Customer-Provided Equipment Customer-provided equipment may be connected to Fidelity’s facilities only if it is registered or otherwise complies with the FCC’s rules and meets the conditions set forth in Fidelity’s tariffs. These FCC rules and tariffs are designed to meet the convenience of the customer and at the same time protect the telephone network against any possible hazards to customers and Fidelity employees. A service charge may be incurred if Fidelity visits the custom- er’s premises and the service difficulty or trouble is found to have been caused by the customer’s equipment or facilities. If you have any questions concerning the use of customer- provided equipment, please contact the Fidelity Service Center at 573-468-8081. Inside Wire Maintenance Nobody likes unexpected repair bills. That’s why we offer Inside Wire Maintenance service. With Inside Wire Maint- enance, when a problem occurs in your home, you’re covered. We maintain the phone wiring outside your home up to the demarcation, at no charge, as part of our basic service to you. Maintaining the wiring inside your home is your responsibility, unless you subscribe to our Inside Wire Maintenance plan. By subscribing to Inside Wire Maintenance, you’ll receive all these services, if needed: • Diagnosis of your inside phone wiring by our trained personnel. • Complete analysis of your phone service problem. • Repairs to your inside wiring.

All it takes is one small monthly fee to get the worry-free cov- erage of Inside Wire Maintenance. For more information about this service, please contact your local business office listed on page 3. To subscribe to Inside Wire Maintenance service, your phone lines must not be part of a telephone system, such as a PBX or Key system. Inside Wire Maintenance does not cover repair of inside wiring due to natural disasters such as flood, wind and earthquake; riot and acts of war; fire; total destruction by fire; wiring that does not meet Fidelity’s installation practices or technical standards or is not in satisfactory working condition; gross negligence; willful damage; and/or vandalism.
