Supporting Our Communities

GRM Networks ® Gives Back GRM Networks Community Improvement Grant Program Community organizations within the GRM Networks service area are eligible to apply for the GRM Networks Community Improvement Grant Program. Groups must submit an application describing a project that will better their community, and are eligible for grants ranging from $500 to $1,500. Applications are available at grm.net/ grants . Aureon Charity Grant Program and FRS Community Grant Program Two partners of GRM Networks also offer grant programs to our members. Aureon invites public and private non-profit agencies to submit proposals for consideration in assisting in charity drives and/ or funding of a specific project for their respective communities. Only those communities served by Aureon participating telecommunications companies in Iowa are eligible to submit proposals. As part of its ongoing commitment to rural communities across the country, the Foundation for Rural Service provides annual grants for communities served by members of NTCA - The Rural Broadband Association. The goal of this program is to support local efforts to build and sustain a high quality of life in rural America. More information on both grant programs can be found at grm.net/grants .

GRM Networks Scholarship Program As a local cooperative business, GRM Networks places great importance on the education of students in our service area. To provide support to the students in rural Iowa and Missouri, GRM Networks has created the GRM Networks Scholarship program. GRM Networks will award up to $5,500 to graduating high school seniors whose parents or guardians are members of GRM Networks in good standing at the time of the application deadline. Instructions on how to apply for the scholarship are posted at grm.net/scholarships .

FRS/GRM Networks Scholarship High school seniors whose parents or guardians are members of GRM Networks can apply for The Foundation for Rural Service (FRS) scholarships. FRS will fund $2,000 of each scholarship, with GRM Networks providing a $500 match. FRS also provides a number of named scholarship opportunities. More information can be found at grm.net/scholarships . Iowa Communications Alliance and Aureon Scholarship Program GRM Networks and SCC Networks ® are proud to partner with the Iowa Communications Alliance and Aureon to make these scholarships available to students in the Iowa GRM Networks and SCC Networks service area. FRS/GRM Networks Youth Tour to Washington, D.C. Every year students who are juniors in high school and whose parents or legal guardians are members of GRM Networks have the opportunity to apply for an all-expense paid trip to Washington, D.C. The trip offers an exciting and fast paced tour of our nation’s capital. Applicants are interviewed by a panel of GRM Networks board members. Applications are available in January at grm.net/youthtour , at the local business offices and from high school guidance counselors.

Customer Rights And Responsibilities

When Will I Receive My Bill? GRM Networks ® and SCC Networks ® mail bills at or near the first day of each month. You should receive your itemized bill within the first few days of the month. If you do not receive your bill, please call our business office by dialing 777 from your landline phone or by calling one of the office phone numbers on page 1; we will mail you a copy of your current bill at no charge. Relay Service for hearing and speech impaired customers can be reached by dialing 711, and a Communications Agent will assist you in reaching our offices. How Can I Pay My Bill? Sign up for eBill to save time, and view and pay your bill online - visit www.grm.net/ebill/ or contact your local business office. You can also sign up for Autopay and pay your bill via automatic bank deduction, debit card or credit card. Another option is to download our convenient app on your mobile device - search for GRM Networks in the Apple App Store or Google Play. You can use the app to pay your bill, set up Autopay, and view your invoices and payment history. Other payment options include mailing your payment as directed on your bill, or you may pay in person at your local business office. There is a charge for each returned check. When Does My Bill Become Delinquent? Your payment due date appears on your bill. Your payment is considered past due if not paid by that date. How Can I Dispute A Charge On My Bill? If you believe a charge appearing on your bill is in error, you can inquire and/ or dispute a charge by calling, writing or coming in to our office. You may call us by dialing 777 or you may come to the office at the addresses given on page 1, between the hours of 8am and 5pm Monday through Friday, except holidays. Some locations are closed daily 12pm - 1pm. When Can My Service Be Disconnected? Your service can be disconnected when you have not paid delinquent charges, fail to pay a required deposit, fail to comply with a settlement agreement, misrepresent your identity, or refuse to allow inspection of Company’s facilities as provided by any state and/or federal law. Basic local phone service cannot be disconnected for non-payment if delinquent charges are for other products and services on the account. Phone service will not be disconnected on days when the office is not open for business or on the day before the office will not be open for business. Phone service may not be disconnected prior to 5 business days after notice has been given of the Company’s intent to do so in Iowa, and prior to 10 calendar days after notice has been given of the company’s intent to do so in Missouri. The Company sends reminder notices by mail or email. The Company may block access to toll and eliminate any optional services and/or features for non-payment of charges for those services. How Can I Prevent Discontinuance Of Service During An Absence? Contact our business office and inquire about pre-payment. How Can I Have My Discontinued Service Restored? You may have your discontinued service restored by paying all undisputed charges and the required deposit, if any, or the Company may elect to enter into a settlement agreement with you that would restore your service. You may be assessed a charge for restoring service and the Company may require a deposit.

When Is A Deposit Or Guarantee Required? You may be required to post a deposit or provide a guarantee of payment if you fail to meet our credit-worthiness criteria. Call our office to inquire about our credit-worthiness criteria. How Can I File A Dispute/Complaint? If you have a dispute/complaint with GRM Networks or SCC Networks, it may be resolved by contacting the District Manager at your local busi ness office, or call one of the numbers given below. A “dispute” must be registered with the Company prior to the delinquent date in order to avoid discontinuance of service. If your complaint is still not resolved, Missouri customers may contact the Missouri Public Service Commission. You may also contact the Office of Public Counsel. They represent the public (resi dential customer) before the Public Service Commission. Iowa customers may contact the Iowa Utilities Board.

Additional Listings For each customer, a single line insertion in the alphabetical list is made without charge. Customers may arrange for additional listings. Rates for additional listings may be obtained from the business office. Directory Errors And Omissions Claims for damages caused by errors or omissions in directory listings will be limited to prorated charges for the directory listing that is affected. Listings Not Appearing In The Directory Between directory issues, many phone numbers do not appear in the current issue of the directory because of changes and new installations. Patrons may obtain these phone numbers from “Directory Assistance” by dialing 4-1-1. Use Of Residential Phones For Business Purposes Residential phones are installed with the understanding that they will be used for domestic purposes. Customers who receive Lifeline benefits are also eligible for the ACP and can call their local GRM Networks office to receive the additional benefit. Individuals receiving both ACP and Lifeline may apply their ACP benefit to their Lifeline service or to a different service. Establishing Or Transferring Phone Service

To establish or transfer residential and/or business phone service, please call your local business office. To enable us to process your order to establish phone service, we will need to know your complete address and how you want to be listed in the directory. We also need to know about previous phone service in your name and other credit information. We will ask for identification and a contact number. Deposit Policy Customers may be asked to pay a deposit to establish service. The amount of your deposit is determined when you apply for service. You Will Be Billed For Installation And Changes In Your Service All changes, except termination of service and disconnection of cer tain options, require a service charge. The amount you are charged depends on the type of change requested. • Service order - you are charged for the processing of your order when you install or make changes in service. • Central office connection charge - this charge is for the work we do to connect your line to the central office. Additional charges may apply if special construction is needed to connect your service. • Time and material charges - this includes an hourly rate for straight time, an hourly rate for overtime, a charge for the material used and work performed, and an equipment charge if applicable. • Trip charge - this charge is for the cost of making the trip to your location. • Programming charge - this charge applies for programming changes in the digital central office equipment after the initial installation. There is no charge to disconnect phone service. Contact your local business office for current rates and charges. How You Are Billed Charges for local service, internet access, and Skitter TV are billed one month in advance. Toll calls and repair charges are billed after they occur.

Error On Your Bill If you have a question regarding your bill, call your local business office. Explain the possible error to a customer service representative. If the error cannot be resolved, please pay the undisputed charges on time so you’ll maintain a good payment record while the problem is being investigated. Paying Your Bill On Time, So Your Service Will Not Be Disconnected If your payment is not received by the due date, or you do not meet payment arrangements, your service may be disconnected. How To Get Your Service Restored All relevant past charges must be paid before your service can be reconnected and you will be charged a restoral fee. You may also be asked to pay a deposit. CPNI Under FCC rules governing the use and disclosure of Customer ProprietaryNetwork Information (CPNI), we canonly discussCPNI at our retail locations with those individuals listed as an authorized user on the account and presenting a government issued photo ID. CPNI includes call detail information and certain account information, including the amount of your bill. For phone inquiries, other rules dictate how we authenticate a customer. Youmay add authorized users to your account by contacting your local business office.

Relay Iowa & Relay Missouri Relay makes it possible for individuals who are deaf, hard of hearing, deaf-blind or have difficulty speaking to make and receive telephone calls. Traditional Relay calls are based on TTY (also known as a TDD or text telephone). However, there are many different types of Relay calls, such as Voice Carry Over and Hearing Carry Over, that accom modate individual calling preferences. Here’s How Relay Works Relay calls are simple - just dial 711 or the appropriate toll free num ber and you will be connected with a highly trained Communication Assistant (CA) that will facilitate your call. Regardless of how you connect, the CA will “voice” everything you type to the other party, and type everything the other party says so that you can read the conversation on the screen of your assistive device. The CA is an invisible link between the calling parties. There are no restrictions or time limits on any call, and Relay is avail able 24 hours a day, seven days a week, 365 days a year. Whether you’re making personal calls or business calls, by law all Relay calls are confidential and there are no records or recordings of any Relay conversation. Specialized Services Relay offers specialized services for individuals who have difficulty speaking and for Spanish speaking residents. Specially trained CAs are on hand to assist in these types of calls and may be reached by dialing the associated number provided. Since Relay offers a vari ety of services, please refer to the website or call your state Relay Customer Service for more detailed instruction on how a particular call is processed. Captioned Telephone, or CapTel, is also available and ideal for indi viduals with hearing loss who are able to speak for themselves. A Captioned Telephone works like any other phone with one important addition: it displays every word the other person says throughout the conversation. Captioned Telephone users can listen to the caller and read the captions on the display window of the Captioned Telephone.

Telecommunications Access Program And Telecommunications Access Iowa Are you, or someone you know, frustrated trying to use the telephone? Is it difficult to hear or understand the conversation? Do others have a hard time hearing you, or ask you to repeat yourself often? Is it challenging to dial a phone with small buttons or hard-to-read displays? Perhaps you don’t always hear the telephone ring or can’t get to the telephone in time before the caller hangs up. The Telecommunications Access Program (TAP) Missouri and Telecommunications Access Iowa can help you get specialized telecommunications equipment at little or no cost. Emergency Calls Please note that 7-1-1 is only to be used to reach Relay. In an EMERGENCY you should continue to use 9-1-1. The Americans with Disabilities Act (ADA) requires that all 911 centers have TTY and be prepared to handle emergency calls placed in this manner. Relay will make every effort to assist you in an emergency. However, it is important to understand that Relay centers are not 911 centers and do not assume responsibility for emergency calls. Customers’ Rights To Own And Provide Their Own Inside Station Wiring You have the right to provide your own inside wiring. Customers are not required to use the services of GRM Networks ® or SCC Networks ® for the installation of new inside wiring. Upon request, GRM Networks will provide technical information. You are responsible for maintaining all inside wiring. GRM Networks and SCC Networks will maintain inside wiring for single-line Business and Residence customers on a contractual basis. Inside wiring maintenance is also available for multi-line business customers. Contact your local business office for more information and current pricing.

Customers’ Rights Under Federal Communications Commission (FCC) guidelines, you may supply your own phone or other terminal equipment and connect it directly to the nationwide phone network. You are not required to obtain your phone or other terminal equipment from GRM Networks or SCC Networks in order to receive service. Rates and services are provided on an equal basis whether you provide your own equipment or obtain it from GRM Networks or SCC Networks. If you supply your own phone or other terminal equipment, it must be approved by the FCC, and you are responsible for its proper connection, maintenance and repair. GRM Networks or SCC Networks will verify the condition of a line if requested at no charge. A charge will apply for each visit by GRM Networks or SCC Networks to a customer’s premises in connection with a service difficulty when it is determined that the difficulty was due to a condition in a customer provided phone or other terminal equipment. Fraudulent Charges It is illegal to charge any calls to another person’s phone number or calling card without his/her permission. Persons placing such calls are subject to prosecution, and, if convicted, are subject to a fine and/or imprisonment. Harassing Calls State and federal law make it a crime to place obscene or harassing phone calls. The penalty is a fine and/or imprisonment. Unlawful Wiretapping It is a crime under federal and state laws to wiretap or otherwise intercept a phone call unless the consent of one or both of the parties actually participating in the call has been obtained. (The exceptions are properly authorized law enforcement offices, acting under court order, according to state and federal law.) The penalty for illegal wiretapping can be a fine and/or imprisonment.

Why Won’t My Phone Work? These tests can help you determine why your phone isn’t working:

1. If you have only one phone and it unplugs, take it to a friend’s home or office and plug it in. If it doesn’t work there, you probably have a phone equipment problem . 2. If you have two or more phones that unplug, unplug all of them. Then try each one, one at a time in each phone jack or outlet. If one phone doesn’t work anywhere, you probably have a phone equipment problem. If none of the phones work in any jack, you may have a jack or inside wiring problem. 3. Borrow a friend’s phone and try it in each one of your jacks. If it doesn’t work in a particular jack, you probably have a jack or inside wiring problem . If it doesn’t work in any jack, you may have a line problem . 4. If your phone doesn’t unplug, contact the company that sold or leased it to you for directions on how to remove it. 5. If you have phone accessories such as answering machines, speaker phones, cordless phones, security systems or night bells, make sure the equipment is working. If your phone service works without the added equipment, the problem may be with the accessories. If You Hear Noise On Your Line: It may be caused by the weather, aerial satellites, poor grounding or other conditions that are only temporary. If the noise persists, the troubleshooting tips listed above may help isolate the problem. The interference may also be caused by radio frequency signals from amateur citizens band (CB) and AM/FM broadcast stations. The installation of a modular filter, obtainable from most stores selling phone equipment, may help. Because cordless phones operate on radio frequencies, they can also receive interference from radio transmitters. Contact your cordless phone equipment supplier or manufacturer for assistance with these problems.

If You Hear A Rapid Busy Signal: While making a local or long distance call, it means that all phone circuits are busy. Wait a few minutes, then try your call again.
