Establishing Service In order to establish telephone service, you will need to present the following information: 1. Complete legal and billing address, including street and number (apartment number, if applicable), route and box number, plus city and zip code. 2. The type of basic or optional monthly service desired. 3. Directory listing preferences. 4. Other credit information, including employment and social security number. Deposits The amount of the deposit to establish telephone service is determined by estimated two-month billing and other credit information. Service Charges New customers are assessed a one-time service connection charge, which includes the cost of processing the order and connecting your service to the network. The charges for work done at your residence are based on the actual amount of time needed to install, move or change the wiring in your home. The initial charge for the first 30 minutes of work includes travel and preparation time.
