Changes In Service We will take your request for service changes, including moves and disconnects, over the telephone, provided that your CPNI password is on file at Nushagak Cooperative. Also, you may send your request by fax or by email to csr@ nushagak.coop . *See more information about CPNI on page 8.
Deposit Requirements Nushagak Cooperative may require a security deposit on any account. Adeposit may be waived with a good credit reference from another telephone or electric utility. Credit must reflect that you have had utility service within the last five years for a consecutive two-year period. You will be
New Service Nushagak Cooperative Inc. is owned by its members. When you begin service with us, you become a member/owner. We require all members to complete and sign a membership card. As a member/owner, you have a voice in your Cooperative. You will be able to vote at the Annual Meeting for Board Members, discuss issues and benefit from the services that Nushagak Cooperative Inc. brings to you at the lowest possible cost. Installation Scheduling Requests for telephone service received by 12-noon will be completed on or before 12-noon of the following business day, providing no premise visit is required. Allow two to five business days for telephone, Internet or cable television connects that require a visit to your premises, and additional time for new construction. Such requests are scheduled in the order they are received.

Lifeline & Linkup Do you need a phone? Are you having trouble paying your telephone bills? If so, you may be eligible to take advantage of two special programs that help reduce the cost of phone service. Lifeline Assistance and Linkup can assist qualified members to receive phone service and pay their bills. Lifeline and Linkup assistance is provided to Nushagak Cooperative’s low-income residential members who meet the following criteria for assistance. The applicant must be a member with Nushagak Cooperative with telephone service and must reside at the location for which the telephone service is provided. The assistance applies to single line residential service only. The applicant must meet the eligibility criteria established by the FCC and RCA to qualify for such support. The qualifying Nushagak Cooperative member will complete and sign an application form that specifies the program that he/she is receiving that qualifies them for the Lifeline & Linkup program. The Nushagak member also agrees to notify Nushagak Coop- erative if he/she ceases to participate in the qualifying program or programs. Nushagak reserves the right to verify or request verification of participation in the qualifying program or programs the member designates.
You may be qualified if your household income is no more than 135 percent of the federal poverty income guidelines or if you participate in any of the following programs: • Medicaid • Food Stamp - Supplemental Nutrition Assistance Program (SNAP) • Supplemental Security Income (SSI) • Federal Public Housing Assistance (FPHA) • Veterans and Survivors Pension Benefits • Bureau of Indian Affairs General Assistance • Tribally Administered Temporary Assistance for Needy Families (Tribal TANF) • Food Distribution Program on Indian Reservations (FDPIR) • Tribal Head Start (only households that meet the income qualifying standard) Lifeline Assistance will help qualified members who already have telephone service save a significant amount of money on their monthly phone bills, while Linkup helps eligible members receive a generous discount on the installation of telephone service in their homes. Also, consumers can voluntarily forego presubscribed long distance telephone service by requesting toll-blocking, a service that prevents toll calls (such as long distance) from being made and prevents these consumers from paying hefty service deposits. Members using this service can still use pre-paid calling cards to place long distance calls from their homes. Lifeline support is due to end December 2022.

Returned Checks Returned Checks may place your telephone service in jeopardy of interruption. All accounts with returned checks will be assessed a $25.00 charge for each returned check. Restoring Service To restore service, you will be required to pay the outstanding balance in full, pay a deposit, as well as a service fee. Your Telephone Bill Telephone service is billed 30 days in advance. Your first bill includes a full 30 days of monthly charges in addition to a prorated amount equal to the days of service prior to the billing date. Your bill will also include installation charges. These are one-time charges for the connection of service. This will appear on your first bill only. Subsequent bills will include only the regular monthly billing. Billing Statements are mailed by the 5th of each month. Payment must reach our Customer Service Office no later than 5 p.m. on the 20th of each month to keep your account in good standing. Disconnect For Non-Payment If a payment is not received by the due date, your service is subject to interruption. A disconnect notice will be mailed on the 10th of each month to members with an unpaid balance.

Telephone Wiring & Equipment Repair
Troubleshooting To save time and costly repair charges on single line residen- tial or business service, we encourage you to attempt to isolate problems before calling for repairs. The following guide- lines may be of assistance: 1. Check each instrument to be sure that a receiver is not off the hook. 2. Be sure that all jacks are securely in their outlets and that there are no loose connections or exposed or frayed wires. 3. To determine whether you may have an isolated inter- nal wiring problem, take a working telephone to each outlet and test for dial tone. 4. If you still have not determined the problem, and your home is so equipped, take a known working telephone outside to your telephone network interface device (where the telephone wire enters your building). Remove the existing plug-in from the connector block and insert the plug on the cord of your mod- ular telephone into the outlet. If you hear a dial tone from your telephone, you may assume that the problem is with your inside wiring. If there is no dial tone, it is probably an external problem in the Nushagak Cooperative telephone line. If the problem is the Nushagak Cooperative line, there will be no charge for the repair.
Federal deregulation of inside wire and jacks requires members to assume responsibility for the maintenance and repair of all “inside wiring.” Inside wiring includes any telephone wiring, either inside or outside of your home or business, which runs from the telephone network interface device and is usually located outside of your building if a single family home or single business location, and inside of your building if a multi-family dwelling or multi- business location, such as an office building. Telephone Wiring Options for Your Home or Business • Call on our Experience - Nushagak Cooperative will install or repair inside wiring and jacks for a fee. • Hire an independent contractor. • Do the Work Yourself - You may provide your own wiring and jacks. Inside wiring and jack kits are available at Nushagak Cooperative.

Your Rights & Responsibilities Tariff Information Nushagak Cooperative’s operating tariffs are the rates, rules and regulations by which we operate. These tariffs are available for inspection at Nushagak Cooperative’s Members Service office, located at 557 Kenny Wren Road, Dillingham, Alaska or online at nushtel.com . Let Us Know If You Are Not Satisfied Taking care of our members is our primary concern. Your customer service representative will answer your questions and resolve any problems that you may have. If your customer service representative is unable to answer your questions, they will direct you to a Nushagak Cooperative employee who will assist you. Your Privacy Is Protected Nushagak Cooperative protects the privacy of both your communications and account records. Unlawfully wiretapping a telephone is a crime under federal and state laws. Anyone who wiretaps or otherwise intercepts a telephone call is subject to prosecution. Nushagak Cooperative does not install or provide the equipment necessary for intercepting communication. However, when we receive a properly issued court order, we are required by law to cooperate with the appropriate law enforcement agency. Be Careful Who You Let Use Your Telephone If telephone service is in your name, you are responsible for all calls made from your telephone, including long distance calls, regardless of who places them. Also keep your calling cards in a safe place where other unauthorized users cannot place calls with your card. Use Your Telephone For The Proper Type of Service Residence telephones are installed with the understanding that they will be used for normal social or domestic purposes. A residence
telephone will be changed to business service if used primarily or substantially for business purposes or if the residence telephone number is advertised in connection with sale of products or services. Use Your Telephone For Lawful Purposes Only Your telephone may be used for legal purposes only and may not be used in a way that interferes with the telephone service of others. Equipment connected to Nushagak Cooperative lines must be in compliance with Federal Communications Commission regulations. Annual CPNI Notice Nushagak Cooperative respects the privacy of our members and our employees do everything possible to protect information that may be found in your account records. The Federal Communications Commission developed new rules in 2007 that require certain steps be taken to protect Customer Proprietary Network Information (CPNI). If requested, Nushagak Cooperative will provide account information to our members in one of the following ways: 1) We will verify the password you provided to us when you call with questions about your CPNI. 2) We may call the telephone number of record and provide the account information; 3) We may mail the information to the address of record; or 4) We may provide the information at our office if we are presented with valid photo identification and the individual so identified is on the account. We do not anticipate that these new rules will interfere with quality member service. If you wish to receive additional information regarding CPNI rules and how they are implemented in Nushagak Cooperative, please call or stop by our office.

between the company and the consumer on the basis of a purchase or transaction regarding products or services within eighteen (18) months immediately preceding a telephone solicitation, and which has not been previously terminated by either the consumer or the company. Such an established business relationship also exists on the basis of a consumer inquiry regarding a company’s products or services within three (3) months immediately preceding the date of a telephone solicitation, and which has not been previously terminated by either the consumer or the company. Handling Nuisance or Obscene Telephone Calls Remember, you are in control of your telephone. Use the telephone on your terms and do not talk to anyone unless you want to. Do not answer personal questions or give your name or telephone number to any unidentified caller. Teach your children to say, “Mom or Dad can’t come to the telephone. May I take a message?” If problem calls persist, contact the police department. If you receive threatening calls, contact your local law enforcement agency immediately.
Do-Not-Call List The Federal Communications Commission (FCC) and Federal Trade Commission (FTC) are now sharing responsibility for enforcing the National Do-Not-Call list. The FCC now requires all telecommunications carriers that provide Local Exchange Service to notify their subscribers of their rights as they relate to the Do-Not-Call rules. You have the right to give or revoke an objection to receiving telephone solicitations. To either register or revoke your registration, please call 1-888-382-1222 (TTY 1-866-290-4236) or register via the Internet at www.donotcall.gov . The national registry prevents telephone solicitation made for commercial purposes only, and will not prevent telephone solicitations made by a tax-exempt nonprofit organization, for example, for charitable or political purposes. Also, the registry will not prevent calls from companies with whom you have an established business relationship. The FCC has defined an established business relationship as one in which the consumer has “a prior or existing relationship” with a company that is formed voluntarily through communications

5. If you have purchased from the caller within the last 24 months and have not asked in writing that the calls be stopped. The statute defines two instances where telephone solicitation is unwanted: 1. If you are Registered on the Do-Not-Call website as not wishing to receive telephone solicitations and you receive a call. If the call is made using an automated or recorded message to advertise or solicit over the telephone. What You Can Do The Federal Trade Commission (FTC) has implemented a National Do-Not-Call Registry. This registry gives you a choice of whether or not to receive telemarketing calls at home. Register on-line today at www.donotcall.gov . A Word of Caution Even though a member may take the appropriate steps and request that their number be added to the Do-Not-Call List, the member may still receive unwanted telephone solicitations. Nushagak Cooperative is not able to block the calls with our equipment. The only way calls are prevented is if they are not made. Telephone solicitors are required by law to obtain a list of all subscribers who do not wish to be contacted. 2.
Prosecuting Fraudulent Callers It is illegal for another person to charge long distance calls to your telephone number or calling card without your permission. People using unauthorized calling cards or telephone numbers to avoid charges are subject to prosecution and Alaska statute, AS45.50.475, adds unwanted telephone solicitations to residential telephone members to the list of unlawful acts and practices in the conduct of trade or commerce. Telephone solicitation is defined as “the solicitation by a person by telephone of a customer at the residence of the member for the purpose of encouraging the customer to purchase property, goods, or services, or make a donation.” The following calls do not fall under the definition of solicitation: 1. If you asked the caller to call you. 2. If the call is made by or on the behalf of a charitable organization that you have made a donation to (or expressed an interest in making a donation) during the last 24 months. If the caller is only asking for your ideas, opinions or vote. may be imprisoned, fined or both. Unwanted Telephone Solicitation 3.
If a telemarketer requests a list, Nushagak Cooperative will direct the telemarketer to the Federal Do-Not-Call website. Nushagak Cooperative does not have the authority to enforce this legislation.

Communications Commission does not offer interference protection to cordless telephones, you should contact your dealer or manufacturer for assistance. Protect Yourself Using Telephones During Electrical Storms You should avoid using a telephone during an electrical storm in your immediate area. Calls of an urgent nature should be brief. Although Nushagak Cooperative uses protective measures to limit abnormal electrical surges from entering your home, absolute protection is impossible. There is a remote risk of a dangerous electrical shock from lightning when using the telephone during a nearby electrical storm.
Telephone Interference Occasionally, you may be bothered by noise interference on your telephone line. This may be caused by one or more conditions over which Nushagak Cooperative has no control of, i.e. weather conditions, aerial satellites or poor grounding conditions. Usually this is temporary and will rectify itself. Radio frequency signals of nearby radio transmitters, such as amateur, citizens band or AM/FM broadcast stations, may also cause interference by entering your unshielded or unfiltered telephone equipment. Should this happen, first disconnect any telephone attachments, such as a message recorder or hearing device. If the interference disappears, reconnect the attachments one at a time until you hear the noise again to determine which attachment needs modification. Contact the dealer or manufacturer for assistance. If the interference is still present without the attachments, you may be helped by the installation of a modular filter, obtainable from Nushagak Cooperative and most stores selling telephone equipment, or by using a different telephone instrument. Cordless telephones operate on radio fre- quencies which may receive interference from nearby radio transmitters. Since the Federal
Using Telephones Near Water Telephones should not be used while you are in a bathtub, shower or pool. Immersion of a telephone or handset in water could cause an electrical shock. Credit For Lost Service If you have reported to our repair service that you are unable to make or receive calls, and your telephone is out of service for an additional 24 hours or more after you reported the problem, you may be entitled to a credit on your monthly service charge. Call Waiting This feature alerts that another call is waiting. You hear a beep tone. If you ignore the first beep, there will be a second one in ten seconds. The party calling hears only ringing. How to Operate: 1. If you hear a beep and want to end the first call, hang up. Your telephone will immediately ring the second call. To answer the second call, press and release the receiver button quickly. The first call will then be on “hold” and you will be connected to the second call. To return to the first call, press and release the receiver button again. You may switch back and forth between the two calls as you wish. 2. 3.
Home Intercom With Home Intercom, no matter where you are – in the house, garage, or barn – you may reach anyone near an extension telephone by dialing your own home telephone number. How To Operate: 1. Lift the handset and listen for a dial tone. Dial your own telephone number. A busy signal will tell you that you have dialed your own number. Once you have heard the busy signal, hang up. The telephone will ring and when the ringing stops (meaning an extension has answered), pick up your receiver. You are connected to the party that answered the extension. Notes: When you hang up the telephone, Home Intercom is disconnected. 2.

5. Call Waiting is disabled for the duration of the call and is automatically restored when the call is terminated. Call Forwarding This service allows you to transfer your incoming calls to any telephone that you may direct-dial without operator assistance. How to Operate: 1. To forward calls from rotary dial telephones, dial 1172 or *72 for push-button phones. Immediately dial the number that will be accepting your calls. Call Forwarding is in effect when the party answers. If the line is busy, or if there is no answer, hang up and repeat the first two steps. No answer is required to establish Call Forwarding on the second attempt. To cancel Call Forwarding from a rotary dial telephone, dial 1173 or *73 for a push-button phone. Wait 4 seconds for two beeps and then hang up. Please see chart to activate and deactivate calling feature. 2. 3. 4. 5.
Cancel Call Waiting This service allows you to disable Call Waiting for the duration of one call. How to Operate: This feature provides two methods to disable Call Waiting: Prior to Originating a Call 1. Listen for a dial tone. Press *70 for push-button telephones or dial 1170 for rotary dial telephones. Dial the number of the person to whom you wish to talk without Call Waiting in effect. For the duration of this call, Call Waiting is disabled. Upon termination of the call, Call Waiting will automatically be restored. Press and release the receiver button quickly to put your call on “hold.” Listen for the dial tone. 2. Press *70 for push-button telephones or dial 1170 for rotary dial telephones. 3. Listen for a confirmation tone informing you that your request to disable Call Waiting has been honored. Three-Way Calling This service allows you to add a third party to your existing conversation without operator assistance. Three-Way Calling may be used whether you have placed or received the first call. How to Operate: 1. Press and release the receiver button quickly to put your call on “hold.” You will hear three beeps then a dial tone. Dial the third person’s number. You may talk privately before including the “holding” party. To add the “holding” party, press and release the receiver button. All three parties will be on the line (if there is a busy signal or no answer you may return to the original call by pressing and releasing the receiver button). While your calls are being forwarded, your telephone will ring briefly each time your number is dialed. You cannot answer the calls, but the ring is a reminder that Call Forwarding is still in effect. While calls are being forwarded, you may use your telephone for outgoing calls. Speed Calling You can select either a short list of eight telephone numbers (Speed Calling 8) or a long list of 30 telephone numbers (Speed Calling 30). How to Operate: 1. For push-button telephones, press *75 or for rotary phones dial 1175 and wait 4 seconds for the dial tone. For one-digit codes press *74 or 1174 for rotary phones. 2. Select a one-digit code (use numbers 2 through 9) for Speed Calling 8. Select a two-digit code (use numbers 20 through 49) for Speed Calling 30. 3. Dial the code you have selected and the complete telephone number as you would normally place the call, including the area code, if applicable. At this point, for push-button telephones, press the # symbol. For rotary dial telephones, you will hear a 4-second pause. Two beeps will confirm your entry. 4.
To call numbers selected for Speed Calling, simply dial the one- and two- digit code that you have chosen, then, if you have a push-button telephone, press the # symbol. If you have a rotary dial telephone, you will hear a 4 second pause before the call is connected. To change an existing Speed Calling number, follow steps 1-4 to reprogram. Notes: You will need to keep a list of your Speed Calling numbers. Nushagak Cooperative does not have this information. Teen Line This feature allows a station to have two directory numbers on the same single- party line so that a subscriber may receive calls dialed to separate numbers without installing a second line. The subscriber is issued a Primary Directory Number (PDN), identified by normal ringing, and a Secondary Directory Number (SDN), identified by distinctive ringing. Calls originate from the PDN only; calls may terminate to either the PDN or to the SDN. All billing is done to the PDN . 5.

Business members may have their primary number listed once in the alphabetical section of the directory and once in the Yellow Pages, without charge. Your listing will be placed in the Yellow Pages under a classified heading of your choice. Additional business listings that may assist customers in finding your business number are available at a low monthly charge. Non-Listed Service For a low monthly charge, we will remove your listing from the directory but maintain it in our directory assistance records. Non-Published Service We will omit your listing from both the directory and directory assistance records. This service is available for a low monthly charge. Errors or Omissions Every effort is made to ensure that the directory is accurate. If there is an error in your listing, contact your Customer Service Representative. Nushagak Cooperative’s liability for non-charged listings is limited to correction of the error in the directory assistance records and in the next publication of the directory.
Allows only 800 + calling card toll calls to be made from your telephone. Any outgoing calls must be made by dialing “0” plus the number. The operator will only complete calls billed collect, billed to a calling card or to another telephone number. Any other outgoing toll call attempt will be routed to a busy signal. Toll restriction does not prevent collect calls or toll calls charged to your telephone number by a third party. “900” Number Restriction This restriction prevents access to “900” telephone service (pay-per-call service) from your local telephone line. If a 900 call is attempted from your telephone, the calling party will hear a recording. Collect & Third Party Call Blocking This service will alert long distance operators outside of Dillingham that you do not wish to accept collect or third party charges to your Nushagak Cooperative number. Nushagak Cooperative cannot guarantee operators will not attempt these types of billings. Unauthorized calls may be disputed if billed. Busy Call Forwarding Whenever you’re on the phone, you can be sure your callers can talk with someone else or can leave a message for you. Easily turn it on or off, or change the forwarding number anytime, once you’ve asked the telephone company to activate this service. How to Operate: 1. To activate and deactivate, please see chart. 2. Press *90 or on rotary phone dial 1190 and listen for the dial tone. 3. Dial the number where you want your calls forwarded. When the phone is answered, your Call Forwarding is in effect. If the line is busy, or there is no answer, hang up. Within two minutes, repeat steps. You’ll hear a confirmation tone to let you know your Call Forwarding Busy is now working. To deactivate press *91 or on rotary phone dial 1191 . Class Features Note:
You will hear a stutter dial tone whenever one of these call features is activated on your phone. This is to remind you that one or more of these features is activated. Caller ID Nushagak Cooperative members can subscribe to Caller ID services which allows you to view the date, time, phone number and directory name (where available) of each incoming call on a Caller ID display box or a Caller ID phone. The Caller ID unit will include instructions on how to display the information desired. You must allow at least one ring before Caller ID information will be displayed. If “out of area” appears on the display unit, the caller is in an area that doesn’t support the service. Blocked calls will not display name and phone numbers of the caller and may be displayed as “Private Caller.”
Remote Call Forwarding To Activate: 1. Call the RACF directory number: 907-842-7206 . 2. Enter your own directory number and PIN when prompted (PIN number is the last 4 digits of your telephone number).

Call Waiting ID If you subscribe to Call Waiting and Caller ID, you can also enjoy the features of Call Waiting ID. Your Caller ID Unit will display the information of an incoming call while you are on the line with another caller, so you can decide whether or not to answer the second call. You may need to program your phone for this feature. Read the instructions included with your phone.
2. Your phone will keep dialing the number for up to 30 minutes and a special ring will notify you when the line is clear and your call is ready to go through. 3. Pick up the phone and the call will be automatically dialed. Automatic Redial This feature allows you to dial the number of the last incoming call that you DID NOT answer. Instructions: 1. The number will be automatically dialed. *Not available on Hunt Groups Call Blocking Per Call Call blocking is a standard feature on every Nushagak Cooperative line. You can activate this feature to prevent your phone number and name from displaying on a Caller ID unit. Instructions: 1. Please see chart to activate or deactivate. 2. Press *67 or 1167 on rotary phone and listen for confirmation tone. 3. Dial the phone number. Your name and number will be blocked and “private” will appear on the other person’s Caller ID unit. Call Blocking Per Line You must request this service from Nushagak Cooperative. Call blocking prevents your name & number from displaying on any Caller ID unit for all calls made. “Private Caller” will be displayed on other Caller ID units. To Deactivate for individual calls: 1. Press *82 or 1182 for rotary phones. 2. Call blocking returns after this call is made.
Automatic Callback This feature allows you to redial the number of the last outgoing call. If the number is busy, call setup will be attempted as soon as both parties are idle. A special ring will alert you when both lines are idle. As soon as you lift the handset, the call will be automatically dialed. Instructions: 1. Pick up the phone and dial *66 ( 1166 from a rotary phone).

Searching For Businesses From the main screen, simply enter the type of business or the name of the business that you are looking for in the “FIND” box. You will be directed to a page containing all of the businesses that match your search, as well as a map indicating where each business is located (if an address was provided) . By clicking on any advertiser’s address, you can get an exact location of the business on the map, as well as the option of obtaining driving directions from your own location! From the listing information on the left, you can also access a business’s ad as it appears in the Yellow Pages, access their website, or send them a quick email (if the advertiser has chosen these options) .
Searching For People From the main screen, click on “PEOPLE SEARCH” and simply enter the name of the person or people you are looking for in the “FIND” box.
