Making an Emergency Call

Dial 911 • Be clear as to what kind of assistance is needed: fire, police, medical. • Stay on the phone and answer all questions. • Give the specific location where help is needed (know the nearest intersection, for example). • Stay calm, and speak clearly. Remember to wait for help to arrive so you can direct it to the emergency. Follow the directions of the call taker. If you know basic first aid and CPR, you can help people by providing emergency assistance until professional help arrives.

9-1-1 calls are answered by trained public safety dispatchers whose priority is the handling of emergency situations. Please take the time to look up a non-emergency telephone number for the appropriate government agency if you do not have a problem that requires a law enforcement officer, fire department or ambulance to come to your location immediately. Do not call “9-1-1” or the police to confirm an earthquake. Listen to your local radio or TV station for information. TTY, TDD Emergency Calls Dial 9-1-1 Tap the space bar until someone answers. After your call is answered, pressing different TDD keys may help shorten the time necessary to respond to your call. Blocking In case of extreme congestion of the telephone network, Cal-Ore Telephone Co. and/or long distance carriers may institute blocking. Blocking prevents overloading the system by diverting some calls to recordings, allowing other calls to complete. If you need to place an emergency call: 1. Make sure receivers of all extension phones are on the hookswitch. 2. Stay on the line. You may not hear a dial tone immediately; the delay could be as long as a minute or more. 3. Do not repeatedly depress the hookswitch, as this will further delay your call. 4. If you receive a “fast busy” or “all circuits are busy” recording, hang up and try again. 5. If physical damage occurs in our equipment or facilities or your wiring, it may not be possible to complete your call. It may be easier to call out of the disaster area during emergencies. Contact a friend or a relative outside the immediate area and have that person relay messages to reduce calling in and out of the affected area.

General Business Service • Tariffs • Bills • Charges

Tariffs and Rules Cal-Ore’s current tariffs, on file with the California Public Utilities Commission, and consumer pro- tection rules for telephone service and facili- ties are available for public inspection in our Business Office. Payment of Telephone Bills All telephone bills are mailed on approximate- ly the first of the month. Bills are due upon presentation and are delinquent fifteen days thereafter. Delinquent accounts will be advised of a disconnect date by means of a “Temporary Disconnect” notice. Late Payment Charge A late payment charge is applicable if payment is not received in our office by the 15th of every month. The late payment charge of 1.5% will be applied to an unpaid balance of $5.00 or more, with a minimum of $0.30. Neither the billing or payment of late charges relieves the customer of the obligation to pay all charges prior to the due date of the bill. Failure to pay charges (i.e., basic flat rate sin- gle line service charges including all mandated surcharges and taxes) may result in a discon- nection of telephone service. Basic Service is defined in D. 96-10-066, Appendix B, page 5. Other services, such as the ability to make toll calls, may be restricted if not paid. Optional services may be disconnected. If service is disconnected for non-payment, a reconnection charge and/or applicable deposit If you have a question or complaint about your service or billing, please call the customer service number on your telephone bill, 530- 397-2211. Disputed Telephone Bills

To avoid having your service discontinued, deposit the disputed amount of the bill, along with an explanation of the dispute, to: California Public Utilities Commission Consumer Affairs Branch State Office Building 505 Van Ness Avenue, Room F2003 San Francisco, CA 94102-3298 The Commission will review the basis of the billed amount and make disbursements in accordance with its findings. Interstate Access Charges The Federal Communications Commission is responsible for authorizing interstate access charges. Any questions regarding the interstate access charge portion of your bill should be directed to: Federal Communications Commission Universal Lifeline telephone service is a discounted telephone service available only to qualified res- idence customers. You qualify for this service if: 1. Your total household gross income is no greater than $25,900 for one to two per- sons. A sliding scale of income eligibility is used for households with more than two persons. 2. You have only one telephone line to your home, and you receive Lifeline service only at your principal place of residence. 3. You are not claimed as a dependent on another person’s income tax return. General Business Important Information for our Customers About Your Telephone Service.

What is a Preferred Interexchange Carrier (PIC) Freeze? A PIC freeze means that your carrier cannot be changed unless you contact us to change it. The PIC freeze may provide additional protection against unauthorized long distance changes. There is no cost to freeze or unfreeze your choice of carrier. Just call us. We have the form for your PIC freeze authorization. We also have a form to unfreeze the PIC or you can call us and give us the required personal verification data in order for us to ensure it is you that is giving us the verbal authorization. What is Toll Restriction? Toll restriction stops unauthorized long distance If you get billed for 900 and 976 calls, residential customers may receive a one-time refund from the carrier if you did not know the calls were chargeable or a child or someone else made the calls without your permission. You may also get a refund if you are not satisfied with a 900 service for any justified reason like poor transmission quality, dissatisfaction with the quality or value of the 900 service, or disputes over the amount of 900 charges billed. To get a refund you must call the carrier within 60 days following the date of the bill in question. We can only adjust 900 calls that appear on our bills. We offer 900 blocking free of charge. Basic telephone service, as defined by D. 96-10-066, Appendix B, page 5, will not be disconnected solely for nonpayment of 700, 900, 976 charges. General Business Locating the Problem When Phone Service Doesn’t Work

When your phone doesn’t work, the problem may come from the telephone set or wiring inside your home/business or the telephone line outside. Where the problem is determines who is responsible for repairs. The diagram illustrates what your telephone company will repair at no charge to you, and what part of telephone service is your responsibility. If You Have a Problem… Attempt to determine if the source of the service interference is caused by your tele- phone or the inside telephone wire. 1. Hang up all telephones. Make sure phone accessories – answering or fax machines, computers, modems, security system, and telephone sets are work- ing. To check, unplug the phone cord from the unit or the jack and check for damage. Leave everything unplugged for 6 minutes. Then try plugging in phone sets one at a time to see if the trouble still exists. 2. If all the phone accessories are working, go to your network interface box. (Outside, where the phone line enters the house). 3. Test to determine if the problem is inside or outside the house: 1. Take a working phone to the network interface box (single line phone only). 2. Remove the modular plug from the test jack and insert the modular plug from your phone. If the same problem occurs: The problem may be in the phone lines. Call your local telephone company. If the problem is in the phone lines, they will repair the lines at no charge to you. If the problem does not occur: The problem is in your wiring. Call your local telephone company or another company to repair your inside wiring.

Custom Calling Features Extra Services for Your Convenience

Cancel Call Waiting To cancel Call Waiting, press *70 , if you have touch-tone service, or dial 1170 if you have a rotary phone. Wait for the dial tone. Then dial the number you wish to call. Call Waiting has been cancelled for the duration of this call only and will be automat- ically restored when your call is completed. Three-Way Calling Add another person to an existing conversation by depressing the hookswitch for one second, and dialing the additional person’s number when you receive a dial tone. The 3-way connection is completed after another hookswitch flash. Add a Third Person to Your Conversation as Follows: 1. Depress hookswitch firmly (one second). 2. Release immediately. This puts the first party on hold. 3. Listen for three brief tones, then a dial tone.

Call Waiting If someone dials your number while you are using your telephone, a short tone will notify you of the call. The calling party will receive normal ringing tone and cannot hear your conversation. If you desire, you can put the original party on hold by depressing the hookswitch for one sec- ond and answer the second call. You may alter- nate between each call as often as necessary, but may not talk to both parties at the same time. A brief tone will alert you when another call is waiting to be answered. A reminder tone will sound 10 seconds later. To Answer the Waiting Call You may terminate the existing call and hang up. Your phone will ring. Answer it and you will be connected to the waiting call. If you wish to answer the waiting call and hold an existing call, depress the hook- switch firmly and release it immediately. Return to the First Call

By depressing the hookswitch firmly and releasing it immediately, you will return to the first call. Alternate Between the Two Calls As often as necessary by depressing the hookswitch firmly and releasing it immediately each time, you can alter-

To Restore Normal Answering: 1. Dial *73 if touch-tone. Dial 73 , and pause if rotary. 2 . Listen for two bursts of tone.

The first person on the connection simply hangs up. The third person is removed by depressing the hookswitch and releasing immediately after the third person hangs up. All three connections are automatically discon- nected when you hang up.

Some Tips: Call Forwarding continues until you cancel it. You can make outgoing calls while Call Forwarding is in effect. One short ring will be heard at your telephone when a call is being forwarded. Notify the person to receive calls if Call Forwarding is used from a measured service telephone, a local mes- sage charge applies for each call forwarded. Call Forwarding to long distance numbers will be charged to you at long distance rates. Caller ID Caller ID is a service that allows you to see who is calling before you answer the phone. There is no way to know if the party you are calling has the service. Caller ID service requires a display unit which will also store the name and tele- phone numbers of recent callers. Calls marked “PRIVATE” or “ANONYMOUS” are from callers who have chosen to block delivery of their name and number. Caller ID also shows your telephone number and name to a person or business you call. Whenever you make a phone call, your tele- phone number is transmitted to those who have Caller ID service and display equipment, even if you do not have the service itself .

Call Forwarding If you are going to be at another location (telephone number) or you wish your calls to be answered by another person, you may do so by entering the desired telephone number. All your incoming calls will be forwarded automatically. You may still use your telephone for outgoing calls.

To Forward Either Local or Long Distance Calls: 1. Dial *72 if touch-tone.

Dial 72 (rotary dial telephones), wait for second dial tone. 2. Dial the telephone number to which your calls are to be forwarded. Ringing will be heard. When the called party answers, Call Forwarding is in effect. If the line is busy or there is no answer, hang up immediately and repeat steps 1 and 2. Call Forwarding will be established automatically when you hear two bursts of tone.

Custom Calling Features Extra Services for Your Convenience

Complete Blocking (With Per Call Enabling)

Press *82 to deactivate on a per call basis. Complete Blocking is a free service that gives you permanent control over the transmission of your phone number. To activate the service you must specifically request it from Cal-Ore Tel. Co. Complete Blocking blocks the trans- mission of your telephone number on ALL of the calls you place, unless you specify otherwise. There is no need to enter a code before making each call. Those with Caller ID units have the word PRIVATE displayed. You may deactivate Complete Blocking on specific calls by pressing *82 on your touch calling phone (or 1182 on a rotary dial phone) before you dial. Non-Published Telephone Numbers If you do not choose a blocking option, you are automatically provided Selective Blocking unless you have non-published service. If you have non-published service you must be defaulted to Complete Blocking as required by the California Public Utilities Commission. These blocking options are free. If you want to change your initial blocking option, you may do so one time free of charge. Important Note You cannot block the transmission of your telephone number for calls to 9-1-1, 800/877/888 and 900 service, regardless of the blocking option you choose. If for some reason, you want to report an emergency without having your number displayed, you should call the agency’s seven digit num- ber instead of 9-1-1. If the number you are calling from is not equipped with Complete Blocking, you will need to press *67 (or 1167 from a rotary dial phone) before you dial the agency’s number in order to block your number from being shown.

To Block Anonymous Calls Use Anonymous Call Rejection, a free service for Caller ID customers, by lifting the receiver and dialing *77 (rotary 1177). Anonymous Call Rejection informs callers who have blocked their name and number that the party they are trying to reach does not accept blocked calls. The blocked caller is informed that they can unblock their call and redial. To cancel Anonymous Call Rejection, lift the receiver and dial *87 (rotary 1187). Your Privacy Whether you choose to transmit your number or prevent it from being shown, Cal-Ore Tel. Co. agrees that the display of your telephone num- ber should be the result of your informed con- sent. You have a legal right to choose whether or not your telephone number will be displayed on a Caller ID unit. That’s why Cal-Ore Tel. Co. provides free block- ing services that give you the freedom to choose if, when, and how your number will be shown to those you call. You may change your blocking option once without charge. Your Choice... Selective Blocking Press *67 before placing your call. Selective Blocking is a FREE service that blocks your telephone number from being transmitted and/or seen on Caller ID units on a per call basis. By simply pressing *67 on your touch calling phone (or 1167 from a rotary dial phone) before placing a call, your phone number will not be trans- mitted. Those with Caller ID units who receive your call will see the word PRIVATE displayed.

Voice Mail

Voice Mail is an Electronic Messaging Service • No machines to buy. The system is “built-in” to your telephone through your local business office. Call for activation and cost of this service. • Every message that you receive is automati- cally recorded with a date and time stamp so you know when messages arrived. • You can check your voice mailbox for mes- sages from any touch-tone telephone – any- time, anywhere. You can even check your messages from your email. • Voice Mail answers your calls and will take messages even when you are talking on your telephone, so you don’t miss messages because people calling got a busy signal. • Access to your mailbox is secured by your own personal code/password. Multiple sub-mailboxes are also available. Voice Mail Definitions: • Mailbox: Where your messages are received and stored. • Mailbox Number: Your 10 digit home or business phone number. • Voice Mail Number: The phone number you call to retrieve your messages: Dorris/Macdoel 397-3030; Tulelake/Newell 667-5000. Dial 1-530 then appropriate number when checking your mailbox from outside our calling area. • Password: Your personal identification number between 4 to 7 digits to open your “electronic” mailbox . If calling from the phone your voice mail is assigned to, Press # , or If calling from a remote phone, enter your mailbox number (your 10 digit telephone number), or If using Auto Login and accessing from your home phone, no entry is required. If requested, enter your password, fol- lowed by the # key. Your password will be a default of four zeros (0000) until you change it. Voice Mail Tips: • Situation: You need to leave a mes- sage in your own or someone else’s mailbox but you do not want to listen to the whole greeting. • Solution: Simply press # when you hear the greeting start. You’ll hear the record- ed tone. Then leave your message. • Situation: You deleted a played mes- sage and you wish you hadn’t! • Solution: Press 6 to replay your “delet- ed” message, then press 2 to re-save the message before playing the next message or exiting your mailbox.

California Relay Service This service relays calls between a person using a TDD and any other telephone user within the United States. It also allows a person without a TDD to call a TDD user. Specially trained per- sonnel are available 24 hours a day, 7 days a week, 365 days a year to relay the call. There is no extra charge for this service. Avoidance of Lawful Charges Persons using unauthorized calling card num- bers or unauthorized telephone numbers to avoid charges will be prosecuted. Anyone violating Section 502.7, California Penal Code, by attempting to avoid lawful charges for tele- phone service may be fined up to $5,000 and imprisoned up to 5 years.

General Information TDD Service - Problem Solving

How to Handle Annoyance Calls It is against the law in the state of California to make an obscene, harassing or threatening tele- phone call. When you receive such a call, follow these suggestions: 1. When answering your telephone, say hello twice. If no answer, HANG UP. 2. Do not give information until you are abso- lutely certain you know who is speaking. 3. Instruct children not to give any information to strangers over the phone. 4 . Hang up when your hear something obscene or annoying. 5 . Never reveal the fact that you are alone. 6. When annoyance calls persist, contact your local law enforcement agency. 7. Calls of a threatening nature should be reported to the local law enforcement agen- cy immediately. Automatic Answering Sets Long distance charges apply to station calls made to telephones with automatic answering sets. Charging begins when the called telephone is answered by the automatic answering set. Telephone Safety The telephone is one of the safest appliances in your home or office. There are, however, a few situations where a telephone user needs to be cautious. Use of the Telephone Near Water The telephone should not be used while you are in a bathtub, shower or pool. Immersion of the -telephone or handset in water could cause an electrical shock. Use of the Telephone During an Electrical Storm You should avoid using a telephone during an electrical storm in your immediate area; calls of an urgent nature should be brief. Although Cal- Ore Telephone Co. uses protective measures to

limit abnormal electric surges from entering your home, absolute protection is impossible. There is a remote risk of a dangerous electrical shock from lightning when using the telephone during a nearby electrical storm.

Use of the Telephone to Report a Gas Leak

If you think you’ve found a gas leak, you should not use a telephone in the vicinity of the leak until the leak is repaired. The telephone contains electrical contacts that could generate a tiny spark to trigger an explosion if the gas concen- tration is high enough. Protect Your Consumer Rights Help Protect Against Unsolicited Telephone Marketing Calls & Faxes!

State Law (Assembly Bill 2134), effective January 1, 1999, requires the Telephone Company to provide customers the following information. Various governmental agencies publish information that generally describes telephone subscribers’ rights under the state and federal law. As a consumer, you have rights to privacy and to protect yourself from potential telemarketing fraud, unsolicited sales calls and faxes. The laws apply to live calls, prere- corded voice calls & artificial (computerized) voice calls. The Federal Communications Commission (FCC) adopted rules and regulations, effective December 20, 1992, imple- menting The Telephone Consumer Protection Act (TCPA). Different rules and regulations apply to calls placed to homes and business. The Federal Trade Commission (FTC) also has Telemarketing Sales Rules, which have been enacted to help protect you as a consumer. State laws are in place to protect citizens who have had their privacy violated or have been victims of fraudulent telemarketers. The State Attorney General’s office, along with the Telemarketing Sales Rules, has the authority to investigate and prosecute fraudulent telemarketers who operate across state lines.

Protect Your Consumer Rights Rules for Monitoring Calls

California laws strictly protect your right to privacy. Your telephone calls may not be intercepted, monitored, or recorded unless you agree to it. Your call can only be monitored and/or recorded if: • Everyone on the call agrees; or

• You hear a beep or warning tone every 15 seconds; or • Law enforcement or national defense agencies get special permission.

Any customer in California that provides its own terminal equipment and who monitors or records conversations over the public telephone network, must provide notice of the monitoring or recording by use of one of the methods listed above. We will discontinue service to a customer for noncompliance with this rule if, after five days written notice, the customer has not complied with these requirements. Service will be restored after the customer complies with these rules and pays the reconnection charge. Although monitoring rules are different for telephone companies, we view your right to privacy very seriously. By law, telephone companies may monitor customer communications without notice as long as the contents of the call are not recorded or transcribed.
