YOUR RIGHTS & RESPONSIBILITIES

DISCONNECTION OF SERVICE At Your Request: If you desire to disconnect your telephone service, you should notify the Telephone Company at least three (3) days ahead of time. You are responsible for all charges until the service is disconnected. Customers may not charge service or authorize the charging of service to any account which has been disconnected. With Notification: Customers will be notified in the “Billing Issues & Policies” section of the Phone Bill of restriction and disconnection dates for non-payment of services. All Phone Bills are due on or before the statement due date. Lifeline: Great Plains Communications (GPC) participates in the Lifeline federal assistance program to help eligible Hoosiers get local telephone/high-speed internet service and help them make their monthly payments. You may qualify for this program if you take part in at least one of these assistance programs: • Federal Public Housing Assistance (FPHA) • Medicaid • Food Stamps/SNAP • Supplemental Security Income (SSI) (Not the same as Social Security benefits) • Veterans Pensions and Survivors Benefit Programs Additional Details: • Household income must be no more than 135% of the Federal Poverty Guidelines. • Lifeline is non-transferable and only one discount is available per household (even if customer has both landline phone and high speed internet services). • Each year, Lifeline customers must certify that they are still eligible for the discount. • Customers must enroll in Lifeline service and must provide proof of eligibility before receiving support. • Customers who willfully make false statements in order to obtain the benefit are subject to fine or imprisonment or may be barred from the program. Enhanced Lifeline benefits are available to qualifying consumers who live on tribal lands.

Complaints and Review: In all cases, your service representative will try to answer your questions and resolve your problems. If you’re not satisfied, feel free to ask for a supervisor. If your problem can’t be solved by the supervisor, ask to speak to the manager or higher levels of management. If you are then not satisfied, you may file a formal complaint or request for a conference with the Company in person, by telephone, by letter or by using a complaint form available from the Company of the Indiana Utility Regulatory Commission. The Company will promptly investigate your complaint and contact you regarding its solution. No service will be disconnected for at least ten (10) days after the Company has notified you of its proposed disposition. If you are still not satisfied, you may appeal your case to the Public Service Commission of Indiana for an informal review. The request to the Commission must be made, in writing, within seven (7) days following the date the Company’s notification of its disposition of your complaint was mailed to you. A copy of your request to the Commission must be sent to the Company. Your request should be mailed to: The Commission will provide an informal review within 21 days, with a full written decision mailed to you and the Telephone Company no later than 30 days after receipt of your request. No service will be disconnected for at least three (3) days after the time the Commission mailed its decision, as long as you continue to pay all undisputed bills and undisputed portions of disputed bills. A copy of the rules and standards concerning customer service is available for inspection in the Company’s Business Office. In addition, the information in the general information page of the telephone directory is being updated as new directories are issued. Great Plains Communications has filed with the Federal Government a Compliance Assurance in which it assures the Rural Utilities Service that it will comply fully with all requirements of Title VI of the Civil Rights Act of 1964, as amended, Section 504 of the Rehabilitation Act of 1973, as amended, the Age Discrimination Act of 1975, as amended, and Rules and Regulations of the U.S. Department of Agriculture issued thereunder, to the end that no person in the United States shall, on the ground of race, color, or national origin, be excluded from participation in, be denied the benefits of, or be otherwise subjected to discrimination in the conduct of its program and the operation of its facilities. GENERAL INFO

Directory Regulations: Each subscriber is entitled to one single line insertion in the alphabetical list, without charge. Any additional listings are subject to a monthly charge. Telephone Numbers: Subscribers have no property rights to the telephone number. The Telephone Company may change the telephone number whenever it deems such change necessary in the conduct of its business. If the subscriber requests that the number be changed, there is a service charge and the account must have a zero balance. Residence telephones are installed with the understanding that they will be used for normal social and domestic purposes and not in connection with any business. Advertising a residence telephone number for business purposes will be considered sufficient cause for the Telephone Company to charge the business rate without further notice. Annoyance Calls: The laws of the State of Indiana provide that - Whoever telephones another person and addresses to or about such other person any lewd, lascivious or indecent words or language or whoever telephones another person repeatedly for the purpose of annoying, molesting or harassing such person, or his or her family, shall be deemed guilty of a misdemeanor. If you receive obscene, harassing or threatening calls, follow these instructions: 1. Hang up at the first obscene word or if the caller doesn’t say anything the second time you say hello. 2. Give no information such as your name and address until the caller has been identified. 3. Advise your children to give no information to strangers. 4. If calls persist, call your Police Department or the Telephone Business Office. Also, if you do not want to talk with a person selling a product or service by telephone, just say “No Thank You,” and hang up. Customer Owned Equipment: Customers are responsible for the maintenance and repair of their equipment. If the terminal equipment provided by the customer interferes with the proper operation of the telephone network, the Telephone Company may temporarily disconnect the customer’s service upon giving notice. Also, if our repair person checks your line and finds the trouble is in your phones, you will be charged a service charge. Listed below are a few tips on how to determine if the problem is your equipment: 1. Unplug your phone and move it to another jack in your home. If it works properly there, the problem is not with the telephone. 2. Check the connection on your modular jack and make sure it’s firmly fastened to the wall. 3. If you have dial tone, but suspect your phone isn’t ringing, check it out by asking a friend to make a test call. 4. Unplug your phone and bring it to our business office. We will check it with our equipment at no charge. If the problem is in our lines, we’ll dispatch a repair person at no charge.

Safety Information: Although Great Plains Communications (GPC) utilizes protective measures which are designed to eliminate harmful voltages to insure the safety of its customers, absolute protection is impossible. Customers are advised to avoid using the telephone during electrical storms. In addition, lightning may also produce acoustical shock which may be potentially harmful to the ear. Inside Wire Maintenance: The ownership of all inside wire was transferred from the Telephone Companies to the customers effective August 1, 1989. Inside wire is the wire and accessories connecting the protector on the outside of the premise to and including the jack to which your telephone attaches. All Telephone Company customers have the option of assuming the responsibility of maintaining their inside wire or subscribing to a maintenance plan. National Do Not Call Registry: The National Do Not Call Registry was created to prevent unwelcome calls from telemarketers. It prohibits all telemarketing except from companies with which you have an existing relationship and certain non-profit organizations from calling you. Your Telephone Billing Statement: The date your bill is mailed is determined by your telephone number prefix. Charges for your local service are billed one month in advance. Charges for long distance service and auxiliary equipment are due when the bill for such service is rendered. Long distance charges will appear on your Great Plains Communications (GPC) statement if your long distance carrier has contracted with GPC. If not, you will receive a separate bill from your carrier. Failure to receive a bill does not relieve the customer of the responsibility of paying promptly. Any bill not paid in full by the statement due date will be considered delinquent and assessed a late fee. SERVICES FOR CUSTOMERS WITH DISABILITIES

Calling Assistance for People with Hearing or Speech Disabilities The Americans with Disabilities Act of 1990 requires all states to provide access to nationwide Telecommunications Relay Services (TRS). Through the TRS, callers using Text Telephones (TTYs/TDDs) are able to communicate with people who use standard voice telephones. TRS communication assistants relay telephone conversations verbatim simultaneously between TTY users and non-TTY users. Relay Indiana operates 24 hours a day, seven days a week. Employees of the relay service are not allowed to disclose any information received during the relay of the call. Calls made through the relay service are confidential. Operators relay entire conversations, leaving nothing out. There is no charge for this service. Using a Text Telephone (TTY) device, someone with hearing or speech disabilities can call the TTY Communication Assistance Operator, toll-free at 1-800-743-3333 or 711 for assistance. Calls to this number are free, but long-distance and operator assistance charges will apply.
