Rights & Responsibilities

Lifeline Service Lifeline Service is a program designed to make telephone and broadband service available at reduced rates to eligible residential custom- ers. The Federal Program assists low-income persons and the Missouri Universal Service Fund Program assists low-income or disabled persons. Eligible residential customers who apply for Lifeline Service will receive a reduction in their basic monthly service charge. Lifeline Service does not discount installation or service connection charges, long distance services, or calling features (Call Waiting, Call Forwarding, etc.). For information about Federal Lifeline Assistance or any other telephone and broadband assis- tance programs, you may contact our business office at 573-663-2000. Lifeline information can also be found at www.lifelinesupport.org or by calling 800-234-9473. To determine if you qual- ify for assistance, visit www.checklifeline.org . Public Service Commission This telephone company is regulated by the Missouri Public Service Commission, and you can make an informal complaint in writing or by telephone or personal appearance to the Commission without the need of legal assis- tance. Rights and Responsibilities of Ellington Telephone Company and Our Customers Billing Ellington Telephone Co. will issue a bill not less than every 28 days or more than 31 days (except for number changes), outlining your toll calls, local service charges and federal, state and local taxes. Payment will be due upon receipt and will become delinquent 15 days after mailing. In the event your bill becomes delinquent, a ten-day notice will be sent before disconnecting your service. Basic local service will not be discontinued for non-pay- ment of any charges other than charges for basic local service. A restoral of service charge will be applied if your service is disconnected. Under our billing procedures and subject to rules established by the Missouri Public Service Commission, we may require a deposit or guarantee of payment. In the event your bill becomes delinquent when a member of your family has a medical emergen- cy requiring the continuance of your telephone service, please call the business office and we may continue your telephone service during such emergency for up to 21 days. Our tele- phone number is 663-2000. Arrangements can also be made with our business office to avoid discontinuance during a period of absence from your home. We will make every effort to clarify and resolve all disputes. Our business office is open from 8:30 A.M. to 4:30 P.M., Monday thru Friday (except holidays), for handling inquiries by telephone or by personal visit.

Service Difficulties Ellington Telephone Company will appreciate receiving prompt reports of service difficulties. We also appreciate receiving prompt reports from anyone concerning poles, wires, or other telephone equipment that might endanger lives or property. Bills and Payments Telephone bills are rendered monthly and are due upon presentation. If bills remain unpaid after 15 days following the date of the bill, service may be denied. For your convenience, a return envelope is included with each monthly bill. Simply detach the bottom portion of your bill and return with your payment. Bills can be paid via mail, online at mccormacksolutions.com , or at the follow- ing locations:

In some cases it is possible for calls to an incor- rect number to be rerouted to the correct num- ber. To arrange this, call our business office and a representative will be happy to help. Every customer receives one listing in the White Pages of the telephone directory without charge. Each business customer will receive one listing in the White Pages and one listing in the Yellow Pages, without charge. Annoying Telephone Calls

Place your telephone number on the National Do Not Call Registry by calling 888-382-1222 or online at www.donotcall.gov . Harassment Calls It is against the law to make obscene, threat- ening, or nuisance calls. If you are unfortunate enough to be a victim of a threatening call, report it to the police and the telephone company immediately. We will work with you and the police to eliminate the problem. If you receive obscene or nuisance calls and have identified them as such, here are some suggestions: • Don’t give the caller a chance to get started. Hang up. • Do not provide any information about yourself. • Advise your children to do the same. If the calls continue, contact Ellington Telephone Company for advice. We have representatives who are trained to investigate and help identify the source of these calls.

Telemarketing Calls Here are some suggestions that will help you handle telemarketing calls: • Identify the calling party – always verify the name of the company and individual with whom you are speaking. • Request that information be sent to you via mail, if you are interested in the telemarketer’s offering. • If you are not interested, politely interrupt the caller and end the conversation. • If you do not wish to continue receiving solicitations from a particular company, ask to be removed from the company’s list. • Do Not Give Your Credit Card Information unless you are certain the call is from a trusted source. To reduce or eliminate the number of at-home solicitation calls from telemarketers, call the Missouri Attorney General’s “No Call” Hotline at 866-662-2551. Special Information

Warning The law of the State of Missouri makes it unlaw- ful for any person to: make a telephone call during which he makes any comment, request, suggestion, or proposal that is lewd and las- civious; or make a telephone call, whether or not conversation ensues, without disclosing his identity and with intent to annoy, abuse, threat- en or harass any person at the called number; or make or cause the telephone of another to continuously ring, with intent to harass any person at the called number; or individually or in conspiracy or concerted action with other persons make repeated telephone calls, during which conversation ensues, solely to harass any person at the called number. It shall be unlawful for any person to knowingly permit any telephone under his control to be used for any purpose prohibited by this sec- tion. Any person who violates the provisions of this act is guilty of a misdemeanor and shall, upon conviction, be punished by confinement in the county jail for not more than one year or by fine not exceeding one thousand dollars, or both such fine and confinement. Any offense committed by use of a telephone as set forth in the foregoing section shall be deemed to have been committed at either the place where the telephone call or calls originated or at the place where the telephone call or calls were received.

Warning Any person, firm, or corporation, who, with intent to defraud or to aid and abet another to defraud any person, firm, or corporation of the lawful charge, in whole or part, for any goods, wares or services, shall obtain, or attempt to obtain, or aid and abet another to obtain or attempt to obtain, any goods, wares or services by charging such goods, wares or services to an existing tele- phone number, credit card, credit card number, or other credit device, without the authority of the subscriber thereto or the lawful holder thereof, or by charging such goods, wares or services to a non-existing, false, fictitious, or counterfeit telephone number, credit card, credit card number, or other credit device, or to a suspended, terminated, expired, canceled, or revoked telephone number, credit card num- ber, or other credit device, or by use of a code, prearranged scheme, or other similar stratagem or device whereby said person, firm, or corpora- tion, in effect, sends or receives information, or by installing, rearranging, or tampering with any facilities or equipment, either physically, induc- tively, acoustically, or electrically, or by any other trick, stratagem, impersonation, false pretense, false representation, false statement, contriv- ance, device, or means is guilty of a misdemean- or and shall, upon conviction, be punished by confinement in the county jail for not more than one year or by fine not exceeding one thousand dollars, or both such fine and confinement.
