Suggestions on How to Handle Obscene or Harassing Calls If you receive obscene, harassing, or threatening calls, follow these suggestions. 1. Hang up at the first obscene word or if the caller doesn’t say anything the second time you say “Hello.” 2. Give no information such as your name and address until the caller has been identified. 3. Advise your children to give no information to strangers. If you’re not home, they should say, “Mother/Father can’t come to the phone right now.” 4. Call the Sheriff’s Department if these annoy ing calls persist. Or, if you receive calls threatening injury to a person or damage to property, call the Sheriff’s Department imme diately. It is a crime under both Utah and federal law for anyone to make obscene or harassing telephone calls. Do Not Call Registry Most telemarketers cannot call your telephone number if it is in the National Do Not Call Registry. You can register your home and mobile phone numbers for free by calling 1-888 382-1222 or register online at www.donotcall. gov . Fraudulent Callers Are Subject to Prosecution It is illegal for another person to charge long dis tance calls to your number or credit card without your permission. Utah law also prohibits the use, possession, or sale of any devices for the purpose of com mitting a theft of telephone service. Any person violating these provisions may be subject to a

Unlawful Wiretapping Is Subject to Prosecution

It is a crime under state and federal law for any person, including a telephone subscriber, to wiretap or otherwise intercept a telephone call, unless that person has first obtained the con sent of one of the parties actually participating in the call. Properly authorized law enforcement officers can engage in interceptions without the consent of either party when proceeding under court orders issued pursuant to applicable pro visions of state or federal law. Emery Telcom does not install wiretaps for any one, but when a court orders a wiretap, we are required by law to cooperate. Emery Telcom believes in your right to privacy and will do everything possible to protect it. Customer’s Privacy When Emery Telcom provides our services, which include Internet access, local and long distance telephone, Voice over Internet Protocol (VoIP), broadband and data, we necessarily obtain cer tain information about you. This information may include your name, address, email address, tele phone number, date of birth, Social Security num ber, driver’s license number, credit information, payment information, and contact information. We may also gather information about how you use our services, such as numbers you call, when you call them, and particular services you use such as call forwarding or voice mail—this infor mation is called Customer Proprietary Network Information (CPNI). CPNI includes what services you subscribe to, how you use them, and what you are charged for them. It does not include your name, address, telephone number, or other types of information such as information about your telephone equipment or Internet access services. The Federal Communications Commission (FCC) has adopted an Order that furthers the privacy rights of telecommunications customers by imposing certain requirements on telephone and VoIP providers about how they can use your personal information. Emery Telcom takes your privacy seriously and is fully compliant with all state and fed eral laws regarding the confidentiality of your personal information. If your conversation is being recorded for business or other reasons, you will hear a beep tone every 15 seconds. Use of a recorder with out a recorder connector containing a beep tone warning device is not permitted, except on emergency reporting systems, including Sheriff and Fire Department lines.

What Emery Telcom Needs In addition to your name and address, Emery Telcom will need to know if and when you have had telephone service. You may also be asked for a number where you can be reached, such as your telephone number at work or the num ber of a close friend or relative who could get a message to you. If you have not had telephone service previously, or if we cannot verify your previous credit histo ry, a series of credit questions may be asked, such as: “Where are you employed?”, “What credit cards do you have?”, etc.

Types of Home Telephone Service The following basic phone services are available: Residential One-Party This single-party service allows you to make all of your local calls for the same charge each month. Business One-Party This single-party service allows you to make all of your local calls for the same charge each month. Business PBX Multiple business lines on a single directory number. Additional charges may apply. Charges to Install and Change Your Service The one-time charges to install or change your service may vary depending on the type of work that is done. The following explains how we determine the charges: Service Order Charge You are charged for the processing of your order when you install or make changes in telephone service.

A Deposit May Be Required A deposit may be required if you have had an unsatisfactory payment record with any tele phone company, or if you have not supplied adequate credit information. If your service is disconnected due to late payment, and there is either no deposit or an insufficient deposit, a new or additional deposit may be required to continue service. You also have the option of providing a letter from a person willing to guarantee payment of your bills if that person has established satis factory credit with us.

Line Connection Charge This charge is for the work we do to connect your line to our switching office. Premise Visit Charge This is the charge you pay when a technician visits your home. These rules cover payment of bills, late charges, security deposits, handling complaints, service disconnection and other mat ters. Call Us First Our business office representatives have a basic responsibility to answer your questions and resolve your problems. If you’re not satisfied, please ask for a supervisor. If you are still not satisfied, you have the right to file a formal or informal complaint with the Public Service Commission of Utah (PSC), which regu lates Emery Telcom. Telephone Safety The telephone is one of the safest appliances in your home or office. There are, however, a few situations where a telephone user needs to be cautious. Avoid Use of the Telephone Near Water The telephone should not be used while you are in the bathtub, shower or pool. Immersion of the telephone or handset in water could cause an electrical shock. You should avoid using a telephone during an electrical storm in your immediate area; calls of an urgent nature should be brief. Although Emery Telcom uses protective measures to limit abnormal electrical surges from entering your home, absolute protection is impossible. There is a remote risk of a dangerous electrical shock from lightning when using the telephone during If you think you’ve found a gas leak, you should not use a telephone in the vicinity of the leak until the leak is repaired. The telephone contains electrical contacts that could generate a tiny spark when you lift the handset and dial. While unlikely, it may be possible for this spark to trigger an explosion if the gas concentration is high enough. Avoid Use of the Telephone During an Electrical Storm a nearby electrical storm. Avoid Use of Telephone to Report a Gas Leak

How to Get Credit for Loss of Service

If you have reported to the Emery Telcom business office that you are unable to make or receive calls and your phone is out of service 24 hours or more, you are entitled to a credit on your next bill of the local service charges for the out-of-service period. Directory Accuracy Every effort is made to keep the directory as accurate as possible. Emery Telcom, however, assumes no liability for damages arising from errors and omissions in the directory except as provided for in its filed regulations and tariffs. Providing Your Own Equipment Under the Federal Communications Commis sion’s (FCC) registration program, you may supply your own telephone or other terminal equipment and connect it directly to the nation wide telephone network. The equipment must be approved by the FCC and you are responsible for its proper connec tion, maintenance and repair. Private equipment may not be connected to coin phone service and party lines.

CUSTOMER INFORMATION

Universal Service Programs The FCC, Emery Telcom and its affiliate com panies are committed to the goal of Universal Service for the residents of Carbon County, Emery County and Hanksville. Universal Service is the promotion of quality telecommunication services at just, reasonable and affordable rates. Basic service includes: phone line providing single-party, voice grade access to the pub lic switched telephone network utilizing dual tone, multi-frequency signaling local usage on this phone line, access to emergency services, access to inter-exchange service, access to Directory Assistance (additional charges per inquiry), and toll limitation for qualifying low-in come consumers. Federal and State Lifeline Assistance Program The Lifeline Program provides a monthly credit to lower qualifying residential monthly voice or Internet service charges. Eligible customers may enroll in the program if they have an income at or below 135% of the federal poverty guidelines or participate in one of several federal assistance programs. Participation in the Lifeline program places a freeze on the Lifeline benefit for a peri od of 60 days (if applied to voice only service) or 12 months (if applied to a qualifying Internet service). During this time, you must remain with your current provider in order to continue receiv ing the discount, unless you can claim a valid exception. Customers applying for Lifeline bene fits will need to complete a certification form and provide proof of eligibility by income verification or qualifying program participation. Lifeline cred its are limited to one benefit per household, wire line or wireless. Lifeline is a government benefit program and customers who willfully make false statements in order to obtain the benefit can be barred from the program and punished with fines or imprisonment.

Utah Relay Service Text Telephone/TTY – A person who is deaf, hard of-hearing, deaf-blind, or speech disabled can use a TTY to type his or her conversation to

a communication assistant (CA) who then reads the typed con versation to a hearing person. The CA relays the hearing person’s spoken words by typing them back to the TTY user. Voice – Standard telephone users can easily initiate calls to TTY users. The relay operator types the telephone user’s spoken words to the TTY user and reads back the typed responses. Voice Carry-Over (VCO/2LVCO) – VCO is ideal for people who can use their voices to speak directly to the hearing person. When the hearing person speaks, the CA types everything that is said to the TTY or text display. Hearing Carry-Over (HCO) – HCO allows speech-disabled users with hearing to listen to the person they are calling. The HCO user types his or her conversation for the CA to read to the standard telephone user. Speech-to-Speech (STS) – STS allows people with speech disabilities to voice their conversa tion. A specially trained Relay Utah CA will listen and repeat the speech-disabled user’s dialogue to the called party. No special telephone equip ment is necessary to use this service, as STS calls can be made from any standard telephone. Captioned Telephone (CapTel) – The captioned telephone, CapTel, is ideal for a person who is hard-of-hearing or deaf and uses his or her voice to speak directly to the called party. When the hearing person talks, the CapTel user reads the responses on a text display. CapTel allows peo ple to place calls just like a traditional phone by dialing the number directly. The CapTel automat ically connects to the Captioning Relay Service when the number is dialed. OPTIONAL SERVICES & FEATURES General Information During conversations, many features can only be used by first depressing the switchhook. To do this, depress the switchhook quickly, then release it within a half second. Caution: If you hold the switchhook down too long, your call will be disconnected. • Some features instruct you to use the # button. If you have a rotary or pulse dial telephone, dial 11 instead of using # . • Some features instruct you to use the # button after dialing a code or number. If you have a rotary or pulse dial telephone, you can ignore the # button and wait four seconds for your feature request. • When certain features are activated you may hear a Distinctive Dial Tone. This is a high- pitched tone which reminds you that a fea ture you have activated is still in operation. Call Waiting Call Waiting alerts you with a beep during a tele phone conversation that another call is waiting to be answered. What Call Waiting Does for You: • Prevents missed calls. • Callers will hear ringing, not busy signals. • Provides many of the advantages of an additional line, but at a fraction of the cost. To End an Existing Call and Answer a Waiting Call: Hang up, then allow the telephone to ring and answer it. To Hold an Existing Call and Answer a Waiting Call: Depress switchhook or flash button. To Alternate Between Calls: Depress switch hook or flash button. To Disconnect Calls: Hang up. Cancel Call Waiting Cancel Call Waiting allows you to cancel Call Waiting before or during one telephone call. What Cancel Call Waiting Does for You: • Prevents Call Waiting interruptions on important calls and long distance calls. • Prevents data transmission errors caused by Call Waiting tones when using com puter modems.

To Cancel Call Waiting Before Making a Call: 1. Press *70 , then listen for three beeps and a steady dial tone. 2. Dial desired telephone number. To Cancel Call Waiting During a Call: (Three-Way Calling feature is required): 1. Depress switchhook, then listen for three beeps and a steady dial tone. 2. Press *70 , then listen for three beeps. 3. Wait for automatic reconnection to existing call. Note: When Cancel Call Waiting is activated, callers will hear a busy signal. Call Forwarding Call Forwarding allows you to redirect all calls to another telephone number. What Call Forwarding Does for You: • Reduces missed calls. • Calls can follow you wherever you go. • Eliminates waiting for important calls. • Enhances home security when you are away. To Forward Calls: 1. Press *72 , then listen for three beeps. 2. Press # (or wait for 4 seconds), then listen for three beeps. 3. Hang up. For Fiber Customers: 1. Press *72 plus 10-digit phone number, no spaces. 2. Press # (or wait for 4 seconds) then lis ten for three beeps. 3. Hang up. To Cancel Call Forwarding: 1. Press *73 , then listen for three beeps. 2. Hang up. Note: Calls forwarded to long distance tele phone numbers will be charged to you. Three-Way Calling Three-Way Calling allows you to add a third party to an existing telephone conversation. What Three-Way Calling Does for You: • Reduces the need for callbacks when trying to confirm plans and schedules. • Brings families and friends together.

Three-Way Calling (Cont.) To Set Up a Three-Way Call: 1. Depress switchhook or flash button, then listen for three beeps and a steady dial tone. (Present call is placed on hold.) 2. Dial third party’s telephone number. If busy or no answer, depress switchhook or flash button twice to reconnect first call. 3. After third party answers, depress switch hook for a three-way call. Notes: a. You may privately converse with third party as long as you wish before using switchhook to establish three-way call. b. After a three-way call is established, you may depress switchhook to drop third party at any time. Speed Calling Speed Calling allows you to call selected telephone numbers quickly by dialing an assigned code. What Speed Calling Does for You: • Eliminates the need to look up or remem ber telephone numbers. • Saves time when dialing numbers, espe cially long distance numbers. • Allows fast, accurate dialing of emergency numbers. To Set Up Speed Calling Numbers: 1. Press *74 for 1-Digit Speed Calling (codes 2 - 9), - or - Press *75 for 2-Digit Speed Calling (codes 20 - 49). 2. Listen for three beeps and a steady dial tone. 3. Enter Speed Calling code (2 - 9 or 20 - 49). 4. Enter desired telephone number, press # then listen for three beeps. 5. Hang up. To Use Speed Calling: • Press desired Speed Calling code ( 2 - 9 or 20 - 49), then press # . To Change Speed Calling Entries: • Repeat first five steps.

General Information Advanced Calling Services offer you conve nience, time savings and greater control in the use of your telephone. This guide will allow you to begin enjoying the many benefits of your advanced Calling Services. • Advanced Calling Services work only on calls made within the Advanced Calling serving area. These services are not avail able in all areas. • If you attempt to use one of your Advanced Call Services with a number outside the area served by Advanced Calling, you will hear an announcement telling you that the service cannot be used with that particu lar number. • Some of the Advanced Calling Services require you to press the * button. If you have a rotary dial phone set, dial 11 instead of the * . This only applies to the first * in the dialing sequence for a service. • Some of the Advanced Calling Services require you to program lists of phone numbers. You must set up these lists prior to using the services. Please refer to the instructions in this guide to program the lists. • For many of the Advanced Calling Services, voice instructions will guide you through the various procedures and sim plify their operation. Call Return allows you to dial a code and have a call automatically returned to the last party who called or attempted to call you. What Call Return Does for You: • Eliminates the aggravation of rushing to the phone when you’re returning from errands or in the shower only to find that the person on the other end has hung up. • Makes it easy to return calls that would normally be missed. To Use Call Return: 1. Press *69. 2. Listen for an announcement that will tell you the phone number of the party who last called you. Call Return (Last Number Redial)

If the line is busy: • Listen for announcement telling you the number is busy. • Hang up . • You will hear a short-short-long ring when the line is free. • Your call will automatically be made when you lift the handset. To Cancel Call Return: Press *89 and listen for tone or announcement. Notes: a. There is no time limit for returning a missed call. However, you will only be able to return the last incoming call you received. b. If the number you are trying is outside the area served by Call Return, you will hear a recording advising you that the call cannot be made. c. After a call during which you heard a Call Waiting tone, you can use Call Return to return a call to the Call Waiting number. Priority Ringing Priority Ringing allows you to program your tele phone line to ring with a special ringing pattern whenever you are called from a select list of phone numbers. Your phone will ring with a nor mal ringing pattern for all other calling numbers. If you also have Call Waiting, you will hear a distinctive Call Waiting tone whenever someone on your list calls you while you are on the phone. What Priority Ringing Does for You: • Allows you to identify special and import ant calls. • Helps you determine who’s calling. To Use Priority Ringing: 1. Press *61. 2. Listen to the voice instructions which will guide you through the steps of how to: • Turn Priority Ringing on / off. • Make changes to your Priority Ringing list.

Call Screening Call Screening allows you to create a list of phone numbers from which you do not wish to receive calls. Calls from phone numbers on your list are sent to an announcement that informs the caller that you are not receiving calls at this time. What Call Screening Does for You: • Allows you to block calls from people with whom you do not wish to speak. • Enhances security and privacy. To Use Call Screening: 1. Press * 60. 2. Listen to the voice instructions which will guide you through the steps of how to: • Turn Call Screening on/off. • Make changes to your Call Screening list.

3. After receiving an annoying call you may wish to prevent that person from calling you in the future. Call Screening’s voice instructions will explain how to add the number of the last caller to your list (even if you do not know that party’s number): • Hang up, then lift the receiver and listen for dial tone. • Press *60 and listen for instructions. • Press #01#. Note: If a number that is on your Call Screening list is also put on any of your other lists (for example, Priority Ringing), Call Screening will override the other services for that phone number.
