GRM Networks Gives Back GRM Networks Community Improvement Grant Program Clubs and organizations within the GRM Networks service area are eligible to apply for the GRM Networks Community Improvement Grant Program. The Grant program authorizes GRM Networks to award up to $1,250 per quarter to groups that submit an application describing a project that will better their community. Typically, the quarterly allotment is divided into two $500 awards and one $250 award. Applications are available at www.grm.net . FRS/GRM Networks Youth Tour to Washington DC Every year students that are juniors in high school and whose parents or legal guardians are members of GRM Networks have the opportunity to apply for an all-expense paid trip to Washington DC. The five day, four night trip offers an exciting and fast paced tour of our nation’s capital. After applicants have been interviewed by a panel of GRM Networks board members, two students are chosen to attend the event. Applications are available in January at www.grm.net , at the local business offices and from high school guidance counselors. FRS/GRM Networks Scholarship Senior students whose parents or guardians are members of GRM Networks can apply for The Foundation for Rural Service’s (FRS) $2,500 scholarships. These scholarships are awarded to rural high school seniors: one per geographic region of the NTCA-The Rural Broadband Association membership, and one to a student sponsored by an NTCA associate member. The remaining awards will be distributed proportionate to the number of applications received from each NTCA region. FRS will fund $2,000 of each scholarship, with the sponsoring telecommunications provider or associate member of each winning applicant providing a $500 match. FRS will also provide four Staurulakis Family Scholarships valued at $5,000 each and two TMS Scholarships for $1,500 each. Students who are majoring in math, science, engineering, or medicine are given preference for the FRS Staurulakis Family Scholarships. Preference will be given to students expressing an interest in returning to a rural area to work following graduation.

New in 2015: The Colt Ford Scholarship program will offer up to $15,000 in one-time scholarships to rural students.

Customer Rights And Responsibilities

When Will I Receive My Bill? GRM Networks mails bills at or near the first day of each month. You should receive your itemized bill within the first few days of the month. If you do not receive your bill, please call our Business Office by dialing 777 from your home or by calling one of the office phone numbers on page 6; we will mail you a copy of your current bill at no charge. Relay Service for hearing and speech impaired customers can be reached by dialing 7-1-1, and a Communications Agent will assist you in reaching our offices. How Can I Pay My Bill? You may mail your payment as directed on your bill, or you may pay in person at our business office. We also offer automatic bank deduction, debit and credit card payment abilities. You may also pay online by going to grm.net and clicking on eBill . Contact your local Business Office for more information. You will save time and reduce chance of error if you use the payment portion of your bill when paying. There is a charge for each returned check. When Does My Bill Become Delinquent? Your bill will be declared delinquent if it is still unpaid on the 21st day after the first mailing. How Can I Dispute A Charge On My Bill? If you believe a charge appearing on your bill is in error, you can inquire and/or dispute a charge by calling, writing or coming in to our office. You may call us by dialing 777 or you may come to the office at the addresses given on page 6, between the hours of 8am and 5pm Monday through Friday, except Holidays. When Can My Service Be Disconnected? Your service can be disconnected when you have not paid delinquent charges, fail to pay a required deposit, fail to comply with a settlement agreement, misrepresent your identity, refuse to allow inspection of Company’s facilities and as provided by any state and/or federal law. Basic local phone service cannot be disconnected for non-payment of a delinquent charge for other than basic local phone service. Phone service will not be disconnected on days when the office is not open for business or on the day before the office will not be open for business. Phone service may not be disconnected prior to 10 calendar days after notice has been given of the Company's intent to do so (Company will mail a 10-day reminder notice). The Company may block access to toll and eliminate any optional service and/or features for non-payment of charges for those services. How Can I Prevent Discontinuance Of Service During An Absence? How Can I File A Dispute/Complaint? If you have a dispute/complaint with GRMNetworks, it may be resolved by contacting the District Manager at your local Business Office, or call one of the numbers given below. Missouri Universal Service Available To Missouri Customers The Missouri Public Service Commission (MPSC) has approved the establishment of a Missouri Universal Service Fund (MoUSF) to help low-income and disabled Missourians receive discounts for basic local telephone service. The Commission established a start date of May 1, 2005, with a mandatory customer surcharge of .18% applied to total local service and interstate toll charges to be applied on all Missouri customer bills. The Missouri Public Service Commission issued an order (Case No TO-2014-0333) decreasing the Missouri Universal Service Fund assessment rate to 0.0010 (0.10%), EFFECTIVE OCTOBER 1, 2014. This surcharge appears as a separate line item on customer bills detailed as “Missouri Universal Service Fund.” Missouri customers who receive Federal Lifeline support auto- matically receive an additional $6.50 per month in Missouri Service Fund support. The Federal Lifeline Program and the MoUSF together offer a discount to qualifying customers of $15.75 per month. Individuals who qualify for low-income or disabled support are required to complete a MoUSF Board approval application. The Application Form includes a release so that the Missouri Public Service Commission or its staff may confirm participation in the qualifying programs. By signing the authorization, the beneficiary recipient authorizes the appropriate federal, state or local agency

Listings Not Appearing In The Directory Between directory issues, many phone numbers do not appear in the current issue of the directory due to changes and new installations. Patrons may obtain these phone numbers from “Directory Assistance” by dialing 4-1-1. Use Of Residential Phones For Business Purposes Residential phones are installed with the understanding that they will be used for normal or domestic purposes.

A residential phone will be changed to business service if used primarily or substantially for business purposes or if the residential phone number is advertised in connection with the sale of products or services.

Service & Billing

Establishing Or Transferring Phone Service To establish or transfer residential and/or business phone service, please call your local Business Office. To enable us to process your order to establish phone service, we will need to know your complete address and how you want to be listed in the directory. We also need to know about previous phone service in your name and other credit information. We will ask for identification and a number where you can be reached, either at work or through a friend or relative, until your service is established. Deposit Policy Customers may be asked to pay a deposit to establish phone service. The amount of your deposit is determined when you apply for service. You Will Be Billed For Changes In Your Service All changes, except termination of service and disconnection of certain options, require a service charge. The amount you are charged depends on the type of change requested. Consult your local Business Office for different services. How You Are Billed Charges for local service are billed one month in advance. Toll calls and repair charges are billed after they occur. How, When & Where To Pay Your Bill Your payment due date appears on your bill. Your payment is considered past due if not paid by that date. You can mail your payment or pay your bill at your local Business Office. Please don’t send cash in the mail. We also offer automatic bank deduction, debit and credit card payment abilities or you may pay online by going to grm.net and clicking on eBill. Contact your local Business Office for more information. You will save time and reduce chance of error if you use the payment portion of your bill when paying. There is a charge for each returned check.

Error On Your Bill If you have a question on your bill, call your local customer service representative. Explain the possible error to the representative who will help you. If the error cannot be resolved, please pay the undisputed charges on time so you’ll maintain a good payment record while the problem is being investigated.

Paying The Bill On Time, So Your Service Will Not Be Disconnected

If your payment is not received by the due date, or you do not meet payment arrangements, your service may be disconnected. How To Get Your Service Restored All relevant past charges must be paid before your service can be reconnected and you will be charged a restoral fee. You may also be asked to pay a deposit. CPNI Under FCC rules governing the use and disclosure of Customer Proprietary Network Information (CPNI), we can only discuss CPNI at our retail locations with those individuals listed as an authorized user on the account and presenting a government issued photo ID. CPNI includes call detail information and certain account information, including the amount of your bill. For phone inquiries, other rules dictate how we authenticate a customer. You may add authorized users to your account by contacting your Business Office.

Charges To Install And Change Your Residential Service The GRM Networks one-time charge to install or change your service may vary, depending on the type of work that is done. SERVICE ORDER: $4.00 You are charged for the processing of your order when you install or make changes in phone service. CENTRAL OFFICE CONNECTION CHARGE: $10.75 This charge is for the work we do to connect your line to the central office. TIME AND MATERIAL CHARGES: Time at $60 per hour/$90 per hour overtime (subject to change) plus a material charge for the material used and work performed which is not provided for elsewhere in our filed tariffs, plus equipment charge if applicable. A trip charge of $30 could apply. PROGRAMMING CHARGE: $5.00 This charge applies for programming changes into the digital central office equipment after the initial installation. Upon reaching the center, provide the relay agent with the area code and phone number you wish to call. If making a long distance call, advise the relay agent of the type of call you are making (direct dial, credit card, collect, person to person, etc.) and how you wish to pay (if using calling card, credit card or third number billing, you will need to provide your name and the necessary billing numbers). Relay Iowa Relay Iowa is a specialized service that guarantees all citizens access to prompt, professional and accurate communication through the phone. Consumers of these specialized services, specifically individuals who are deaf, deaf-blind, hard of hearing or have difficulty speaking, can communicate on the phone via TTY, Voice Carry Over (VCO), Hearing Carry Over (HCO), Speech-to- Speech (STS), Spanish-to-Spanish and Captioned Telephone in order to connect with family, friends or businesses with ease. Here’s How Relay Iowa Works: Dial 711 or the appropriate toll-free number provided to connect with Relay Iowa. A specially trained operator, called a Communication Assistant (CA) will ask for the area code and number of the person you wish to call and will begin relaying the conversation. Generally, the CA will voice the typed message from the text phone (TTY) user to the person on the other end. The CA then relays the spoken words by typing them back to the TTY user. All calls are held strictly confidential. Specialized Services Relay Iowa offers specialized services for individuals who have difficulty speaking and for Spanish speaking residents. Specially trained CAs are on hand to assist in these types of calls and may be reached by dialing the associated number provided on page 14. Since Relay Iowa offers a variety of services, please refer to the website listed or call Relay Iowa Customer Service for more detailed instruction on how a particular call is processed. Captioned Telephone is also available and ideal for individuals with hearing loss who are able to speak for themselves. Customers’ Rights To Own And Provide Their Own Inside Station Wiring You have the right to provide your own inside wiring. Customers are not required to use the services of GRM Networks for the installation of new inside wiring. Upon request, GRM Networks will provide technical information. You are now responsible for maintaining all inside wiring. GRM Networks will maintain inside wiring for single line Business and Residence customers on a contractual basis for $ .50 per month per line. Inside wiring maintenance is also available for multi-line business customers. Contact your local Business Office for rates. The above rates were correct as of the date this directory was printed. Current rates and charges are always available from your Business Office. Under Federal Communications Commission (FCC) guidelines, you may supply your own phone or other terminal equipment and connect it directly to the nationwide phone network. You are not required to obtain your phone or other terminal equipment from GRM Networks in order to receive service. Rates and services are provided on an equal basis whether you provide your own equipment or obtain it from GRM Networks. If you supply your own phone or other terminal equipment it must be approved by the FCC and you are responsible for its proper connection, maintenance and repair. GRM Networks will verify the condition of a line if requested at no charge. A charge will apply for each visit by GRM Networks to a customer’s premises in connection with a service difficulty when it is determined that the difficulty was due to a condition in a customer-provided phone or other terminal equipment. Fraudulent Charges It is illegal to charge any calls to another person’s phone number or calling card without his/her permission. Persons placing such calls are subject to prosecution, and, if convicted, are subject to a fine and/or imprisonment. Harassing Calls State and federal law make it a crime to place obscene or harassing phone calls. The penalty is a fine and/or imprisonment . Unlawful Wiretapping It is a crime under federal and state laws to wiretap or otherwise intercept a phone call unless the consent of one or both of the parties actually participating in the call has been obtained. (The exceptions are properly authorized law enforcement offices, acting under court order, according to state and federal law.) The penalty for illegal wiretapping can be a fine and/or imprisonment. These tests can help you determine why your phone isn’t working: 1. If you have only one phone and it unplugs, take it to a friend’s home or office and plug it in. If it doesn’t work there, you probably have a phone equipment problem . 2. If you have two or more phones that unplug, unplug all of them. Then try each one, one at a time in each phone jack or outlet. If one phone doesn’t work anywhere, you probably have a phone equipment problem. If none of the phones work in any jack, you may have a jack or inside wiring problem. 3. Borrow a friend’s phone and try it in each one of your jacks. If it doesn’t work in a particular jack you probably have a jack or inside wiring problem . If it doesn’t work in any jack, you may have a line problem . 4. If your phone doesn’t unplug, contact the company that sold or leased it to you for directions on how to remove it. 5. If you have phone accessories such as answering machines, speaker phones, cordless phones, security systems or night bells, make sure the equipment is working. If your phone service works without the added equipment, the problem may be with the accessories. If You Hear Noise On Your Line: It may be caused by the weather, aerial satellites, poor grounding or other conditions that are only temporary. If the noise persists, the troubleshooting tips listed above may help isolate the problem. The interference may also be caused by radio frequency signals from amateur citizens band (CB) and AM/FM broadcast stations. The installation of a modular filter, obtainable from most stores selling phone equipment may help. Because cordless phones operate on radio frequencies, they can also receive interference from radio transmitters. Contact your cordless phone equipment supplier or manufacturer for assistance with these problems. If You Hear A Rapid Busy Signal: While making a local or long distance call, it means that all phone circuits are busy. Wait a few minutes, then try your call again.
