FEDERAL COMMUNICATIONS COMMISSION (FCC) Common Carrier Bureau, Consumer Complaints Mail Stop Code 1600A2 2025 M St NW Washington, DC 20554 800, 888, 877, 866, 855, 844, 833 AND 900 TELEPHONE NUMBERS YOUR NUMBER MAY BE DISCLOSED. When you call an 800, 888, 877, 866, 855, 844, 833, or 900 num ber, your number may be disclosed to the party you are calling and could be recorded in a database sold to telemarketers. The California Public Utilities Com mission wants customers to know that some com panies using 800 numbers are now using Automatic Number Identification (ANI), which is approved by the FCC. When you dial their number, ANI equipment can automatically add your phone number to their cus tomer database – and companies are not required to tell you if they have ANI. Companies with ANI can then use your phone number to get your address, in come level, items purchased, and other similar infor mation from other marketing databases. Or they can sell your number to telemarketers, who can then pitch their products and services to you. Never assume that an 800 number is a free call. If the number uses ANI, you may reach a recording advising that you will be called back collect. Or you may be told to call a 900 number. In either case, the call will be billed to you. If a person answers an 800 number, you can advise them that you do not want your num ber, name, or address kept in the company records. You may want to make it clear that you do not want this information rented or sold to other companies. You may also advise them that you do not want the company to solicit future business from you. To com plain about a company using ANI, write to: OFFICE OF THE ATTORNEY GENERAL Public Inquiry Unit PO Box 944255 Sacramento, CA 94244-5225 Or Call 800-952-5225 You may also contact the: FEDERAL COMMUNICATIONS COMMISSION 1919 M St NW Washington, DC 20522 If you suspect that your name is on a direct marketing list and want it removed, write to: TELEPHONE PREFERENCE SERVICE c/o Direct Marketing Association PO Box 9008 Farmingdale, NY 11735

or items that the customer did not order. These types of charges are often unrelated to actual telephone usage. Slamming is defined as changing a consumer’s long distance carrier without the customer’s knowledge or consent. Volcano Telephone Company is required to process orders to change long distance carriers for other telecommunications providers. If your long distance provider is changed, you will see a note regarding the bill change and a change fee of $5.25 on your bill. In most cases, you will begin to get a separate bill from your new long distance company. If you did not authorize the change, you may have been slammed. If you have been slammed, you should: • Call the company that slammed your service and advise them that you will not pay any charges in curred. • Contact us and explain that you did not authorize the change, and request to be changed back to your preferred long distance provider at no charge. • Contact your preferred long distance company and ask to be switched back. • You can confirm who your long distance carrier is by dialing toll-free 700-555-4141 from the tele phone you believe was slammed.
