STATEMENT ON NONDISCRIMINATION Ardmore Telephone Company is the recipient of federal financial assistance from the U.S. Depa ment of Agriculture (USDA). The goal of universal services is to provide all citizens access to essential telecommunications services. This telephone service includes access to: • Single pa y touch-tone residential and business service • Long distance carriers • Emergency services • Operator services • Directory assistance • Telecommunications Relay Services • Other services designed for persons with disabilities • Toll Blocking Use of these services result in added charges in most cases. In addition, Ardmore Telephone Company provides one copy of its annual local directory without charge. Ardmore Telephone Company offers qualified customers Lifeline service. If you are a low-income customer you may qualify for the Lifeline program. This means you may receive a monthly discount for your basic phone charges. This service also includes a discount for line connection charges and free toll blocking for qualifying customers. Customer Proprietary Network Information Special Notice Regarding Your Account Information

Ardmore Telephone Company knows how impo ant personal privacy is to its members, so we treat as confidential ce ain information we have about you. A recent change in federal law pe aining to telecommunications companies applies to our use and disclosure of that information. To what kind of information are we referring? This information, called Customer Proprietary Network Information or CPNI, is information we acquire in the course of providing service to you. It includes the calling features, options and plans to which you subscribe, billing information and the identity of the long-distance carrier you have chosen. You have the right, and we have the duty under federal law, to preserve the confidentiality of this information. How is this information used now and how might it be used? We use this information today to market features, options and calling plans relating to the service you currently are receiving from us. It is never released to outside companies. If you consent, this information may be shared with our affiliates so they can market their telecommunications services to you. What do I need to do to give my consent? No action on your pa is necessary unless you wish to restrict Ardmore Telephone Company from sharing this information with its affiliated companies. If you wish to deny the use of your CPNI, please dial 256-423-2131 or 800-830-9946 to reach your local business office, or send an email to cpni@ardmore.net with your request within 30 days of receipt of this notice.
