The Volcano Blast App. The Volcano Blast app is an end-user tool available to Volcano customers that is designed to assist customers in man aging their accounts and to provide them with additional functionality. In order to provide its customers with the Volca no Blast app, Volcano has contracted with third party vendors. These third party vendors collect information from you after you install and use the app. However, the information collected is limited to information necessary to perform the business purposes of the app. Information collected from users of the Volcano Blast app includes: customer name, e-mail address, location, browser information and session cookies related to access and use of the app, IP address, in stalled Wi-Fi router device serial numbers, MAC address, and analytical information regarding use of the app. The infor mation collected is also used in aggregated and dis-aggregated formats to enhance the app or to provide app features. Use of Website Information. We use aggregate information to analyze our Site traffic, but we do not examine this information for individually identi fying information. We may use anonymous IP addresses to help diagnose problems with our server, administer our site, or display website content according to your preferences. Use of Information in Business Transfers. The law may permit us to disclose the location of a device on our network without a user’s consent to: (1) a user’s legal guardian or member of user’s immediate family; (2) database management services or information providers solely to assist in delivering emergency services; or (3) a government entity if we reasonably believe that an emergency involving immediate danger of death or serious physical injury to any person requires disclosure without delay. NETWORK AND INFORMATION SECURITY. We maintain a variety of physical, electronic, and procedural safeguards to prevent the loss, misuse, and alteration of the information that we obtain from you, but we cannot guarantee our ability to prevent loss, misuse, or alteration. GENERAL INFORMATION

Volcano Business Operations, Rules, & Regulations AVOIDANCE OF LAWFUL CHARGES Persons using unauthorized credit card numbers or unauthorized telephone numbers to avoid charges will be prosecuted. Anyone violating Section 502.7, California Penal Code, by attempting to avoid lawful charges for telephone service may be fined up to $5000 and imprisoned up to 5 years. NUMBER CHANGES

DIRECTORY ACCURACY The Company does not assume liability because of errors or omissions in compiling this directory. Di rectory listings are carefully handled in order to en sure directory accuracy, but in spite of this care and attention, errors occasionally occur. If your listing is incorrect in any way, please call the Business Office promptly so that our information records and fu ture issues of the directory may be corrected. Also, please promptly advise the Company of any other desired changes in your listing, such as a change of the house number or street, name, etc. RESPONSIBILITY FOR ADVERTISING Neither the Telephone Company nor the Directo ry Company will assume any liability for damages arising from errors or omissions in free listings. Its liability in the case of paid listings shall not exceed the charge for such listings during the life of this di rectory. Its liability in the case of advertising shall not exceed the charge for such advertising during the life of the directory. WHEN YOU HEAR A “BEEP” TONE A short “beep” tone heard on your telephone line about every 15 seconds could mean that the per son with whom you are talking to is recording your conversation. The signal is provided for your protec tion. Use of a recorder without recorder-connected equipment containing a tone-warning device is con trary to the Company’s tariffs and is not permitted. If you do not want a record made of what you are saying, ask the person with whom you are talking to disconnect the recording machine. When it is dis connected, you will no longer hear the “beep” tone. CUSTOMER-PROVIDED EQUIPMENT Customer-provided equipment may not be attached to or connected with telephone facilities unless (1) it is registered or otherwise approved by the Federal Communications Commission and/or the California Public Utilities Commission, (2) it meets the condi tions set forth in the Company’s tariffs, and (3) ad vance notification is given to the Telephone Compa ny, by the customer, of the type of equipment to be connected and the type of telephone facility required to make the connections. These rules and tariffs are designed to meet the convenience of the customer, and at the same time protect the telephone network against any possible hazards to customers and com pany employees. The Telephone Company will not maintain or repair customer-provided equipment. A service charge is applicable if the Telephone Company visits the premises of the customer, and the service difficulty or trouble is found to have been caused by the cus-

4. Extra Paid Classified Listings – where a busi ness has several lines of merchandise or ser vice which it wishes to publicize. For example, a building material firm will want to be listed under building supplies, lumber, paint, sash and doors, hardware, etc. LISTINGS NOT APPEARING IN THIS DIRECTORY Between directory issues, a great many telephones are installed. Customers may obtain telephone numbers not shown in this directory by dialing or calling Directory Assistance, where an up-to-date list of all subscribers is maintained. After asking Directory Assistance for a number that has been changed since the current directory was issued, write the new number in your directory immediately for future reference. It will expedite your service.

The Telephone Company reserves the right to and may, at its discretion, change the subscriber’s tele phone number. TELEPHONE DIRECTORIES This directory is the property of Volcano Telephone Company and is issued without charge for the sole and exclusive use of our subscribers, who are re quested to maintain it in its original condition, rea sonable wear and tear excepted. The subscribers shall not conceal the outside of the directory by placing a cover thereon or by other means. Insertion of unauthorized advertising or other matter by rub ber-stamping pages or inserting advertising sheets is expressly forbidden. One directory for each access line leased through this business office is furnished without charge. DIRECTORY LISTINGS For each subscriber, a listing is provided in the al phabetical section of the directory without addition al charge. In telephone directories having classified business sections, one additional business listing is also furnished without additional charge to each busi ness subscriber under standard business headings. Subscribers may arrange for extra listings by applying at or calling the Business Office. Business subscribers may arrange for additional listings both in the main and in the classified sections of the directory. Addi tional listings under various classified headings in the classified section will be helpful in stimulating addi tional business. ADDITIONAL DIRECTORY LISTINGS Additional directory listings are available for a small extra monthly charge. Here are some of the directo ry listings which may be of service to you. 1. Additional Residence Listings – for other mem bers of your household, especially those with dif ferent surnames. 2. Additional Business Listings – for the person who needs their own name in the directory, in ad dition to their firm’s name. 3. After-Hours Listings – to give direction where to call after-hours and on Sundays/holidays.

INFORMATION AND TRANSMISSION SERVICE

The Telephone Company assumes no liability for errors which may arise in imparting information, nor for any errors or delays in transmission over the in struments and lines of the Telephone Company. RESPONSIBILITY FOR LISTINGS The Company does not undertake to ascertain the right of any subscriber to use any title or business description in connection with their directory listing, and may reserve the right to change these listings at any time in accordance with any provisions that may be laid down by law concerning directory list ings and similar privileges.

Volcano Business Operations, Rules, & Regulations tomer-provided equipment or facilities. If you have any questions concerning the use of customer-provided equipment, please contact your Business office. WHEN TO PAY YOUR BILL

Basic telephone service, as defined by D.96-10-066, Appendix B, Page 5, will not be disconnected solely for nonpayment of 700, 900 or 976 charges. If you have disputed charges, you may be entitled to a credit. At no charge, residential customers can block such calls from being made from their phones. You must lodge your complaint within 60 days of receipt of the bill in dispute. To lodge a complaint, please call or write to the Business Office listed on the front of your bill. TARIFFS Tariffs, showing rates and rules for telephone ser vice and facilities, are on file with the California Public Utilities Commission. They are available in our Business Offices for public inspection. Volcano Telephone Customers are billed one month in advance. Rental charges vary according to loca tion and type of service, as authorized by the Cal ifornia Public Utilities Commission. Charges are payable to the date on which the company deems it necessary to terminate service or to the date on which the subscriber requests that the service is to be disconnected. DISCONNECTION NOTICE Written notice of our intent to disconnect telephone service is mailed to a customer at least seven days before the scheduled disconnect date. If payment has not been made by the specified date, the ser vice will be temporarily disconnected without further notice. There will be a service connection charge to restore the service, and a deposit may also be re quired. The company assumes no responsibility for undelivered notices. BUSINESS HOURS Business Office is open from 8 a.m. to 5:30 p.m. Monday through Friday (except holidays) and 8 a.m. to 4:30 p.m. on Saturday. ALLOWANCE FOR FAILURE OF SERVICE The Telephone Company does not guarantee unin terrupted working of its lines or equipment. In case service is interrupted, otherwise than by negligence or willful act of the subscriber, an adjustment will, upon application by a subscriber, be made in the amount of the charges for such of the service equip ment and facilities furnished and are rendered use less or inoperative. Any adjustment shall apply only to the period the interruption continues beyond 24 hours after the notice of the interruption is received TELEPHONE RENTAL AND ACCESS LINES

BILLING AND PAYMENT FOR SERVICE Volcano Telephone’s monthly bills for telephone ser vice are issued in advance, are payable in advance, and are due upon presentation. Long distance charges for the month preceding are included on the current bill. When paying in person at the Busi ness Office, it will save you time if you have your bill with you. When paying by a check, please enclose the payment stub with your check. If the stub has been mislaid, you can make certain that your ac count will be promptly credited by writing your tele phone number on the check. Payment can be made by Auto Payment through your checking account or credit card. Call us for these sign up forms. You may also pay in the office or by phone with Visa, MasterCard, or Discover. In the of fice, you can also use your bank debit card. You may also pay online with Volcano e-bill payment service. Log on to http://volcanotel.billtrust.com . If you desire additional assistance or explanation regarding your bill, please call the Business Office. Note: All rules and regulations in effect at time of printing are subject to change without notice. ANNOYING, OBSCENE AND THREATENING CALLS If you get an annoying, obscene, or threatening call, hang up immediately. If the calls persist, inform your telephone company business office. Threatening calls should be reported immediately to the police. Obscene or harassing calls are prohibited by feder al and state laws. A person who makes or permits such calls to be made over a telephone under their control may be fined or imprisoned, or both. CALIFORNIA LIFELINE PROGRAM California Lifeline Telephone Program is available to all Volcano Telephone Company residential custom ers who meet certain eligibility rules established by the California Public Utilities Commission. Native American residential customers who reside on federally-recognized tribal lands may be eligible to receive additional federal Lifeline Support in order to reduce the price for basic local telephone service. You are responsible for payment of authorized charges on your bill. Payment is due when you receive your bill. If this bill is not paid within 15 days from date of presentation, service may be discontinued. Failure to pay charges (i.e. basic flat-rate single-line service charges, including all mandated surcharges and tax es) may result in disconnection of telephone service. Basic Service is defined in D.96-10-066, Appendix B, Page 5. Other services, such as the ability to make toll calls, may be restricted if not paid. Optional services may be discontinued. If we disconnect your service for non-payment, you must pay the past due amount and a charge to reconnect your service. You may be required to pay a deposit. LATE PAYMENT CHARGE If a bill is not paid by the 15th day from the metered date, a 1.5% late payment charge will be applied to an unpaid balance of $20.00 or more.

When you call an 800, 888, 877, 866, 855, 844, 833, or 900 number, your number may be disclosed to the par ty you are calling and could be recorded in a da tabase sold to telemarketers. The California Public Utilities Commission wants customers to know that some companies using 800 numbers are now us ing Automatic Number Identification (ANI), which is approved by the FCC. When you dial their number, ANI equipment can automatically add your phone number to their customer database – and compa nies are not required to tell you if they have ANI. Companies with ANI can then use your phone num ber to get your address, income level, items pur chased, and other similar information from other marketing databases. Or they can sell your number to telemarketers, who can then pitch their products and services to you. Never assume that an 800 number is a free call. that the customer did not order. These types of charges are often unrelated to actual telephone usage. Slamming is defined as changing a consumer’s long distance carrier without the customer’s knowledge or consent. Volcano Telephone Company is required to process orders to change long distance carriers for other telecommunications providers. If your long distance provider is changed, you will see a note regarding the bill change and a change fee of $5.25 on your bill. In most cases, you will be gin to get a separate bill from your new long dis tance company. If you did not authorize the change, you may have been slammed. If you have been slammed, you should: • Call the company that slammed your service and advise them that you will not pay any charges incurred. • Contact us and explain that you did not authorize the change, and request to be changed back to your preferred long distance provider at no charge. • Contact your preferred long distance company and ask to be switched back. • You can confirm who your long distance carrier is by dialing toll-free 700-555-4141 from the tele phone you believe was slammed.

Outside the 209 Area, Dial 1, then the Area Code for the City you want then 555-1212, or dial “00” for your long distance company. (There is a charge for this service.) WHAT IF I REACH A WRONG NUMBER? Volcano Subscribers, Dial “00” right away and tell the Operator what happened. They will make sure that you are not charged for the call. WHAT IF I GET A POOR CONNECTION OR AM CUT OFF? Dial “00” immediately and tell them. They’ll see that you get a good connection, and that you are charged only for the time you’ve been able to talk and hear satisfactorily. REPAIRING YOUR WIRES

With Maintenance Inside Your Home Effective November 1, 1990, the California Public Utilities Commission ordered that repair of simple Inside Wiring Maintenance would be regulated. The Customer still has four options: 1. Do the work themselves. 2. Hire Volcano Telephone to perform the repairs. 3. Purchase Maintenance Plan from Volcano Telephone. 4. Hire someone else to do the work. The customer will be responsible for any interruption of service caused by faulty inside wire. Volcano Telephone will offer Maintenance under the following options.

Option 1 – Maintenance Program (Applicable to single-line services only)

Maintenance Service Agreement – For a small monthly fee (billed on your telephone bill) we will maintain your existing wiring, protecting you from any unexpected repair bills. Maintenance Service Agreement rates are as follows (Rates do not cover charges for installation, obvious neglect, will ful misuse, or reinstallation of inside wire where the wiring was destroyed by fire, major flood, or similar catastrophes.):
