated. To Cancel Call Waiting During a Call: • You must have THREE-WAY CALLING to CANCEL CALL WAITING during a call. • Place your current call on hold by depressing the switchhook/plunger for one second. • Listen for the dial tone, then dial 7-0-#. (On a rotary dial phone, dial 7-0 and wait four seconds.) • Listen for a special “beep-beep” tone, then depress the switchhook/plunger for one second and return to your call. • When you disconnect your call, CALL WAITING is automatically reactivated. Help Line (Direct Line) This feature sometimes called Direct Connect or Warm Line, provides automatic placement of a call to a pre-selected Directory Number upon detection of a call origination. No dialing is required by the calling party to complete the call. The service may be used for intraoffice or interoffice calls. Calls can also be made by dial- ing when critical timeout is used. The feature does not affect terminations to the line. Teen Service Individual Ring Teen Service enables two telephone numbers to be assigned to a single-party line. Each tele- phone number is assigned a unique ringing pat- tern so the called party can determine the nature of the call.

Call Waiting If you have an incoming call while you are already on the line, the Call Waiting feature will alert you. You may then put the first party on hold while you catch your second call. It will greatly reduce missed calls due to a busy phone! Here is how your Call Waiting feature alerts you: • A beep tone tells you another call is waiting. Only you hear this tone. • Another reminder tone will be heard 10 seconds later if the call remains unanswered. • The second caller hears the normal ringing tone only. To Answer the Second Call: • Depress the switchhook/plunger for about a second, to place your first call on hold. • You will automatically be connected with the second caller. To Alternate Between Calls: • By depressing the switchhook/plunger for about a second, you may alternate between calls. • Each conversation is private and cannot be heard by the other caller. To Terminate Either Call: • Simply hang up. • Your telephone will then ring. • When you answer it, you will be connected

with the other caller. Cancel Call Waiting

Now when you have a call that you don’t want interrupted, you can prevent a second call from

Call Forwarding This feature means your phone can transfer calls to another number of your choice. Call Forwarding is great for the business person who wants to catch after hours business calls at home or for anyone who doesn’t want to miss an important call. To Forward Your Calls: • Lift the receiver and listen for dial tone. • Dial 72 . • Depress the # button (with a rotary dial, skip this step and wait for a 4-second pause). • Again, listen for dial tone. • Now, dial the number where you wish your calls forwarded. (Speed Calling codes may be used if you also have this feature.) • When someone answers at the forwarded number, Call Forwarding feature is in effect. If there is no answer, or the line is busy, hang up and repeat the previous steps. If you do this within two minutes, you will hear two beeps meaning your Call Forwarding feature is in effect. Once you have activated Call Forwarding, the phone will make one short ring each time a call is forwarded. However, you can still make out- going calls from this phone. If you wish to change the number your calls are being transferred to, just discontinue the first (see below) and redirect your calls using the above steps. To Disconnect Call Forwarding: • Lift the receiver and listen for dial tone. • Dial 73 . • Depress the # button (with rotary dial, skip this step and wait for a 4-second pause). • Listen for two beeps. Call Forwarding is now disconnected.

Call Forward Busy This feature means you can transfer calls to another number of your choice when your phone is busy. Call Forward Busy is great for the phone that has a computer online. To Activate Call Forward Busy: • Lift the receiver and listen for dial tone. • Dial *90 . • Again, listen for dial tone. • Now dial the number where you wish your calls forwarded. (Speed calling codes may be used if you also have this feature.) • When someone answers at the forwarded number, Call Forward Busy feature is in effect. If there is no answer, or the line is busy, hang up and repeat the previous steps. If you do this with- in two minutes, you will hear two beeps meaning your Call Forward Busy feature is in effect. To Discontinue Call Forward Busy: • Lift the receiver and listen for dial tone. • Dial *91 . • Listen for two beeps. • Call Forward Busy is now disconnected. Call Forward Remote Access (CFRA) All Exchanges (232, 559, 665, 738) will dial the same number. To Activate Call Forward from any line other than your own: • Call 738-2999 (It will beep, then give you a dial tone). • Enter your number ###-####, then enter your assigned PIN ##### (It will beep, then give you a dial tone). • Dial 72# (It will beep, then give you a dial tone). • Enter the number youwant to forward to. Then hit # and hang up. (Remember, if it is long distance, to put 1+ area code). To Deactivate Call Forward from any line other than your own: • Call 738-2999 (It will beep, then give you a dial tone). • Enter your number ###-####, then enter your assigned PIN ##### (It will beep, then give you a dial tone). • Dial 73# (It will beep, then give you a dial tone). • Hang Up.

To Establish Your Speed Calling List: • Listen for dial tone and dial 74 (for 8 code calling) or 75 (for 30 code calling). • Then depress the # button. (With rotary dial phone, skip this step and allow a 4-second pause.) • Dial one of the 8 one-digit access code numbers (2-9) for 8 code speed calling or dial one of the 30 two-digit access code numbers (20-49) for 30 code speed calling. • Dial the complete local or long-distance number you are assigning to that code. • Listen for confirmation tone to indicate your Speed Calling number is established. To Change Your Speed Calling List: • Simply follow the same steps for establishing your list. A new number automatically can- cels out the corresponding existing number. To Use Your Speed Calling Feature: • Lift the receiver and listen for dial tone. • Dial the appropriate one-digit access number (2-9) for 8 code speed calling or the two-digit access number (20-49) for 30 code speed calling. • Push the # button. (With rotary dial phone skip this step and allow a 4-second pause for your number to be connected.) Customer Local Area Signaling Service (CLASS) is a group of capabilities that use industry standard protocols to effectively manage call flow. This is accomplished by the central office development of technology which routes the calling party’s telephone number from the cen- tral office serving the called party. That tele- phone number is held in network giving the called party options including answering the call and calling back to the originator. Automatic Callback Automatic Callback redials a busy number and keeps dialing the busy number for up to 30 minutes. When the number is available, the call- er will be alerted by a special ring. After picking up the receiver, the caller is connected to the number that was dialed.

Three-Way Calling This feature lets you turn an everyday two-way call into a three-way conversation. It’s easy! To Add a Third Party: • First, to hold your existing call, depress the switchhook/plunger for about a second. • Then dial the third number. (Speed calling codes may be used if you also have this feature.) • When the third party answers, you may talk privately before completing your three-way connection. • With your third party on the line, depress the switchhook/plunger for a second to add the holding party. Your Three-Way call is now in effect. If for some reason the call to the third party is not completed, depress the switchhook/plunger twice to get back to your held party. To Disconnect the Third Party: • Depress the switchhook/plunger for about a second. You will now have only the original party on the line. To Disconnect Completely: • Simply hang up. • If either of the two parties hangs up, you can continue to talk to the one remaining. Speed Calling This feature lets you reach frequently called local or long-distance numbers by dialing a one or two digit code.

Automatic Recall Automatic Recall dials the last number that called the subscriber, whether the subscriber answered the call or not, and keeps dialing the number for up to 30 minutes if it is busy. A spe- cial ring alerts the subscriber when the called number is available. Call Number Delivery (Caller ID) Call Number Delivery allows the subscriber to view the telephone number of the calling party before answering the telephone. A number will appear on a special display unit after the first ring. A Caller ID Unit can store from 1 to 100 numbers so that calls can be returned at a more convenient time. In some cases, the number may be blocked because the originating caller has elected to block his or her number from being delivered, or the serving office is not equipped to pass the number, or the call is from out of state. Call Number Delivery Blocking (Caller ID Blocking) Call Number Delivery Blocking allows the subscriber to restrict his or her number from being delivered to the called party. This feature allows the subscriber the privacy desired if he or she has a non-published or unlisted number. Dial *67 before each call to block number. Call Name Delivery Blocking Call Name Delivery Blocking allows the sub- scriber to restrict his or her name from being delivered to the called party. This feature allows the subscriber the privacy desired if he or she has non-published or unlisted number. Dial *98 before each call to block name. Customer Originated Trace This feature enables the customer to initiate an automatic trace of the last call received. This feature is provided to all subscribers of the Madison County Telephone Company. The results of the trace are recorded in the telephone company’s switch and will be furnished only to legally constituted authorities and never to the customer.

Selective Call Acceptance Selective Call Acceptance allows the subscriber to receive calls from a list of not more than 32 subscribers’ specified directory numbers. Selected Call Forwarding This is a call management feature that allows the customer to transfer up to 32 selected calls to another telephone number. Selective Call Rejection This incoming call management feature allows the customer to define a list of up to 32 calling directory numbers to be screened. Distinctive Ringing-Call Waiting Distinctive Ringing provides a different ringing pattern for up to 32 selected numbers. The service also provides a different call waiting tone for those selected numbers if the subscriber also has call waiting. Equipment Information How to connect or disconnect your phones: To install your phone, match to plug at the end of the cord with the jack, and insert. To disconnect your phone, squeeze the clip on the plug and pull it out of the jack. A special converter can be pro- vided to permanently change the non-standard jacks into modular out- lets to use with your modular cord. Plug the converter into the jacks, then plug the cord into the converter. To connect your wall phone, match the holes in the back of the phone with the rivets and the wall mount- ing. Push the phone lightly against the mounting and slide it down into place. To disconnect, slide the phone up to lift it off the rivets, then pull it away from the wall. If your house or apartment has “hard-wired” telephone outlets, you may be able to save time and money on installation by changing them to mod- ular “plug-in” outlets yourself with a modular jack converter.
