Lifeline Broadband Service BBT understands the importance of con- sumers having access to quality Broadband service. To help consumers obtain and keep their service, BBT offers reduced rates to eligible consumers under a government assis- tance program called Lifeline. Lifeline provides monthly discounts to an eligible customer’s service. Basic services are offered to all consumers in the Company’s service territory at the rates, terms and conditions specified in the Company’s tariff on file with the Public Utility Commission of Texas. However, BBT would be pleased to provide you with spe- cific rates for your area upon request. If you have questions regarding BBT services or rates, please call 432-364-1000 or toll free 1-800-592-4781.

you, your dependent, or anyone else in your household must receive assistance from one of the following programs: Medicaid, Supplemental Nutrition Assistance Program (SNAP), Supplemental Security Income, Federal Public Housing Assistance or Section 8, Low-Income Home Energy Assistance Program, National School Lunch Program free, State Child Health Plan health benefits or Temporary Assistance for Needy Families (TANF). Your household may not receive the Lifeline benefit from more than one Broadband com- pany. For the purpose of Lifeline, a household is an individual or any group of individuals who live together at the same address and share income or expenses. Your Lifeline discount is not transferable to another person, even if he or she is eligible. If you violate the one- per-household rule or otherwise make false statements to receive Lifeline, you may lose your Lifeline benefit and may be prosecuted by the United States government. Availability of Lifeline Broadband Service

Lifeline is a federal benefit that makes monthly Broadband service more affordable for eligi- ble households. To receive Lifeline associated with your Broadband service from BBT, your total household income must be at or below 150% of the federal poverty guidelines, OR
