Customer Rights & Responsibilities
If the customer is receiving benefits from any of the following programs, he/she could be eligible for help from the Universal Service Fund: • Medicaid – MO Healthnet • Food Stamps • Supplemental Security Income • Veterans Benefits and Survivors Pension • Federal Public Housing Assistance (Section 8) • 135% of the Federal Poverty Level • Veterans Administration Disability Benefits • State Blind Pension • State Aid to Blind Persons Contact the local telephone company for more information concerning the Universal Service Fund. Disconnection or Interruption of Telephone Service Phone service is subject to disconnection or interruption for any of the reasons listed on page 5. If service is disconnected and reconnected within 30 days, the customer’s phone number is reserved, and he/she will not be charged for installation again. However, he/she will be charged a reconnection fee. If service is disconnected, a new phone number will be assigned, and the customer will be required to pay installation charges again. • Federal Social Security Disability • State Supplemental Payments
Phone Bill Fidelity will provide the customer with a phone bill each month. Fidelity provides basic local phone service (including access to 911, where available) and certain custom calling services. Payment in full is due within 21 days of the date of the bill. If we do not receive payment within 21 days, the service is subject to disconnection. When paying by mail or online, please allow enough time for payment to reach Fidelity by the due date. Universal Service Fund The goals of Universal Service, as mandated by the 1996 Act, are to promote the availability of quality services at just, reasonable, and affordable rates; increase access to advanced telecommunications services throughout the Nation; and advance the availability of such services to all consumers, including those in low-income, rural, insular, and high-cost areas at rates that are reasonably comparable to those charged in urban areas (as cited from http://www.fcc.gov ). Customers may qualify for a discount on their monthly phone bill through the federal and/or the Missouri program if they are receiving low-income or disability benefits under certain programs. The discount varies depending on the local telephone company and method of qualifications.

Billing & Payments 1. Nonpayment of an undisputed delinquent charge for basic local telecommunications service. A customer’s service will not be discontinued for nonpayment of a delinquent charge until Fidelity has notified the customer in writing at least ten days in advance of the discontinuance. Additionally, Fidelity will make reasonable efforts to contact the customer by phone at least 24 hours prior to disconnecting telephone service. Basic local service will not be discontinued for nonpayment of any charges other than those for basic local service; however, Fidelity may activate global toll blocking and eliminate any optional, non-basic calling features for nonpayment of charges for other than basic local service. 2. Using phone utility equipment in a manner that creates an unsafe condition or creates the possibility of damage or destruction to such equipment. 3. Refusal after reasonable notice to permit inspection, maintenance or replacement of phone utility equipment. 4. Misrepresentation of identity in obtaining phone utility service. 5. Failure to post a required deposit or guarantee. 6. Failure to comply with the terms of a settlement agreement. 7. Failure to act in accordance with any state or federal law. Reconnection of Service After local phone service has been interrupted or disconnected, Fidelity will restore the customer’s service when the reason for the disconnection or interruption has been remedied. Before restoring the customer’s service, the following will be required: 1. Reconnection charges must be paid if the service has been interrupted or disconnected. 2. One month’s advance payment and/or a deposit is required. 3. Payment for all undisputed amounts must be received by Fidelity. Billing & Collection Local phone service is classified as a business or residence based on the character of the usage and not upon the location of the phone. New customers may be required to pay a security deposit. Fidelity will issue a bill not less than every 28 days or no more than every 31 days. Payment will be due upon receipt and will become delinquent 21 days after mailing. A fee will be applied
to your bill if paid after the delinquent date. If your bill becomes delinquent, a 10-day notice will be sent before disconnecting your service. A charge for restoring service will be applied when your service is reconnected. Under our billing procedure and subject to rules established by the Missouri Public Service Commission, Fidelity may require a deposit or a guarantee of payment. How to Pay Your Bill 1. Online: Go to http://www.fidelitycommunications.com/billpay 3. By phone: Customers can use a credit card, debit card or eCheck to pay their bill over the phone. A customer can pay over the phone by using the automated system or talking to a Fidelity representative. If the customer chooses to talk to a representative, he/she will be assessed a “Pay by Phone Conv Fee” of $4.50 added to each account. When you regularly pay your phone bill by the
due date, you maintain good credit with us. Good credit entitles you to establish new services or add extra services without paying a deposit. If you cannot pay your bill on time, your phone service may be disconnected, and you may
have to pay all charges billed for service and an additional payment or security deposit to reconnect your service. Help us provide you with continuous service by paying on time.

Customer Information
Local Service Local service is furnished for the primary use of the customer and members of his firm or family. As a consumer, you have the right to expect safe, reliable and uninterrupted service and to be charged for only the amount of service you use. As your service provider, we expect you to promptly pay for the service you use. Any questions you may have should be directed to your local business office.
If you are not satisfied with the help you receive from a customer representative, ask for a supervisor. Written inquiries may also be directed to Fidelity Communications, 64 N. Clark St., Sullivan, MO 63080. If you are still not satisfied, call our consumer hotline at 1-888-779-4163. If your complaint is about credit, billing, deposits or termi- nation of service, you can ask for a personal meeting with a Fidelity representative, who is there to resolve such disputes. If you don’t resolve the dispute with the representative, you can file an informal complaint with the Missouri Public Service Commission. The Missouri Public Service Commission is the state agency that regulates Fidelity Telephone Company and other public utilities in the state. Addresses and phone numbers are:
Advertising Phone Numbers Residential phones are installed with the understanding that they will be used for normal residential purposes and not in connection with any business. Advertising residential phone numbers for business purposes will be considered sufficient cause for Fidelity to charge the business rate. Who Owns My Phone Number? Your telephone number is the property of Fidelity Telephone Company. We may change your number if it becomes advis- able for us to do so in the conduct of our business. How Can I Temporarily Stop Phone Service When I’m Out of Town? If you wish to stop your phone service, call your local cus- tomer representative. There is no charge for disconnection and charges to your bill will be stopped up to five (5) business days after you call. As an alternative, you can have your phone service temporarily suspended and services discounted (for example when going on vacation). Service can be restored on short notice for a $25 restoral fee. You Have the Right to Complain If you have a complaint about any aspect of service that you receive from Fidelity, call your customer representative, page 3. He or she will answer your questions and make an effort to resolve your problems.

Customer-Provided Equipment Customer-provided equipment may be connected to Fidelity’s facilities only if it is registered or otherwise complies with the FCC’s rules and meets the conditions set forth in Fidelity’s tariffs. These FCC rules and tariffs are designed to meet the convenience of the customer and at the same time protect the telephone network against any possible hazards to customers and Fidelity employees. A service charge may be incurred if Fidelity visits the custom- er’s premises and the service difficulty or trouble is found to have been caused by the customer’s equipment or facilities. If you have any questions concerning the use of customer- provided equipment, please contact the Fidelity Service Center at 573-468-8081. Inside Wire Maintenance Nobody likes unexpected repair bills. That’s why we offer Inside Wire Maintenance service. With Inside Wire Maint- enance, when a problem occurs in your home, you’re covered. We maintain the phone wiring outside your home up to the demarcation, at no charge, as part of our basic service to you. Maintaining the wiring inside your home is your responsibility, unless you subscribe to our Inside Wire Maintenance plan. By subscribing to Inside Wire Maintenance, you’ll receive all these services, if needed: • Diagnosis of your inside phone wiring by our trained personnel. • Complete analysis of your phone service problem. • Repairs to your inside wiring.
All it takes is one small monthly fee to get the worry-free cov- erage of Inside Wire Maintenance. For more information about this service, please contact your local business office listed on page 3. To subscribe to Inside Wire Maintenance service, your phone lines must not be part of a telephone system, such as a PBX or Key system. Inside Wire Maintenance does not cover repair of inside wiring due to natural disasters such as flood, wind and earthquake; riot and acts of war; fire; total destruction by fire; wiring that does not meet Fidelity’s installation practices or technical standards or is not in satisfactory working condition; gross negligence; willful damage; and/or vandalism.

Fraudulent Calls It is illegal for another person to charge long distance calls to your number or calling card without your permission. For your protection, new equipment and procedures enable Fidelity to detect and investigate fraudulent calls. State law provides that no person shall defraud or attempt to defraud the telephone company of its lawful charges. Violation of this law is punishable by a $1,000 fine or one year in jail or possibly both. What is a 900 Number? Businesses, as well as public and private organizations, commonly use “900” numbers to provide messages or services on a variety of topics. These numbers are not the same as toll-free “800” numbers. When placing a 900 number call, the calling party will incur a flat charge or a per minute rate charge that can range from 50 cents to several dollars. Fidelity offers 900 number blocking from its digital central offices. Digital blocking prevents 900 number calls from being completed from the selectively blocked line. Toll-free 800 numbers and regular long distance calls can still be placed. 900 Billing Disputes For 900 billing disputes or inquiries, please call 468-8081 toll-free from any Fidelity exchange. You have 60 days from the date of the bill to dispute a 900 number billing error. You have the right to withhold payment of the disputed 900 number charge only during the billing error review. All other phone charges are due upon receipt. No collection activity for disputed 900 number charges is legitimate; the information provider may proceed with outside collections against your account for nonpayment of these charges. Your local and long distance service cannot be disconnected for nonpayment of 900 number charges. Failure to pay legitimate 900 number charges may result in involuntary blocking of your access to 900 number services. Voluntary blocking of access to 900 number service is available upon request from your local Fidelity office at an additional charge. Toll Calls You are responsible for toll calls placed from your phone. In the interest of fast service, we do not secure authorizations before completing calls; therefore, please see that calls are placed by authorized persons only.
Harassment by Phone Missouri law defines harassment by phone as the use of phone communications for any of the following purposes: • Making any lewd or indecent comment or request with intent to offend. • Making one or repeated phone calls, whether or not conversations occur, with intent to abuse, threaten, or harass. • Causing the phone of another to ring repeatedly with intent to harass. • Knowingly allowing any phone under one’s control to be used for any of the above. If you receive obscene, harassing or threatening calls: • Hang up at the first obscene word or if the caller doesn’t say anything the second time you say hello. • Give no information, such as your name and address, until the caller has been identified. • Advise your children and guests not to give out any information to strangers. If you’re not home, they should say, “They can’t come to the phone right now, may I take a message?” If the calls persist, call your local law enforcement.

How to Outfox Phone Fraud in Your Car • Remove handset and antenna from car when not in use. This will help avoid the unwanted attention of criminals with Electronic Serial Number (ESN) cloning devices. • Protect your Electronic Serial Number. Never give your ESN number to anyone. And don’t put your subscriber agreement in an unsafe place, such as the glove compartment. • Never let anyone else use your phone unless you are present. If someone wants to use your phone, offer to dial the number for them. Fraud criminals can easily access the codes stored in your phone. • Have service performed only at reputable locations. If you need service, take your phone only to an authorized distributor of your cellular service. • If your phone is stolen, call your local police and cellular carrier immediately. The sooner you do, the less likely your phone will be used fraudulently. Internet Usage
The Internet is the newest target for fraudsters. Nearly 240 million people in the U.S. have access to the Internet, and due to the rapid growth in this industry, increased consumer awareness is critical. If you are an Internet user or prospective Internet user, please help protect yourself by following these tips: • Use extreme caution when giving out financial or personal information such as credit card numbers, bank accounts and Social Security numbers. • Conduct business with companies you know and trust. • Look carefully at information about products or services being offered. Ask for more information if needed. • Check out the company’s reputation. The Better Business Bureau is one agency that can help you. • Use caution when downloading programs or services. This is a potential avenue for viruses.
Phone Fraud How to Outfox Phone Fraud!
Don’t look now, but phone fraud could be happening right behind your back! Phone fraud is a subtle, but fast-paced crime, that rings up billions in fraudulent phone charges and victimizes millions of people each year. How to Outfox Phone Fraud When at Home • Don’t accept third-party or collect calls that are suspicious or from someone you don’t know. When you accept, you have agreed to pay for the charges. • Phone companies or law enforcement officials will never ask customers to accept collect or third-party charges as part of an investigation. The only time a phone company representative will request specific billing information is if you make a call through an operator. • Restrict third-number or collect calls from being made to your home. Some phone companies offer this service, thus preventing fraudulent calls from being billed to your line.

Phone Safety The phone is one of the safest appliances in your home or office. There are, however, a few situations where a phone user needs to exercise caution.
Directory Accuracy Directories are furnished to customers to facilitate the use of phone service. It is not intended to be an address book. The addresses listed are not, in all cases, the mailing address. Neither Fidelity nor the Publisher assumes any liability whatsoever for damages arising from errors or omissions in the making up or printing of this directory. If your listing is incorrect in any way, please call your local business office. If there are mistakes in Yellow Pages advertising, you can call DPS Media, Inc., 1-866-221-4300. Listings We provide an updated directory each year for your convenience. Every customer may be listed once in the alphabetical section of the directory without charge. Additional listings may be arranged for an additional charge according to the type of listing required. If you don’t want your name published in the book or given out by directory assistance, then you may arrange for a nonpublished number for an additional charge. Once you have a nonpublished number, be very careful how you give it out. You can help protect your number by giving it out only when absolutely necessary. Contact your local business office for information on our different types of listings.
Use of the Phone Near Water The phone should not be used while you are in a bathtub, shower or pool. Immersion of the phone or handset in water could cause electrical shock. Use of the Phone During an Electrical Storm Calls of an urgent nature should be brief. Although the phone system uses protective measures to limit abnormal electrical surges from entering your home, there is a remote risk of a dangerous electrical shock from lightning when using the phone during an electrical storm. Use of the Phone to Report a Gas Leak If you think you detect a gas leak, you should not use a phone in the vicinity of the leak because of the contacts that could generate a tiny spark when you lift the handset and dial. While unlikely, it may be possible for this spark to trigger an explosion if the gas concentration is high enough.

Problem Trouble on all phones. Possible Cause & Solution
DIAL 611 24 HOURS A DAY FROM YOUR FIDELITY HOME PHONE
Unplug all of the phones, answering or fax machines, and computers from the wall or baseboard jacks inside your home. (An AC adapter powers any of the above equipment; unplug it from the AC outlet also.) Then try each one, one at a time, in each phone jack or outlet. If none of the phones work in a particular jack, the trouble is with that jack. You can also take at least one of your phones without a built-in speaker or lights to the Network Interface Box. (This is where our outside line and your inside phone wiring connect or “interface”.) This box is usually located on the outside wall of your home near the electric meter. Apartment interfaces are located inside your apartment. Use the screwdriver to open the bottom or left portion of the Interface Box and disconnect the modular plug from the test jack. 1. Plug your phone into the test jack. 2. Make a phone call and listen for the problem. 3. If the same problem occurs, use a second phone and repeat above steps. If you have the same trouble as you were having inside your home with ONE of the phones you just tested – the cord or phone you were using is defective. Repair or replace it. If you have the same trouble as you were having inside your home with TWO DIFFERENT phones – the trouble is probably outside your home. Call the Repair Service Center. We will make repairs to our outside line and switching equipment at no charge to you. If NEITHER of the phones you just tested give you trouble, the trouble is inside your home. It could be with a telephone you were not using outside or with the inside wiring. To repair the wiring inside your house, you have three options. 1. Call Fidelity to make the repairs. If you subscribe to our Inside Wire Maintenance service, we will repair your jack or inside wiring at no charge to you. If you do not subscribe, we will still be happy to repair the problem. There will be charges for labor and materials. 2. Hire someone else to do the work. 3. Repair the problem yourself.
What to do if Your Phone Does Not Work
The trouble can be in your phone, the wiring inside your home, or the outside lines and switching equipment maintained by Fidelity. There are some simple tests that can help you determine whether to call Fidelity or correct the problem yourself. These tests can save you time and money. Problem Hear other conversations while on the phone. Possible Cause & Solution The interference may be caused by radio frequency signals from amateur citizens band (CB) and AM/FM broadcast stations. Sometimes your telephone equipment can act like an “antenna” for these signals. The installation of a modular filter, obtainable from Fidelity or from most stores selling phone equipment, may help. If you have a cordless phone, unplug your cordless phone from the wall telephone jack and the AC outlet. If the problem goes away, your cordless phone is the problem. Some of these phones have buttons to change channels or frequencies. If yours does not, you may wish to have it repaired or replaced. If you do not have a cordless phone, or if the problem still persists, the problem is most likely outside your home. Call our Service Center at 611 for assistance.

Do Not Call Registry
National Do Not Call Registry The National Do Not Call Registry is open for business, putting consumers in charge of the telemarketing calls they get at home. The Federal government created the national registry to make it easier and more efficient for you to stop getting telemarketing calls you don’t want. You can register online at www.donotcall.gov or call toll-free, 1-888-382-1222 (TTY 1-866-290-4236), from the number you wish to register. Registration is free. The Federal Trade Commission, the Federal Communications Commission, and individual states are enforcing the National Do Not Call Registry. Placing your number on the registry will stop most, but not all, telemarketing calls. This site has information for you – whether you’re a consumer interested in signing up for the National Do Not Call Registry, or a telemarketer or seller interested in learning more about your responsibilities related to the Telemarketing Sales Rule.
Missouri No Call Registry The Missouri No Call registry is a list of residential customers’ telephone numbers who have indicated to the Attorney General’s Office that they don’t want to be called by telemarketers at home. The law prohibits telemarketers from calling households that have been added to the list, with some exceptions that have been written into the law. You can register all your home phone numbers.
You may register online at http://ago.mo.gov/divisions/consumer/no-call or toll-free 1-866-NOCALL1 (1-866-662-2551).
The state law allows certain telemarketers to continue calling consumers on the No Call list. These exceptions include: • Telephone companies. • Companies that have a current business relationship with you. • Those that you have expressly invited or permitted (for example, filling your name and number on an entry form). • Certain not-for-profit groups if the person calling is a member of the group. • Persons calling on a referral or working from their homes, or any person licensed for a trade or profession who is setting or attempting to set an appointment. To further decrease telemarketing calls under a federal law, tell telemarketers to put you on their No-Call lists. You can sue them if they call you back within a year.

Relay Missouri Service & How it Works
Emergency Assistance Callers should use 9-1-1 or other existing emergency numbers when possible in crisis situations. The Relay Missouri Center DOES NOT provide emergency assistance, but merely relays the emergency call. Delays may be encountered because the relay slows response time. Directory Assistance Callers should utilize existing numbers as provided in your telephone directories. TDD users need to call 1-800-855-1155 for directory assistance. Missouri Public Service Commission Inquiries/Complaints If you have complaints about a public utility (electric, gas, telephone or water), first try to resolve it by contacting the company. Speak to the company’s service representative. If the problem is not resolved, you may ask to speak with a supervisor. If your problem still is unresolved, please call the Missouri Public Service Commission recording machine toll-free at 1-800-829- 7541 (TTY). The Missouri Public Service Commission (MPSC) does not regulate cooperatives, municipal utilities, public water supply districts, cable TV systems or cellular service. Therefore, the MPSC does not have the power to resolve complaints concerning them. Billing of Calls The Relay Missouri Center handles local and long distance calls to numbers both within and outside the State of Missouri. There is no charge for local calls. Long distance calls are billed to the customer on a monthly basis. All long distance calls handled by the Relay Missouri Center are billed at a special discounted rate. All calls handled by the Relay Missouri Center are strictly confidential. Relay agents cannot disclose any information from a conversation, and records of conversations are not kept.
